Özet
Sanal gerçeklik egzersizleri (SGE), bireyin çevresiyle etkileşimini artırarak motor, duyusal ve algısal süreçleri etkileyebilen rehabilitasyon yaklaşımları arasında yer almaktadır. Beden sahipliği ise kişinin bedenini kendisine ait olarak deneyimlemesini ifade etmekte olup, sanal ortamların immersiyon düzeyinden etkilenebileceği düşünülmektedir. Bu tez çalışmasının amacı, sağlıklı bireylerde, kısa süreli, tam immersif ve kısmi immersif SGE'nin kauçuk el illüzyonu (KEİ) testi ile değerlendirilen beden sahipliği üzerindeki etkilerini araştırmak ve birbirlerine üstünlüğünü karşılaştırmaktır. Sağlıklı erişkinin dahil edildiği bu araştırmada, Botvinick beden sahipliği anketi (BSA) ve proprioseptif sapma (PS) ölçümü aracılı klasik masaüstü KEİ testi yapılarak katılımcılarda illüzyon oluşup oluşmadığı test edildi. Katılımcıların Semmes-Weinstein monofilament testi (SWMT) ile dokunma algısı eşiği (DAE), algometre ile basınç ağrı eşiği (BAE) değerlendirildi. KEİ testi sonucunda illüzyon oluşan ve oluşmayan katılımcılar tam immersif ve kısmi immersif SGE gruplarına randomize edildi. Tam immersif sanal gerçeklik egzersizleri Oculus Quest 2 aracılı Hand Physics Lab oyunuyla, kısmi immersif egzersizler ise Leap Motion Controller aracılı ErgoActive oyunu ile gerçekleştirildi. Egzersizler birer gün arayla 10 dakika süresince toplam 3 seans olacak şekilde terapist gözetiminde gerçekleştirdi. Başlangıçta illüzyon oluşmayan katılımcılara 3. seans sonunda KEİ testi uygulanarak illüzyon oluşup oluşmadığı incelendi. Başlangıçta illüzyon oluşan katılımcılara ise 1. ve 3. seans sonunda SMWT ile BAE testleri tekrarlandı ve KEİ testi uygulanarak BSA ve PS skorları ölçüldü. İllüzyon oluşan 30 ve illüzyon oluşmayan 30 katılımcı olmak üzere toplam 60 katılımcının olduğu bu araştırmada, her subgrupta 15 birey vardı. Grupların yaş, cinsiyet ve diğer sosyodemografik özellikleri benzerdi. Başlangıçta illüzyon oluşmayan katılımcıların SGE sonunda 8'inde (5'i tam immersif, 3 kısmi immersif) illüzyon gelişti. Başlangıçta illüzyon oluşan katılımcıların PS değeri yalnız tam immersif grupta istatistiksel olarak anlamlı artış gösterdi (p=0,001). Başlangıçta illüzyon oluşan katılımcıların BSA'nın 3., 7. ve 9. soru puanları tam immersif grupta zaman içinde istatistiksel olarak anlamlı artış gösterirken (sırasıyla p=0,018, p<0,001 ve p<0,001), 2. soru puanındaki değişim Bonferroni düzeltmesi sonrasında istatistiksel anlamlılığını koruyamadı (p=0,030). Kısmi immersif grupta ise BSA'nın hiçbir sorusunda zaman içindeki değişim istatistiksel olarak anlamlı değildi. BAE skorlarında grup içi ve gruplar arası anlamlı değişim saptanmadı. Tam immersif SGE grubunda DAE skorunda zaman içerisinde değişim Bonferroni düzeltmesi sonrası istatistiksel anlamlığını koruyamazken (p=0,018), kısmi immersif gruptaki değişim de anlamlı değildi (p=0,124). Bu araştırmada, sağlıklı bireylere uygulanan SGE'nin KEİ'yi etkileyebileceği, ayrıca illüzyonun öznel ve nesnel göstergelerinin tam immersif uygulamalarda daha belirgin biçimde güçlenebileceği gösterilmiştir. Elde edilen bulgular, SGE'nin beden sahipliği ve beden temsili üzerinde etkili olabileceğine işaret etmekte; bu etkilerin farklı uygulama süreleri, popülasyonlar ve klinik gruplarda ileri çalışmalarla araştırılmasına zemin hazırlamaktadır.

Virtual reality exercises (VREs) are among the rehabilitation approaches that can influence motor, sensory, and perceptual processes by enhancing an individual's interaction with the surrounding environment. Body ownership refers to the experience of one's body as belonging to oneself and is thought to be influenced by the level of immersion provided by virtual environments. The aim of this thesis was to investigate the effects of short-term fully immersive and semi-immersive VREs on body ownership, assessed using the Rubber Hand Illusion (RHI) paradigm, in healthy individuals and to compare the superiority of these two approaches. In this study involving healthy adults, the classical tabletop RHI test was performed using the Botvinick Body Ownership Questionnaire (BOQ) and proprioceptive drift (PD) measurements to determine whether the illusion was elicited. Tactile perception threshold (TPT) was assessed using the Semmes–Weinstein Monofilament Test (SWMT), and pressure pain threshold (PPT) was evaluated using a pressure algometer. Participants in whom the RHI was elicited and those in whom it was not elicited were randomized into either the fully immersive or the semi-immersive VRE groups. Fully immersive virtual reality exercises were performed using the Hand Physics Lab application with the Oculus Quest 2, whereas semi-immersive exercises were conducted using the ErgoActive application with the Leap Motion Controller. The exercises were supervised by a therapist and consisted of three 10-minute sessions performed on alternate days. Participants who did not experience the illusion at baseline underwent the RHI test again after the third session to determine whether the illusion had developed. In participants who experienced the illusion at baseline, the SWMT and PPT assessments were repeated after the first and third sessions, and the RHI test was repeated to evaluate BOQ and PD scores. The study included a total of 60 participants, comprising 30 individuals with RHI and 30 without RHI at baseline, with 15 participants in each subgroup. The groups were comparable in terms of age, sex, and other sociodemographic characteristics. Among participants who did not experience the illusion at baseline, the illusion developed in eight individuals following the VRE intervention (five in the fully immersive group and three in the semi-immersive group). In participants who experienced the illusion at baseline, PD increased significantly over time only in the fully immersive group (p = 0.001). Scores for Questions 3, 7, and 9 of the BOQ increased significantly over time in the fully immersive group (p = 0.018, p < 0.001, and p < 0.001, respectively), whereas the change in Question 2 did not remain significant after Bonferroni correction (p = 0.030). No significant changes over time were observed in any BOQ item in the semi-immersive group. No significant within-group or between-group differences were found in PPT scores. Although a change over time was observed in TPT scores in the fully immersive group, this significance was not maintained after Bonferroni correction (p = 0.018), while no significant change was observed in the semi-immersive group (p = 0.124). The findings of this study demonstrate that VREs may influence the Rubber Hand Illusion in healthy individuals and that both the subjective and objective indicators of the illusion may be enhanced more prominently with fully immersive applications. These results suggest that VREs may modulate body ownership and body representation and provide a basis for future studies investigating these effects across different intervention durations, populations, and clinical groups.
