Özet
Bu çalışma, Streptococcus (S.) pneumoniae'nin invaziv pnömokok hastalıklarındaki (İPH) rolünü ve kesin tanısında kullanılan yöntemlerin güncel yerini değerlendirmeyi amaçlamaktadır. Ayrıca üst solunum yolu sürüntü örneklerinde moleküler yöntemle saptanan S. pneumoniae varlığının, alt solunum yolu balgam örneklerinde klasik kültür yöntemleri ile gösterilip gösterilemediğinin araştırılması ve üst solunum yolunda tespit edilen etkenin kolonizasyon mu yoksa enfeksiyon etkeni mi olduğunun ortaya konulması hedeflenmiştir. Çalışmada 311 nazofarengeal ve orofarengeal sürüntü örnekleri ile 96 balgam örnekleri değerlendirilmiştir. Üst solunum yolu örneklerinde moleküler yöntem olarak Real-Time PCR kullanılmıştır. Alt solunum yolu örneklerinde klasik kültür yöntemleri uygulanmış; izolasyon sonrası identifikasyon Matriks destekli lazer desorpsiyon iyonizasyon- uçuş zamanı kütle spektrometresi (MALDI-TOF MS) sistemi ile yapılmış, optokin duyarlılık testi ile fenotipik doğrulama gerçekleştirilmiştir. Elde edilen veriler istatistiksel olarak analiz edilmiştir. Üst solunum yolu örneklerinde Real-Time PCR ile S. pneumoniae pozitiflik oranı 193/331 (%62,1) idi. 18-65 yaş arası grupta % 67,3, 65 yaş üstü grupta % 54,3 olarak saptanmıştır ve yaş grupları arasındaki fark istatistiksel olarak anlamlıydı (χ²=5,443; p=0,02). PCR'da pnömokok pozitifliği saptanan 193 hastadan alınabilen 96 balgam örnekleri kültür yöntemi ile değerlendirildiğinde hiçbir örnekte S. pneumoniae üremesi saptanmamıştır. Moleküler yöntemlerin kültür yöntemine kıyasla daha yüksek pozitiflik oranı verdiği görülmüştür. Çalışmaya dahil edilen 311 sürüntü örneğinin PCR'da pnömokok pozitifliğinin, pnömoni tanısına göre dağılımı arasındaki fark istatisksel olarak anlamlıydı (χ²=9,525; p=0,002). Komorbid hastalık varlığı genel olarak PCR pozitifliği ile ilişkili bulunmamıştır (p=0,525). Hematolojik malignite, diyabet ve hipertansiyon ile PCR pozitifliği arasında anlamlı ilişki saptanmamıştır (p>0,05). Buna karşın solid organ malignitesi varlığı ile PCR sonuçları arasında istatistiksel olarak anlamlı bir ilişki bulunmuştur (χ²=6,014; p=0,014). Solid organ malignitesi bulunan hastalarda PCR pozitifliği anlamlı derecede daha düşük saptanmıştır. Pediatrik grup yaşa göre değerlendirildiğinde, ≤2 yaş grubunda PCR pozitifliği % 91,2 gibi yüksek bir oranda bulunmuştur. Bu bulgular, moleküler yöntemlerin kolonizasyon saptamada duyarlılığının yüksek olduğunu ve bazı klinik değişkenlerin pnömokok pozitifliği üzerinde anlamlı etkisinin bulunmadığını göstermektedir. S. pneumoniae'nin neden olduğu invaziv enfeksiyonlarda hızlı ve doğru tanı hayati öneme sahiptir. Moleküler yöntemler özellikle antibiyotik kullanımı sonrası tanısal üstünlük sağlamaktadır. Ancak kolonizasyon ile enfeksiyon ayrımında tek başına moleküler pozitifliğin yeterli olmadığı, klinik bulgular ve kültür sonuçları ile birlikte değerlendirilmesi gerektiği sonucuna varılmıştır. Doğru tanı yönteminin seçimi, uygun tedavi stratejisinin belirlenmesine ve gereksiz antibiyotik kullanımının azaltılmasına katkı sağlayacaktır.

This study aims to evaluate the role of Streptococcus (S.) pneumoniae in invasive pneumococcal diseases (IPD) and the current status of methods used for its definitive diagnosis. Furthermore, it aims to investigate whether S. pneumoniae detected by molecular methods in upper respiratory tract swab samples can be cultured from lower respiratory tract sputum samples, and to determine whether the pathogen detected in the upper respiratory tract is a colonizer or an infectious agent. In this study, 311 nasopharyngeal and/or oropharyngeal swab samples and 96 sputum samples were evaluated. Real-Time PCR was used as the molecular method for upper respiratory tract samples. Classical culture methods were applied to lower respiratory tract samples; identification after isolation was performed using matrix-assisted laser desorption ionization-time-of-flight mass spectrometry (MALDI-TOF MS), and phenotypic confirmation was performed with optochin susceptibility testing. The obtained data were statistically analyzed. The positivity rate for S. pneumoniae in upper respiratory tract samples using Real-Time PCR was 193/331 (62.1%). This rate was 67.3% in the 18-65 age group and 54.3% in the over-65 age group, with a statistically significant difference between age groups (χ²=5.443; p=0.02). Of the 193 patients with pneumococcal positivity detected by PCR, 96 sputum samples were cultured, and no S. pneumoniae growth was detected in any sample. Molecular methods yielded a higher positivity rate than culture methods. The distribution of pneumococcal positivity in PCR among the 311 swab samples included in the study, according to pneumonia diagnosis, was statistically significant (χ²=9.525; p=0.002). The presence of comorbid diseases was generally not associated with PCR positivity (p=0.525). No significant association was found between hematological malignancy, diabetes, and hypertension and PCR positivity (p>0.05). However, a statistically significant association was found between the presence of solid organ malignancy and PCR results (χ²=6.014; p=0.014). PCR positivity was significantly lower in patients with solid organ malignancy. When the pediatric group was evaluated by age, PCR positivity was 91.2% in the ≤ 2-year age group. These findings demonstrate that molecular methods are highly sensitive for detecting colonization and that some clinical variables do not significantly affect pneumococcal positivity. Rapid and accurate diagnosis is vital in invasive infections caused by S. pneumoniae. Molecular methods offer superior diagnostic performance, especially after antibiotic use. However, it was concluded that molecular positivity alone is insufficient to differentiate between colonization and infection and should be evaluated in conjunction with clinical findings and culture results. Choosing the correct diagnostic method will help determine the appropriate treatment strategy and reduce unnecessary antibiotic use.
