Özet

ÖZET Bu çalışmanın amacı, anterior maksillada implant destekli farklı protez tasarımlarının implantlarda ve destekleyen kemikte meydana getirdiği stresleri üç boyutlu sonlu elemanlar stres analizi ile araştırmaktır. Bu amaç için anterior dişlerinin tamamı eksik olan bir hastadan (Kennedy IV) alınan bilgisayarlı tomografi görüntülerinden yararlanılarak sonlu elemanlar modeli oluşturuldu, implant çapı ve protez tasarım değişiklikleri ile 10 farklı sabit protez modeli hazırlandı. Bu modellere yükleme 1: Kanin implant bölgesine 469 N ve yükleme 2: Kesici dört implant bölgesine toplam 222 N olmak üzere şeklinde ısırma fonksiyonunu taklit etmek amacıyla iki ayrı yükleme uygulandı. Stres analizlerinin sonuçlarına göre, farklı protetik tasarımlarının değişen yük koşulları altındaki biyomekaniksel davranışları farklı bulunmuştur, implant çap ve sayısının artması ve implantların splintlenmesi maksiller anterior implantlardaki ve destekleyen kemikteki stresleri azaltmaktadır. Maksiller anterior bölgedeki implant destekli restorasyonlarda anterior ve posterior kantilever oluşturan tasarımlarda daha yüksek stres değerleri bulunmuştur. Anterior maksillanın implant destekli sabit bölümlü protezlerle tedavisinde implant sayısı ve çapının arttırılması, implantların splintlenmesi ve kantilever uzantıların kullanılmaması maksiller implantlar ve çevresindeki kemikte daha az stres değerlerine neden olmaktadır.

ABSTRACT The purpose of this study was to explore the stress distribution in bone surrounding implants in different implant-supported fixed partial prosthetic designs in anterior maxilla using three-dimensional finite element stress analysis. In this regard, a finite element model was created by reconstructing a series of computed tomography images obtained from an individual. On this mathematical model, 10 implant-supported fixed prosthetic dentures were constructed on different number and diameter of supporting implants. The models were loaded in two separate cases; loading 1: 469 N load application in the canine implant region and loading 2: a total of 222 N load applicaton on four incisor implants, to simulate forces introduced on implants in the anterior maxilla. Finite element stress analysis revealed that, the biomechanical behavior of prosthetic designs depended on the mode of loading. Increase in implant diameters and numbers, and splinting of implants led to a remarkable decrease in stresses within the implant complex and in bone surrounding implants. Anterior and posterior cantilevered restorations resulted in high stress magnitudes in and around implants. During the treatment of anterior maxilla with implant supported fixed partial prosthetic reconstructions, increase in implant number and diameter, splinting of implants, and avoiding cantilever extensions reduce stress magnitudes in the implants and the supporting bone.
