Özet

Amaç: Liken planus (LP), kutanöz ve mukozal yüzeyleri etkileyen, kronik ve inflamatuar bir dermatozdur. LP hastalarında ön planda oral mukoza tutulumu görülmekle beraber, nadiren diğer mukozalar da etkilenebilmektedir. Bu çalışma, liken planusun nazal mukoza üzerindeki etkilerini değerlendirmeyi, hastalığın kronik inflamatuar doğası ve mukozal tutulum kapasitesi doğrultusunda, hastaların koku ve tat duyularında meydana gelebilecek değişiklikleri incelemeyi amaçlamaktadır. Gereç ve Yöntem: Bu tek merkezli, prospektif kesitsel çalışmaya klinik ve histopatolojik olarak LP tanılı 44 hasta ile yaş ve cinsiyet açısından bu hastalarla eşleştirilmiş 44 sağlıklı gönüllü dahil edildi. Tüm katılımcılara ayrıntılı oral ve nazal mukoza muayenesi yapıldı. Koku fonksiyonları CCCRC (Connecticut Chemosensory Clinical Research Center) testi ile, tat fonksiyonları tat şeritleri yöntemiyle değerlendirildi. Hastalık şiddeti, mukozal tutulum şiddeti, histopatolojik özellikler ve eşlik eden sistemik hastalıklar değerlendirilerek koku ve tat fonksiyonları ile olan ilişkileri incelendi. Nazal endoskopik muayeneyle nazal kavite, oral kavite ve orofarenkste mukozal tutulum varlığı ve tipleri değerlendirildi, şüpheli lezyonlar ayrıntılı olarak incelendi. Bulgular: 44 LP tanılı hastanın 27'sinde (%61,4) oral, 7'sinde (%15,9) nazal mukozada liken planusla uyumlu lezyonlar saptandı. LP grubu ve kontrol grubunun koku ve tat fonksiyonları karşılaştırıldığında, LP grubunda koku puanlarında anlamlı azalma görüldü; LP grubunda tat fonksiyonlarında azalma mevcut olsa da bu azalma kontrol grubuna göre anlamlı düzeyde değildi. OLP tanılı hastalarda oral tutulum olmayan gruba ve kontrol grubuna göre total koku ve tat puanlarında anlamlı azalma mevcuttu. Nazal mukozal tutulumlu hastaların total koku puanlarında da nazal tutulum saptanmayan gruba ve kontrol grubuna göre anlamlı azalma saptandı. Tat puanları özellikle dil tutulumu olan hastalarda anlamlı düzeyde azalmıştı. Sonuç: Nazal mukozada LP'ye bağlı tutulum saptanabilmekte olup özellikle oral mukozal tutulumu olan hastalarda mukozal lezyonlarının tedavisi ve takibi amacıyla endoskopik nazal muayene gerekli olabilir. LP hastalarında, diğer mukozal tutulum yapabilen dermatolojik hastalıklarda da olduğu gibi, koku ve tat fonksiyonlarında azalma olabilmekte; bu azalma mukozal tutulum saptanan hastalarda öne çıkmaktadır.

Background and Aim: Lichen planus (LP) is a chronic inflammatory dermatosis affecting cutaneous and mucosal surfaces. Although oral mucosal involvement is the predominant manifestation in patients with LP, other mucosal sites may rarely also be affected. This study aimed to evaluate the effects of lichen planus on the nasal mucosa and to investigate possible alterations in olfactory and gustatory functions in affected patients, considering the chronic inflammatory nature and mucosal involvement potential of the disease. Materials and Methods: This single-center, prospective cross-sectional study included 44 patients with clinically and histopathologically confirmed LP and 44 age- and sex-matched healthy volunteers. All participants underwent detailed oral and nasal mucosal examinations. Olfactory functions were assessed using the CCCRC (Connecticut Chemosensory Clinical Research Center) test, and gustatory functions were evaluated using taste strips. Disease severity, mucosal involvement severity, histopathological features, and accompanying systemic diseases were evaluated, and their relationships with olfactory and gustatory functions were analyzed. The presence and types of mucosal involvement in the nasal cavity, oral cavity, and oropharynx were assessed by nasal endoscopic examination, and suspicious lesions were examined in detail. Results: Among 44 patients with LP, lesions compatible with lichen planus were detected in the oral mucosa in 27 patients (61.4%) and in the nasal mucosa in 7 patients (15.9%). When olfactory and gustatory functions were compared between the LP and control groups, olfactory scores were significantly decreased in the LP group. Although gustatory functions were also reduced in the LP group, this decrease was not statistically significant compared with the control group. Patients with OLP had significantly lower total olfactory and gustatory scores than both patients without oral involvement and healthy controls. Total olfactory scores were also significantly lower in patients with nasal mucosal involvement compared with those without nasal involvement and healthy controls. Gustatory scores were significantly decreased, particularly in patients with tongue involvement. Conclusion: Nasal mucosal involvement associated with lichen planus may be detected, and endoscopic nasal examination may be necessary, particularly in patients with oral mucosal involvement, for the evaluation and follow-up of mucosal lesions. Similar to other dermatological diseases with mucosal involvement, patients with lichen planus may exhibit impaired olfactory and taste functions, with this impairment being more prominent in patients with mucosal involvement.
