Özet
Laparoskopik sleeve gastrektomi uygulanan hastalarda enerji harcaması, hormon düzeyleri ve beslenme durumunun değerlendirilmesi amacıyla yürütülen bu araştırmaya; Erciyes Üniversitesi Tıp Fakültesi Hastanelerinde laparoskopik sleeve gastrektomi geçiren 20 kadın hasta katılmıştır. Hastaların cerrahi öncesinde ve cerrahi sonrası birinci, ikinci ve üçüncü ayda; vücut analizleri yapılmış ve biyokimyasal parametreleri kaydedilmiştir. Bunun yanında, hastaların iştah durumları değerlendirilmiş ve her izlemde üç günlük besin tüketim kayıtları alınmıştır. Hastaların izlem süresince vücut ağırlıkları, beden kütle indeksleri ve vücut yağ yüzdelerinde anlamlı azalma görülmüştür (p<0,05). Hastaların fiziksel aktivite seviyeleri ve toplam enerji harcamaları ise postoperatif dönemde preoperatif döneme göre anlamlı bir artış göstermiş (p<0,05); hastaların toplam enerji, karbonhidrat ve yağ alımı postoperatif dönemde azalmıştır (p<0,05). Biyokimyasal parametreler incelendiğinde ise hastaların serum açlık kan glukozu ve trigliserit düzeyleri izlem süresince anlamlı bir azalma göstermiştir (p<0,05). Cerrahi sonrası dönemde cerrahi öncesine göre plazma tiroid situmulan hormon ve ghrelin düzeylerinin anlamlı olarak azaldığı saptanmıştır (p<0,05). Plazma ghrelin ve tiroid situmulan hormon düzeylerinin izlemler arası değişimi ile vücut ağırlığı, ağırlık kaybı, beden kütle indeksi arasında anlamlı bir ilişki bulunmamıştır (p>0.05). Sonuç olarak, cerrahi sonrası dönemde hastalarda gerçekleşen ağırlık kayıpları ile hormonal değişimler arasında ilişi bulunmamıştır. Plazma ghrelin ve tiroid situmulan hormon düzeylerindeki değişimlerin sleeve gastrektomi sonrası ağırlık kaybı ve metabolik parametrelere etkisinin net olmadığı,bu durumun pek çok fak törden etkilendiği sonucuna varılmıştır.

The research that created by the reason to evaluate hormone level, nutrition status and energy expenditure of patients with laparoscopic sleeve gastrectomy; performed test on 20 female patient who had laparoscopic sleeve gastrectomy operation in the medical faculty of Erciyes University. Body analysis and biochemical parameters of these patient noted before the surgery and after the surgery following months of first, second and third. Beside this, the appetite conditions of these patients evaluated and three days' food consumption records taken within the monitoring process. The percentage of fat in the body, weight, body mass index of the patients decreased significantly (p<0,05). The physical activities and total energy expenditure increased substantively in the postoperative term than the preoperative term (p<0,05). In addition to that; total of energy, carbohydrate and fat consumption of patients decreased (p<0,05). Fasting blood glucose and level of triglyceride decreased according to examination of the biochemical parameters in the monitoring term (p<0,05). At the postoperative term, the level of tiroid situmulan hormone and ghrelin hormone decreased significantly in respect to the preoperative term (p<0,05). There is no relation detected between change of ghrelin hormone and tiroid situmulan hormone level that are within monitoring term and body weight, body mass index and weight loss (p>0.05). Ultimately, there is no detected correlation between weight loss and hormonal changings in the postoperative term. The effect of ghrelin hormone and tiroid situmulan hormone levels on weight changes in post sleeve gastrectomy operation term is not clear, this effected by many factors.
