Özet
Giriş: Dünyada yüzbinlerce insanın ölümüne yol açmış, insanların yaşam tarzını kökten değiştirmiş COVİD-19 pandemisi hastaların hekime başvurularında çekince yaratmıştır. Diyabetes Mellitus (DM) regülasyonunun sağlanmasının Diyabete bağlı oluşan mortalite ve morbiditeyi azalttığı bilinmektedir. Yakın doktor takibi gerektiren DM tanılı hastaların yeterli sağlık hizmeti alamamasının ve bu hastalarda pandemi önlemlerinin oluşturduğu kısıtlılıkların ne gibi sonuçlara yol açtığının belirlenmesi büyük önem taşımaktadır. Amaç: Pandemi sürecinde, pandemi öncesine göre Diabetes mellitus hastalarının glisemik kontrolünde bozulma olup olmadığı ve diyabetik komplikasyonlarında artış meydana gelip gelmediğini araştırmak amaçlanmıştır. Gereç ve Yöntem: Çalışmamızda belirlediğimiz polikliniklere pandemi sürecinde DM sebebiyle başvuran hasta sayısı 986 idi. Bunlardan çalışmaya kabul kriterlerini karşılayan toplam 207 DM hastası çalışmaya dahil edildi. Türkiye'de pandemi başlamadan önceki 1. yıldaki , 2. yıldaki ve pandemi sürecindeki HbA1c değerleri, pandemi başlamadan önceki 1.yıldaki ve pandemi sürecindeki Açlık plazma glukozu, Plazma üre düzeyi, Plazma kreatinin düzeyleri ve Spot idrarda mikroalbumin düzeyi ve anamnez verileri retrospektif olarak taranıp karşılaştırıldı. Bulgular: Pandemiden 1 yıl öncesi ve Pandemi süreci arasında HbA1c değerleri açısından istatistiksel olarak anlamlı fark saptandı(p<0,05). Pandemi sürecinde tedavisinde değişiklik yapılmamış olanların Pandemiden 2 yıl öncesi ve Pandemi süreci ile Pandemiden 1 yıl öncesi ve Pandemi süreci HbA1c değerleri arasında istatistiksel olarak anlamlı fark bulundu(p<0,05). Pandemi öncesindeki 1 yılda rutin doktor takiplerine giden ancak pandemi sürecinde gitmeyenlerde Pandemiden 1 yıl öncesi ve Pandemi süreci HbA1c değerleri arasında istatistiksel olarak anlamlı fark vardı(p<0,05). Yine Pandemi öncesindeki 1 yılda diyet uyumu olup da Pandemi sürecinde diyet uyumu olmayanlardan Pandemiden 2 yıl öncesi ve Pandemi süreci arasında ve Pandemiden 1 yıl öncesi ve Pandemi süreci HbA1c değerleri arasında istatistiksel olarak anlamlı fark saptandı(p<0,05). Pandemi öncesindeki 1 yılda egzersiz yapıp da Pandemi sürecinde egzersiz yapmayanlardan, Pandemiden 2 yıl öncesi ve Pandemi süreci ile Pandemiden 1 yıl öncesi ve Pandemi süreci HbA1c değerleri arasındaki fark istatistiksel olarak anlamlıydı(p<0,05). Pandemi sürecinde 1-2 kg alanlardan, Pandemiden 1 yıl öncesi ve Pandemi süreci HbA1c değerleri arasında istatistiksel olarak anlamlı fark bulundu(p<0,05). Hastaların Pandemi öncesindeki 1 yıla göre, diyet uyumu, egzersiz uyumu, rutin doktor takipleri Pandemi sürecinde aksarken (p<0,01), evdeki kan şekeri ölçümleri Pandemi sürecinde daha düzgün seyretmişti(p<0,05). Hastaların 99'unda (%47,8) diyabetik periferik nöropati semptomları (ellerde veya ayaklarda yanma, uyuşma, hissizlik, karıncalanma, güç kaybı vb. semptomlar) mevcuttu ve pandemi sürecinde bu semptomlarda bir artış olmamışken, 55'inde (%26,6) bu semptomlar hiç olmamıştı, 50'sinde (% 24,2) ise semptomlar mevcuttu ve pandemi sürecinde daha da artmıştı(Tablo 6). Plazma Üre, Plazma Kreatinin, Açlık Plazma Glukozu değerlerinde ve Spot idrar Mikroalbumin pozitifliğinde (≥30mg/l) istatistiksel açıdan anlamlı bir fark bulunmadı(p>0,05). Sonuç: Yaptığımız çalışma Covid – 19 Pandemisinin Diabetes Mellitus hastalarının glisemik kontrolünü bozduğunu ortaya koymuştur. Diyabetik nöropati dışında diyabetik komplikasyonların ortaya çıktığına ve ilerlediğine dair herhangi bir bulgu elde edilemedi.

