Özet
Amaç: Bu araştırmada, kolorektal kanser tanısı ile yatan ve kolorektal cerrahi yapılacak olan hastalara ameliyat öncesi ERAS protokolleri doğrultusunda verilen eğitimin hastaların ameliyat sonrası iyileşmeleri ile birlikte ameliyat sonrası hastanede yatış süreleri, toplam hastanede yatış süreleri, ameliyat sonrası yatış süresince komplikasyon nedeniyle tekrar ameliyat olma durumları ve komplikasyon nedeniyle 30 günlük tekrar hastaneye başvurma durumlarına etkisinin incelenmesi amaçlanmıştır. Gereç ve Yöntem: Araştırma tam deneysel, son test kontrol gruplu tasarımda bir randomize kontrollü çalışma olarak İzmir Şehir Hastanesi genel cerrahi kliniklerinde Eylül 2024-Kasım 2025 tarihleri arasında yapılmıştır. Örneklemi, adı geçen kliniklerde araştırmanın yapıldığı tarihler arasında kolorektal kanser tanısı ile yatan, kolorektal cerrahi yapılacak olan ve örnekleme alınma ölçütlerini karşılayan hastalar oluşturmuştur. 90 hasta randomize edilmiş, her iki grupta da 1'er hasta takipte kaybedilmiştir. Araştırma, kontrol grubunda 44, deney grubunda 44 olmak üzere 88 hasta ile tamamlanmıştır. Randomizasyon bilgisayar destekli bir yöntemle gerçekleştirilmiştir. Araştırma tek kör olarak uygulanmıştır. Veriler Hasta Tanıtım Formu ve İyileşme Kalitesi-40 (QoR-40) Anketi kullanılarak toplanmıştır. Müdahale grubuna ameliyat öncesi ERAS protokolleri doğrultusunda eğitim verilmiştir. Eğitim, ameliyat öncesi, sırası ve sonrası uygulamaları kapsamıştır. Eğitim materyali olarak eğitim kitapçığı kullanılmıştır. Kontrol grubu standart uygulamayı almıştır. Veriler bağısız örneklem t-testi, Mann Whitney U testi ve ki-kare testi kullanılarak analiz edilmiştir. Bulgular: Müdahale grubunda kontrol grubu ile karşılaştırıldığında ameliyat sonrası 1. günde, duygusal durum (t=2.972, p=0.004, d=0.63), fiziksel konfor (t=3.848, p=0.000, d=0.82), psikolojik destek (t=2.112, p=0.038, d=0.45), fiziksel bağımsızlık (t=3.041, p=0.003, d=0.65), ağrı (t=2.726, p=0.008, d=0.58) olmak üzere tüm alt boyut ve toplam QoR-40 puanlarının (t=3.778, p=0.000, d=0.81) daha yüksek olduğu gösterilmiştir. Müdahale ve kontrol grupları arasında ameliyat sonrası hastanede yatış süresi (z=-0.979; p=0.328), toplam hastanede yatış süresi (z=-0.302; p=0.762), ameliyat sonrası yatış süresince komplikasyon nedeniyle tekrar ameliyat olma oranı (χ2=1.011, p=0.315, Pearson) ve komplikasyon nedeniyle 30 günlük tekrar hastaneye başvurma oranı (χ2=0.000, p=1.000, Yates) açısından fark olmadığı belirlenmiştir. Sonuç: Kolorektal kanser tanısı ile yatan ve kolorektal cerrahi yapılacak olan hastalara ameliyat öncesi ERAS protokolleri doğrultusunda verilen eğitimin hastaların ameliyat sonrası iyileşmelerini artırdığı, buna karşılık diğer sonuçları etkilemediği sonucuna ulaşılmıştır.

Objective: This study aims to investigate the effect of preoperative education provided to patients hospitalized with a diagnosis of colorectal cancer and scheduled for colorectal surgery, in accordance with ERAS protocols, on postoperative recovery, postoperative length of hospital stay, total length of hospital stay, the need for reoperation due to complications during the postoperative hospital stay, and the need for 30-day readmission to the hospital due to complications. Materials and Methods: The study was a fully experimental, randomized controlled trial with a test-control group design conducted in the general surgery clinics of Izmir City Hospital between September 2024 and November 2025. The sample consisted of patients hospitalized with a diagnosis of colorectal cancer during the study period in the aforementioned clinics, who were scheduled to undergo colorectal surgery and who met the inclusion criteria. 90 patients were randomized, and 1 patient in each group was lost to follow-up. The study was completed with 88 patients, 44 in the control group and 44 in the experimental group. Randomization was performed using a computer-assisted method. The study was conducted single-blind. Data were collected using the Patient Information Form and the Quality of Recovery-40 (QoR-40) Questionnaire. The intervention group received preoperative education based on ERAS protocols. The education covered preoperative, intraoperative, and postoperative practices. An educational booklet was used as the educational material. The control group received standard care. Data were analyzed using the independent samples t-test, Mann Whitney U test, and chi-square test. Results: Compared to the control group, the intervention group showed significant differences on the first day after surgery in emotional state (t=2.972, p=0.004, d=0.63), physical comfort (t=3.848, p=0.000, d=0.82), psychological support (t=2.112, p=0.038, d=0.45), physical independence (t=3.041, p=0.003, d=0.65), pain (t=2.726, p=0.008, d=0.58), and total QoR-40 scores (t=3.778, p=0.000, d=0.81) were higher. There were no significant differences between the intervention and control groups in terms of postoperative length of hospital stay (z=-0.979, p=0.328), total length of hospital stay (z=-0.302, p=0.762), the rate of reoperation due to complications during the postoperative hospital stay (χ2=1.011, p=0.315, Pearson), and the rate of 30-day readmission to the hospital due to complications (χ2=0.000, p=1.000, Yates). Conclusion: It has been concluded that preoperative education provided to patients hospitalized with a diagnosis of colorectal cancer and scheduled for colorectal surgery, based on ERAS protocols, improves postoperative recovery in these patients, but does not affect other outcomes.
