Özet
VÖZETBu tezde hasta ile hekim arasındaki geleneksel otoriter ilişkinin, toplumsaldeğişimlerden ne derece etkilendiği incelenmiştir. Toplumsal roller ve kurumsalyapılar bireylerin demokratik talepleri doğrultusunda değişmektedir. Bu değişimhasta ile hekim arasında ilk çağlardan beri var olan bilgiye sahip hekim ve onunotoritesi karşısında daha güçsüz bir konumda olan hasta birey ilişkisinde degörülmektedir.İleri teknoloji ülkelerin de yaşanmış olan bir dönüşümün eşiğinde olduğumuzsöylenebilir. Bilgi tekeli zayıfladıkça bilgiye ulaşım herkes için mümkün oldukçahasta ile hekim arasında ki otoriter ilişki, eşitler arası iletişime dönüşecektir.Bu çalışmada bir kamu hastanesinde çalışan hekimlerin ve muayene olanhastaların bakış açısından tedavi konusunda ?karar alma mekanizması? ve ?katılımcıkarar alma? irdelenmiştir. Hasta ile hekim arasında ki ilişkinin niteliğini her şeydendaha çok karar alma mekanizmasının belirlediği düşünülmektedir. Bu amaçla 55doktor ve 126 hasta üzerinde anket uygulanmıştır.Bu çalışma hasta ile hekim arasında ki ilişkide henüz yaşanmamış dönüşümütespit etmek için oldukça isabetli olmuşsa da, var olduğunu düşündüğüm dönüşümünhenüz çok başında olduğumuzu görmek biraz hayal kırıklığı yaratmıştır. Anketuygulanan hastalardan %85,5'i ?tedavi seçenekleri konusunda kararı ve sorumluluğuhekime bırakmayı tercih ederim? derken % 57,9'u tedaviyi seçme, reddetme vedurdurma hakları olduğunu düşünmektedirler. Fakat bu sorularla yapılacak 10 yılsonraki bir çalışmaya değişimin yansıyacağı kanaatimdeyim.

VISUMMARYThis thesis analyzes the basic effects of the social changes on the traditionalpatient-physician authoritarian relationship. Social roles and institutional structureshave been changing in accordance with the democratic demands of the individuals.This principle change can also be seen in the relationship between physician who hasthe knowledge of the long historical patient-physician relationship and patient whohas mostly a weaker position before the authority of physician.Alvin Toffler summarizes the relationship between knowledge and power insuch a way mentioned above. This condition has not been valid for our country yet.Nevertheless, it can be said that our country is close to be faced with thetransformation which had occurred in the advanced technology countries. Thepatient-physician communication will get become a communication among the equalsubjects as far as the monopoly over knowledge gets weaken and availability ofknowledge gets become easier.This study analyzes the ?decision-making mechanism? and the ?participantdecision-making? about treatment through the eyes of the physicians who work for apublic hospital and the patients who take treatment from those physicians. Havinganalyzed this process, it has yielded that the ?decision-making mechanism? is thedominant factor affecting the character of the patient-physician relationship. For thisaim, the survey method has been applied on 55 physicians and 126 patients.Although this study has been very important and useful to prove that thedemocratic transformation on the patient-physician relationship has not occurred yetin our country, it has been disappointing to see the transformation is still far awaythan I guess. According to the results of the survey, 85,5% of the patients say ?Iprefer to leave the decision and the responsibility about treatment to the physician?;and only 57,9% of the patients claim their rights to choose, refuse or halt treatment.However, I believe that the transformation will reflect to another survey which willbe applied ten years later with the same questions.
