Özet

Amaç: Çalışmamızın amacı F-18 FDG PET/BT'nin rektum kanseri evrelemesinde histopatolojik ve prognostik etkilerini ortaya koyarak tedavi öncesi preoperatif evrelemede yerini saptamaktır. Gereç-Yöntem: Bu retrospektif klinik çalışmaya arşivimizde Ocak 2010-Eylül 2014 tarihleri arasında rektum kanseri tanısıyla evreleme amaçlı F18-FDG PET/BT görüntüleme yapılan, ardından neoadjuvan tedavi verildikten sonra (90 hasta) ya da neoadjuvan tedavi verilmeden (25 hasta) opere olan 115 hasta (39 kadın, 76 erkek; ortalama yaş: 58.7 ± 11.4) dahil edildi. Hastaların PET/BT çalışmalarında rektum tümörü ve lenf nodlarındaki maksimum standardize tutulum değerleri (SUVmax) hesaplandı. Rektum kanseri TNM 2010 evresine göre; 11 hasta (%10) evre 1, 81 hasta (%70) evre 2-3 (lokal ileri) iken 23 hasta (%20) evre 4 (metastatik) saptanmıştı. Tümör seviyesine göre 32 (%28) hastada rektosigmoid, 49 (%43) hastada proksimal rektum ve 34 (%29) hastada distal rektum tümörü bulundu. Hastalar tedavi sonrası minimum 12 ay (12-75 ay) takip edildi. Bulgular: Hastaların tümünde primer tümöre ait artmış FDG tutulumu gösterilmiştir. Çalışmamızda metastatik lenf nodu saptanmasında SUVmax değeri 1.7 cut-off alındığında; F-18 FDG PET/BT'nin doğruluk, duyarlılık ve özgüllüğü sırasıyla %67.8, %76.9 ve %63.1 bulunmuştur. Primer tümör SUVmax değeri ile hastalık nüks-progresyonu ya da sağkalım arasında ilişki saptanmamış olup primer tümör SUVmax değerinin belirgin prognostik faktör olmadığı düşünülmüştür. Hastaların TNM 2010 evre grupları ile artmış FDG tutulumu gösteren lenf nodu SUVmax değerleri arasında ilişki saptanmıştır. Ayrıca nüks-progresyon ve sağkalımı öngörmede lenf nodu SUVmax değeri incelenmiş ve sınırda anlamlılık bulunmuştur. Venöz invazyon bulunması hastalık nüks-progresyonu açısından risk faktörü olarak saptanmış olmasına rağmen genel sağkalım ile ilişkisi bulunamamıştır. Lenfatik ve perinöral invazyonun hastalık nüks-progresyonu ya da sağkalım ile ilişkisi saptanmamıştır. En geniş primer tümör boyutu hastalık nüks-progresyonu açısından prognostik faktör olarak saptanmış olmasına rağmen genel sağkalım ile ilişkisi bulunamamıştır. Nodal evre hastalık nüks-progresyonu ve sağkalım üzerine etkili prognostik bir faktör olarak saptanmıştır. F-18 FDG PET/BT'nin metastatik lenf nodlarını saptayarak nodal evrelemeye ve prognoza katkısı olacağı görülmektedir. Çalışmamızda toplam 90 hastaya neoadjuvan tedavi verilmiş olup bu hasta grubundaki operasyon sonrası histopatolojik tümör regresyon skorları ile hastalık nüks-progresyonu ve sağkalım ilişkisine bakıldığında neoadjuvan tedaviye kötü yanıt veren hastalarda hastalık nüks-progresyon oranı daha yüksek saptanırken çalışmamızdaki hasta grubunda sağkalım ile sınırda ilişki saptanmıştır. Sonuç: F-18 FDG PET/BT rektum kanserinde primer tümör ve metastaxlarının saptanmasında etkili bir görüntüleme yöntemi olup lenf nodu SUVmax değerlerinin prognostik değerinin bulunduğu ve rektum kanserinin tedavi öncesi preoperatif evrelemede hastaların tedavi yönetimine önemli katkılar sağlayacağı kanısına varılmıştır.

Purpose: To establish the role of F-18 FDG PET/CT in the preoperative staging of rectal cancer by demonstrating its histopathological and prognostic effects. Material - Method: One hundred and fifteen patients (39 females, 76 males; mean age: 58.7 ± 11.4) from our archive that underwent F18-FDG PET/CT imaging for the staging of rectal cancer that were operated after being delivered neoadjuvant treatment (90 patients) or without being delivered neoadjuvant treatment (25 patients) between January 2010 and September 2014 were included in this clinical retrospective study. The maximum standardized uptake values of the rectal tumor and lymph nodes on the PET/CT studies were calculated. According to the rectal cancer TNM 2010 staging system, 11 patients (10%) were stage 1, 81 patients (70%) were stage 2-3 (local advanced) and 23 patients (20%) were stage 4 (metastatic). According to the tumor level, 32 patients (28%) had rectosigmoid tumors, 49 (43%) had proximal rectum tumors and 34 (29%) had distal rectum tumors. The patients were followed-up for a minimum of 12 months (12-75 months) after treatment. Results: Increased FDG activity belonging to the primary tumor was shown in all of the patients. When the SUVmax cut-off value for the detection of metastatic lymph nodes was taken as 1.7; the accuracy, sensitivity and specificity of F-18 FDG PET/CT was calculated as 67.8%, 76.9% and 63.1% respectively. No association was detected between the SUVmax value of the primary tumor and the relapse-progression of the disease or survival, and the SUVmax value of the primary tumor was not considered to be a significant prognostic factor. An association was detected between the TNM 2010 stage groups and the increased SUVmax values of lymph nodes that demonstrated increased FDG activity. Additionally, the SUVmax values were investigated for predicting relapse-progression and survival and a borderline significance was identified. Although the presence of venous invasion was identified as a risk factor for disease relapse-progression, it had no association with general survival. No association was detected between lymphatic and perineural invasion and disease relapse-progression or survival. Although the largest primary tumor size was identified as a prognostic factor, it had no association with general survival. Nodal stage disease was identified as a prognostic factor for relapse-progression and survival. It is clear that F-18 FDG PET/CT will contribute to nodal staging and prognosis through the detection of metastatic lymph nodes. A total of 90 patients were delivered neoadjuvant treatment in our study. When the association between the histopathological regression scores and the disease relapse-progression and survival was reviewed, the disease relapse-progression rate was higher in the group that responded poorly to neoadjuvant treatment and a borderline association with survival was detected in the patient group of our study. Conclusion: F-18 FDG PET/CT is an effective imaging modality for detecting primary tumors and metastases in rectal cancer and it was concluded that the SUVmax values of lymph nodes have a prognostic value and that this will contribute to the preoperative staging and management of treatment in patients with rectal cancer.
