Özet
2. ÖZET Nervus medianus'un orijini, diğer sinirlerle anastomozları, önkoldaki dalları ve kompresyon noktaları 30 üst ekstremitede değerlendirildi. Nervus medianus orijinin processus coracoideus'a uzaklığı ortalama 38.5 mm olarak ölçüldü. Nervus medianus'un önkoldaki 6 ana somatomotor dalı önce makro, ardından mikrodiseksiyon aşamalarında incelendi. L, II., III., IV., V. ve VI. Somatomotor Dal'ların LI'ye uzaklıkları sırasıyla ortalam +62.2, +30.4, -2.4, -67.9, -104.8 ve -142.8 mm, kas dalı sayıları sırasıyla 2-3, 3, 3, 2, 1 ve 1 olarak saptandı. Nervus medianus'dan ayrılma yönleri I., II., ve IV: Somatomotor Dal'ların sıklıkla medial'e, III. ve V. Somatomotor Dal'ların posterior'e ve VI. Somatomotor Dalın ise lateral'e bulundu. I. Somatomotor dal musculus pronator teres'i, II. Somatomotor dal musculus flexor carpi radialis ve musculus palmaris longus'u, III: Somatomotor Dal musculus flexor digitorum profundus, musculus flexor pollicis longus ve musculus pronator quadratus'u, IV., V., ve VI. dalların musculus flexor digitorum superficialis'i inerve ettiği saptandı. II. Somatomotor Dal'da daha sık varyasyon örneklerine rastlandı. Kolda nervus medianus ile nervus musculocutaneus arasında 7 olguda (%23.3), önkolda ise nervus medianus arasında 3 olguda (%10.0) anastomoza rastlandı. Nervus medianus'un önkoldaki 5 kompresyon noktası olan lacertus fibrosus, musculus pronator teres, musculus flexor digitorum superficialis arkusu ve Gantzer kası'nın arkasına geçtiği yer ile arter dallarının siniri çaprazladıkları yerin linea interepicondylaris'e uzaklıklarının ortalaması sırasıyla 21.9, 38.2, 69.2, 77.9 ve 88.9 mm olarak belirlendi. Ayrıca önkoldaki kompresyon noktalarında fibröz değişikliklerin varlığı incelendi. Bu veriler çeşitli nedenlerle hasara uğrayan nervus medianus'un onarımında, greftinde, flep kullanımında, diğer sinirlerle olan anastomozların klinik ve elektofizyolojik çalışmalarda değerlendirilmesinde ve önkoldaki kompresyon nedenlerinin incelenmesinde anlamlı olacaktır

3. SUMMARY THEMJSCULAR BRANCHES AND THE COMPRESSION POINTS OF THl MEDIAN" NERVE IN THE FOREARM The origin, anastomosis with other nerves branches and compression points in the forearm of median nerve was evaluated in 30 upper extremities. The distance of the origin of the median nerve to processus coracoideus was measured to be 38.5 mm. The six major Somatomotor Branches of the median nerve was studied with macro and microdissections. The mean distance of 1st, 2nd, 3rd, 4th, 5th, 6th Somatomotor Branches to LI were measured to be +63.2, +30.4, -2.4, -67.9, -104.8 and -142.8 mm respectively. The number of muscle branches were 2-3, 3, 3, 2, 1 and 1 respectively. The 1st, 2nd and 4th branches coursed medially from median nerve, 3rd and 5th Somatomotor Branches coursed posteriorly and 6th Somatomotor Branch coursed laterally. The 1st Somatomotor Branch innervated the pronator teres muscle, 2th Somatomotor Branch innervated the flexor carpi radialis and the palmaris longus muscle, 3th Somatomotor Branch innervated the flexor digitorum profundus muscle, the pollicis longus muscle and the pronator quadratus muscle, 4th, 5th and 6th Somatomotor Branches innervated the flexor digitorum superficialis muscle. 2th Somatomotor Branch showed more variations than the others. The median nerve anastomosed with the musculocutaneous nerve in seven cases (%23.3) in the arm. The median nerve anastomosed with the ulnar nerve in three cases (%1 0.0) in the forearm. The mean distance of the five compression points the median nerve which are lacertus fibrosus, the pronator teres muscle, the flexor digitorum superficialis muscle and Gantzer's muscle, to LI were 21.9, 38.2, 69,2, 77.9 and 88.9 mm. In addition the fibrotic changes in the compression points in the forearm were evaluated. This findings may be use in the traumatic median nerve repair, usage in the flaps surgery. This findings may also be used in the clinical evaluation of anastomosis with other nerves and in the examination of the forearm compression pathologies.
