Özet

HLA-B27 Ankilozan Spondilit (AS) ile sık olarak birliktedir ve HLA-B27 antijeni Ankilozan spondolitin önemli bir genetik markındır. HLA-B27 rutin olarak serolojik metotlarla tiplendirilmektedir. Ancak yalancı pozitif neticeler bildirilmiştir. Bu yüzden HLA-B27 genotiplemesi için daha güvenilir birçok moleküler metotlar geliştirilmiştir. Biz bu çalışmamızda HLA-B27 saptanması için Real Time PCR metodunu araştırdık ve bu metotla akım sitometri ve konvansiyonel SSP-PCR metotlarını karşılaştırdık. Çalışmamızda SSP-PCR rutin HLA tiplemesi yapılan 60 olguyu (33 olgu HLA-B27 pozitif, 27 olgu HLA-B27 negatif) bu metotlarla araştırdık. Akım sitometri ile 3 olguda yalancı pozitiflik saptanırken yalancı negatif olgu görülmedi. Real Time PCR da 4 yalancı pozitif olgu görülürken hiç yalancı pozitif olgu saptanmadı. Akım sitometri ile HLA-B27 testinin spesifitesi % 89 sensitivitesi % 100 bulunmuştur. Real Time PCR ile HLA-B27 testinin spesifitesi % 100, sensivitesi % 88 saptanmıştır. Bulgularımız Real Time PCR testinin HLA-B27 tiplendirilmesinde rutinde güvenli olarak kullanılabileceğini ve akım sitometri ile HLA-B27' si şüpheli pozitif olgularda kullanılması gerektiğini göstermiştir. HLA-B27 saptanmasında Real Time PCR metodu laboratuardaki büyük sayıdaki örneklerin rutin analizinde güvenle kullanılabilir. HLA-B27 is strongly associated with ankylosing spondylitis (AS) and The HLA-B27 antigen is an important genetic marker in AS. As typing for HLA-B27 is routinely performed by serological methods, false-positive results can be generated. Therefore, several more accurate molecular methods have been developed for HLA-B27 genotyping. We described a real-time PCR method for the detection of the HLA-B27 and this method were compared with a flow cytometry and conventional SSP-PCR methods. We tested this methods on 60 subjects (33 HLA-B27 positive and 27 HLA-B27 negative) who underwent routine HLA typing testing by SSP-PCR. 3 false positive and no negative results were observed by flow cytometry. 4 false negative and no false positive results were detected by Real Time PCR. The specificity and sensitivity of HLA-B27 test with flow cytometry were 89 % and 100 %. The specificity and sensitivity of HLA-B27 test with Real Time PCR were 100 % and 88 %. Our results suggest that; Real Time PCR test for HLA-B27 typing can be used safely in routine practice and it should be used for doubt positive cases by detected flow cytometry as HLA-B27 positive. Real Time-PCR method for the detection of HLA-B27 can be used safely for the routine analysis of large numbers of samples in the laboratory.
