Özet
1. ÖZET Bu çalışmanın amacı, postmenopozal dönemde bulunan postmenopozal osteoporozlu kadınlarda iskelet BMD'si ve mandibula BMD'sini dansitometrik açıdan ve laboratuar bulguları ile karşılaştırarak değerlendirmek ve osteoporozun hem iskelet hemde mandibula üzerindeki etkisini saptamaktır. Bunun yam sıra osteoporozlu hastalara uygulanan dental implantlann osseointegrasyon dönemi içinde kemik iyileşmesinin radyolojik ve klinik olarak takip edilmesi ve bu dönem içinde osteoporozun implant kaybma sebep olup olmayacağının incelenmesi amaçlanmıştır. Bu çalışma Marmara Üniversitesi Dişhekimliği Fakültesi Ağız-Diş-Çene Hastalıkları ve Cerrahisi Anabilim Dalı'na başvuran 55-75 yaş grubu kadm hastalarda yapılmıştır. Bu hastalar, I.Ü. Tıp Fak. Fizik Tedavi ve Rehabilitasyon Anabilim Dalı- Osteoporoz Ünitesi'ne gönderilerek muayeneleri yapılmış ve postmenopozal osteoporoz tamları konulmuştur. Dansitometrik incelemelerde kullanılan BMD ölçümleri Cerrahpaşa Tıp Fakültesi Nükleer Tıp Anabilim Dalı'nda, kan ve ürin tahlilleri Ria Biyokimya Laboratuarı'nda gerçekleştirilmiştir. Çalışma grubumuz randomize olarak belirlenen alendronat+Ca+D vitamini tedavisi alacak 23 kadm hasta ve kontrol grubu olarak Ca+D vitamini kullanacak 13 kadm hastadan olmak üzere 2 gruptan oluşmaktadır. Hastaların başlangıç ve ö.aydaki dansitometrik ölçümlerinde lomber, radius, total iskelet BMD ve mandibulamn (korpus,ramus^nentum) BMD değerleri incelendi. Ayrıca başlangıçta, 3. ayda ve ö.ayda, kemik formasyon belirleyicileri (BALP, N-Telopeptid(s) ) ile kemik rezorbsiyon belirleyicileri (N-Telopeptid(u), pridinolin, deoksipridinolin) değerleri incelendi. Her iki gruptan beşer hastaya toplam 19 implant uygulandı ve diğer hastalarla birlikte 6 ay süresince takip edildi. Alendronat+Ca+D vitamini tedavisi gören grupta lomber bölgede ve total BMD değerinde yüzdesel bir artış gözlenirken, radius BMD'si sabit kalmıştır. Buna karşılık Ca+D vitamini alan hastalarda lomber bölge ve radius'ta BMD değerinde yüzdesel olarak bir azalma olmasına rağmen total BMD değerinde istatiksel anlamlı olmayan bir artış saptanmıştır. Mandibula değerlerine göre ise, alendronat+Ca+D vitamini alan grupta ramus, mentum ve korpus bölgelerinde artma eğiliminin olduğu ancak anlamlı farklılığa ulaşmadığı gözlendi. Ca+D vitamini grubunda ise mentum hariç diğer bölgelerde BMD değerinde azalma tespit edilmiştir (p>0,05). Ayrıca, uygulanan 19dental implantta ise kayıp olmadığı belirlendi. Çalışmamızda, erken dönemde postmenopozal osteoporozun osseointegrasyonu etkilemediğini ve alendronatm kemik dansitesinde artışa olumlu katkılannm olduğu gözlenmiştir.

2. SUMMARY EVALUATION OF THE EFFECTS OF POSTMENOPAUSAL OSTEOPOROSIS ON THE MANDIBLE AND APPLICATIONS OF DENTAL IMPLANTS ON POSTMENOPAUSAL OSTEOPOROTIC PATIENTS The aim of this study is to evaluate the BMD's of the skeleton and mandible in postmenopausal osteoporotic women by demonstrating their relations with bone resorption/formation markers. Besides, we targeted to evaluate the osseointegration rate of the dental implants by radiologic and clinical examination and planned to show if postmenopausal osteoporosis leads to implant losses. This study was held out on women, between 55-75 years of age, referred to Department of Oral and Maxillofacial Surgery. BMD measurements were performed in Cerrahpaşa Medical Faculty, Department of Nuclear Medicine. Bone formation and resorption markers were measured in Ria Biochemistry Laboratory. Patients were randomised into two groups. The treatment group (n=23) received alendronate+Ca+Vit D while the control group received Ca+Vit D. BMD of the lumbar region, radius, total skeleton and mandible (corpus, ramus, mentum) were measured at the beginning and in the 6 month. In addition, bone formation (BALP, N-Telopeptid(s) ) and resorption (N-Telopeptid(u), pyridinolin, deoxypridinolin) markers were obtained at the beginning, in the 3rd month and in the 6th month. 5 patients from each group were placed 19 dental implants and they were followed-up for 6 months. In the treatment group, we found out an increase in the BMD values of lumbar region and total skeleton (p>0,05) while BMD of radius remained stable. But, there was a decrease in BMD of lumbar region and radius while the BMD of total skeleton increased in the control group (p>0,05). Regarding the BMD values of mandible, BMD was increased in the treatment group, meanwhile in the control group it was decreased in ramus and corpus excluding mentum region. Moreover, there was not any loss in the dental implants.We concluded that postmenopausal osteoporosis did not have a negative effect to osseointegration during the healing period and alendronate does have a positive effect in the bone density.