Introduction: The COVID-19 pandemic, which has caused the death of hundreds of thousands of people in the world and radically changed people's lifestyles, has created hesitations in patients' applications to the physician. It is known that the regulation of Diabetes Mellitus (DM) reduces the mortality and morbidity associated with diabetes. It is of great importance to determine the consequences of the inability of patients with a diagnosis of DM, who require close doctor follow-up, to receive adequate health care and the limitations of pandemic measures in these patients. Aims: It was aimed to investigate whether there is deterioration in glycemic control of Diabetes mellitus patients and whether there is an increase in diabetic complications during the pandemic process compared to the pre-pandemic period. Materials and Methods: The number of patients who applied to the outpatient clinics we determined in our study due to DM during the pandemic was 986. Of these, a total of 207 DM patients who met the inclusion criteria were included in the study. In Turkey, HbA1c values in the 1st year, 2nd year and during the pandemic period, Fasting plasma glucose, Plasma urea level, Plasma creatinine levels and microalbumin level in spot urine and anamnesis data in the 1st year before the start of the pandemic and during the pandemic period in Turkey were retrospectively scanned and compared. Results: A statistically significant difference was found in terms of HbA1c values between 1 year before the pandemic and during the pandemic period (p<0.05). A statistically significant difference was found between the HbA1c values 2 years before the Pandemic and 1 year before the Pandemic and during the Pandemic period of the patients whose treatment was not changed during the pandemic process (p<0.05). There was a statistically significant difference between the HbA1c values 1 year before the pandemic and during the pandemic in those who went to routine doctor follow-ups in the 1 year before the pandemic but did not go during the pandemic period (p<0.05). Again, a statistically significant difference was found between the HbA1c values 2 years before the Pandemic and during the Pandemic, and between the HbA1c values 1 year before the Pandemic and during the Pandemic process, among those who had diet compliance in the 1 year before the Pandemic but did not comply with the diet during the Pandemic period (p<0.05). The difference between HbA1c values 2 years before the Pandemic and 1 year before the Pandemic and during the Pandemic process was statistically significant (p<0.05). A statistically significant difference was found between the HbA1c values 1 year before the Pandemic and during the Pandemic period (p<0.05). Compared to the 1 year before the Pandemic, the diet compliance, exercise compliance, and routine doctor follow-ups of the patients were disrupted during the Pandemic period (p<0.01), while the blood glucose measurements at home were more accurate during the Pandemic period (p<0.05). Diabetic peripheral neuropathy symptoms (burning, numbness, numbness, tingling, loss of strength, etc. in the hands or feet) were present in 99 (47.8%) of the patients and while there was no increase in these symptoms during the pandemic process, 55 (26.6%) ) never had these symptoms, 50 (24.2%) had symptoms and increased during the pandemic process (Table 6). There was no statistically significant difference in Plasma Urea, Plasma Creatinine, Fasting Plasma Glucose values and Spot urine Microalbumin positivity (≥30mg/l)(p>0.05). Conclusion: Our study revealed that the Covid-19 Pandemic disrupted the glycemic control of Diabetes Mellitus patients. Except for diabetic neuropathy, there was no evidence of the emergence and progression of diabetic complications.
