Özet

Amaç: Endoprostetik rekonstrüksiyon ile ekstremite koruyucu cerrahi yapılan, proksimal tibia ve distal femur yerleşimli tümörlerin; sağ kalım ve komplikasyon profillerini ortaya koymak, yumuşak doku rekonstrüksiyon yöntemlerinin muhtemel komplikasyonlara olan etkisi araştırmak amaçlanmıştır. Gereç ve Yöntem: 2011 ve 2019 yılları arasında MS Baltalimanı Eğitim ve Araştırma Hastanesi, kas iskelet tümörleri kliniğinde, diz çevresinde yerleşimli benign agresif ve malign tümörler nedeniyle rezeksiyon ve endoprotez ile rekonstrüksiyon yapılan hastalar incelendi. Verileri tam olan ve en az 36 ay takip süresi olan 80 hasta çalışmaya dahil edildi. Hastalardaki; yapılan yumuşak doku rekonstrüksiyon işlemleri, komplikasyonlar, komplikasyonların gelişme süresi, komplikasyonlar için uygulanan tedaviler, MSTS skorları, revizyon yapılanlar, implant ve hasta sağ kalımları, amputasyonlar retrospektif olarak taranarak not edildi. Veriler SPSS 26 programı ile istatistiksel olarak değerlendirildi. Bulgular: Hastaların 49'u erkekti (%61.3). Ortalama yaş: 31.05±15.09 idi. 64 hastanın tümörü malign (%80.0) ve 45 hastanın tanısı osteosarkomdu (%56.). Tümörlerin 48 tanesi femur distal yerleşimliydi (%60.0). Ortalama takip süresi: 54.35±26.10, ortalama MSTS skoru: 21.64±4.41 (%72.1) idi. Hastaların 40 tanesinde (%50.0) yumuşak doku rekonstrükisyonu yapıldı. Ekstansiyon lag derecelerinde PTR'de anlamlı derecede fark bulundu. (p: 0.019). YDR'de, Tip 3 komplikasyon gelişme süresi, stem sağ kalımı, fleksiyon derecelerinde ve ekstansiyon lag derecelerinde istatistiksel açıdan anlamlı farklılık tespit edildi (p: 0.030 ,p: 0.000 , p: 0.046, p:0.021). Stem sağ kalımı üzerinde; patellar tendonun proteze dikilmesinin, YDR yapılmayanlara kıyasla istatistiksel olarak anlamlı etkisi olduğu, (p: 0,000). Patellar tendonun proteze dikilmesinin; lokal gastro kas flebi uygulmasına ve prolen meş uygulamasına göre anlamlı etkisi olduğu (p: 0,001 ve p: 0,001). Ekstansiyon lag derecelerinin; prolen meş yapılanlarda YDR yapılmayanlara göre anlamlı olarak az olduğu bulundu (p: 0,021). 5 yıllık hasta sağ kalımı %59.0, 5 yıllık İmplant sağ kalımı %72.0 bulundu. Sonuçlar: Ekstremite koruyucu cerrahide, YDR komplikasyonları artırmamaktadır, sağ kalımları etkilememektedir. Ekstremite fonkisyonları üzerine olumlu etkisi vardır

Purpose: It was aimed to reveal survival and complications profiles of proximal tibial and distal femoral tumors, that underwent limb sparing surgery and endoprosthetic reconstruction and to investigate the effects of soft tissue reconstruction methods on possible complications. Matherial and Methods: Patients who underwent wide resection and reconstructed with tumor endoprosthesis dueo to benign aggressive and malign tumors located around knee region in musculoskeletal tumors clinic in Baltalimanı Bone Disease Hospital between 2011 and 2019 were retrospectively reviewed. 80 Patients with complate data and at least 36 months follow-up were included. Soft tissue reconstruction procedures, complications, complication times, treatments applied for complications, MSTS scores, revisions, implant and patient survivals, amputations were noted. The Datas were analyzed statistically with SPSS version 26. Results: 49 of patients was male (61.3%). The mean age was 31.05±15.09. The tumors of 64 patients had malignant (80.0%) and diagnosis of 45 patients was osteosarcoma (56.0%). Fourty-eight of tumors were located in distal femur (60.0%). Mean follow-up time was 54.35±26.10 months, mean MSTS score was 21.64±4.41( %72.1%). In 40 patients (50.0%) soft tissue reconstruction was performed. There was significant difference in extension lag degress in PTR group (p: 0.019) In the soft tissue reconstruction group statistically significant differences were found in the development time of Type 3 complication, stem survival, degrees of flexion and extension lag degress (p: 0.030 ,p: 0.000 , p: 0.046, p:0.021) On stem survival, patellar tendon reattachment on endoprosthesis had statistically significantly effect on complications compared to patients without soft tissue reconstruction (p: 0,000). Patellar tendon reattachment on endoprosthesis had statistically significantly effect on complication rates compared to local gastrocnemius rotation flep and prolene mesh applications (p: 0,001 ve p: 0,001). Extension lag degress was statistically significantly less compared to patients without soft tissue reconstruction (p: 0,021). 5 year patients survival was 59.0% and implant survival was %72.0 Conclusions: In limb sparin surgery, soft tissue reconstruction does not increase the complication and does not affect survivals. It has possitive effects on exrtemity functions
