Özet
AMAÇ:Çalışmamızda lomber disk hernisi cerrahisi için preoperatif ESP bloğu uygulanan olgularda adjuvan olarak deksametazon eklenmiş ve eklenmemiş olguların postoperatif ağrı düzeylerinin retrospektif karşılaştırılmasını amaçladık. GEREÇ-YÖNTEM:Bu çalışma Kütahya Sağlık Bilimleri Üniversitesi Evliya Çelebi Eğitim ve Araştırma Hastanesi Ameliyahanesi'nde Kasım 2022 ile Mart 2023 tarihleri arasında elektif şartlarda ve genel anestezi altında lomber disk cerrahisi nedeniyle opere edilen 120 hastalanın dosyaları üzerinden retrospektif olarak yapıldı. 60 hasta çalışmaya dahil edildi. Hastalar üç gruba ayrıldı. Birinci gruba ESP bloğu sadece bupivakain ile uygulanan grup, ikinci grup ESP bloğu uygularken bupivakaine 4 mg deksametazon eklenen grup, üçüncü grup ise hiç blok uygulanmamış kontrol grubu olarak belirlendi. Hastalara blok genel anesteziden sonra prone pozisyonunda uygulanmıştı.Tüm olguların operasyon başı, intraoperatif ve postoperatif kalp atım hızı, sistolik ve diyastolik basınçları ile postoperatif birinci saat, ikinci saat, dördüncü saat, altıncı saat, 12. Saat ve 24. Saat NRS skorları kaydedildi. İlk analjezik uygulanan saat, postoperatif 24 saat kullanılan analjezik miktarı ve toplam opioid kullanımı kaydedilmişti. AİM:In our study, we aimed to retrospectively compare the postoperative pain levels of patients with and without dexamethasone added as an adjuvant in patients who underwent preoperative ESP block for lumbar disc herniation surgery. MATERIALS AND METHOD: This study was carried out retrospectively on the files of 120 patients who were operated for lumbar disc surgery under general anesthesia under elective conditions between November 2022 and March 2023 in Evliya Çelebi Training and Research Hospital, Kütahya Health Sciences University. 60 patients were included in the study. The patients were divided into three groups. The first group was determined as the group in which ESP block was applied only with bupivacaine, the group in which 4 mg dexamethasone was added to bupivacaine while the second group applied ESP block, and the third group was determined as the control group, which did not receive any block. The block was applied to the patients in the prone position after general anesthesia. The preoperative, intraoperative and postoperative heart rate, systolic and diastolic pressures, and postoperative first hour, second hour, fourth hour, sixth hour, 12th hour and 24th hour NRS scores were recorded for all patients. The time of first analgesic administration, the amount of analgesic used in the postoperative 24 hours and total opioid use were recorded.
