Özet

Amaç: Erektör spina plan (ESP) bloğu paraspinal bölgeye uygulanan, visseral ve somatik analjezik etkinliği olan bir interfasial plan bloğudur. Çalışmamızda hemoroid cerrahisi geçiren hastalarda sakral ESP bloğunun postoperatif analjezik tüketimi üzerine olan etkinliğini araştırdık. Materyal ve Metod: Kahramanmaraş Sütçü İmam Üniversitesi Sağlık Uygulama ve Araştırma Hastanesi Klinik Araştırmalar ve Etik kurulunun ve hastanın onayı alınarak 18-65 yaş arası, ASA 1-3 olan, hemoroid cerrahisi planlanan 40 hasta çalışmamıza dahil edildi. Hastalar randomize olarak kontrol grubu (Grup Kontrol) ve ESP bloğu yapılan grup (Grup ESP) olarak iki gruba ayrıldı. ESP hasta grubuna, prone pozisyonunda, S4 vertebra seviyesinden in-plane teknikle, %0,25 bupivakainle 0,5ml/kg olacak şekilde ESP bloğu yapıldı. Kontrol grubu hastalara herhangi bir rejyonel anestezi tekniği uygulanmadı. Her iki grup hastaya postoperatif tramadol içeren hasta kontrollü analjezi parenteral yolla uygulandı. Operasyon sonrası 1, 2, 4, 6, 12 ve 24. saatlerde tramadol tüketimi ve Vizüel Analog Skala (VAS) skoru değerlendirildi. 24 saatin sonunda kullanılan analjezik miktarı, VAS skorları, kurtarma analjezisi, hasta ve cerrah memnuniyeti, yan etki gelişimi ile mobilizasyon saatleri kaydedildi. İstatistiksel analiz SPSS 22 programı ile; bağımsız gruplarda t testi, Mann-Whitney U testi ve Kikare analizi uygulanarak yapıldı. Bulgular: Postoperatif istirahat ve dinamik durumunda 6.saat ve 12.saat VAS skorları açısından gruplar arasında istatistiksel açıdan anlamlı fark vardı. ESP blok grubunda, kontrol grubuna göre VAS skorları daha düşüktü (istirahat ve dinamik sırasıyla, p<0.001, p<0.001, p<0.001, p<0.001). Postoperatif 24 saat tramadol tüketimleri açısından gruplar arasında istatistiksel açıdan fark vardı. ESP blok grubunda, kontrol grubuna göre postoperatif tramadol tüketimi daha düşüktü (p<0.001). Postoperatif bulantı, kusma, kaşıntı ve glob vezikale açısından gruplar arasında istatistiksel açıdan anlamlı bir fark görülmedi (p=0.342). Postoperatif kurtarma analjezisi ile hasta ve cerrah memnuniyeti açısından blok ve kontrol grubunda istatistiksel anlamlı fark vardı (sırayla p<0.001, p<0.001, p<0.001). Mobilizasyon saatleri, operasyon geçmişi ve ilaç kullanım öyküsü açısından gruplar arasında istatistiksel açıdan anlamlı bir fark görülmedi (sırasıyla, p=1, p=0.394, p=0.476). Çalışmamızda blok ve/veya cerrahi ilişkili herhangi bir komplikasyonla da karşılaşılmadı. Sonuç: Çalışmamızda multimodal analjezinin bir parçası olarak uygulanan sakral ESP bloğun, erişkin anorektal cerrahide, cerrahi sonrası ek analjezik ihtiyacını azalttığı ve etkin bir ağrı kontrolü sağladığı gösterilmiştir.

Objective: The Sacral Erector Spinae Plane (ESP) block is an interfascial plane block applied to the paraspinal region, known for its visceral and somatic analgesic efficacy. In our study, we investigated the effectiveness of sacral ESP block on postoperative analgesic consumption in patients who underwent hemorrhoid surgery. Material and Method: With the approval of the Kahramanmaraş Sütçü İmam University Health Application and Research Hospital Clinical Research and Ethics Committee and the consent of the patients, 40 patients between the ages of 18-65 with ASA scores of 1-3, who were scheduled for hemorrhoid surgery, were included in our study. The patients were randomly divided into two groups: the control group (Control Group) and the group in which the ESP block was performed (ESP Group). In the ESP patient group, the ESP block was performed in a prone position at the level of the S4 vertebra using an in-plane technique with 0.25% bupivacaine at a dose of 0.5 ml/kg. Regional anesthesia techniques were not applied to the control group patients. Both groups received patient-controlled analgesia with postoperative tramadol administered parenterally. Tramadol consumption and Visual Analog Scale (VAS) scores were evaluated at 1, 2, 4, 6, 12, and 24 hours postoperatively. The amount of analgesic used at 24 hours, VAS scores, rescue analgesia, patient and surgeon satisfaction, side effects, and mobilization times were recorded. Statistical analysis was performed using the SPSS 22 program with independent group t-tests, Mann-Whitney U tests, and Chi-square analysis. Results: The statistical analysis of the study revealed significant differences between the groups in terms of VAS scores at 6 hours and 12 hours in both the postoperative resting and dynamic states. In the ESP block group, VAS scores were lower than in the control group (respectively, p<0.001, p<0.001, p<0.001, p<0.001). There was a statistically significant difference between the groups in terms of tramadol consumption at 24 hours postoperatively. Tramadol consumption was lower in the ESP block group compared to the control group (p<0.001). There were no statistically significant differences between the groups in terms of postoperative nausea, vomiting, itching, and postoperative urinary retention (p=0.342). There were statistically significant differences between the block and control groups in terms of postoperative rescue analgesia, patient satisfaction, and surgeon satisfaction (respectively, p<0.001, p<0.001, p<0.001). There were no statistically significant differences between the groups in terms of mobilization times, surgical history, and medication use history (respectively, p=1, p=0.394, p=0.476). The study did not encounter any complications related to the block and/or surgery. Conclusion: In our study, it has been demonstrated that the sacral Erector Spinae Plane (ESP) block, when implemented as a part of multimodal analgesia, reduces the need for additional analgesics postoperatively in adult anorectal surgery and provides effective pain control.
