Özet

Amaç: Bu çalışma, geç postpartum dönemde uygulanan akupresurun sezaryen sonrası sırt ağrısına etkisini incelemek amacıyla yapıldı. Yöntem: Deneysel tipteki randomize kontrollü çalışma Haziran 2023-Kasım 2023 tarihleri arasında, Kocaeli Kandıra İlçe Sağlık Müdürlüğü'ne bağlı Kandıra Merkez Aile Sağlığı Merkezi'nde, aile hekimliği birimlerinde yürütüldü. Çalışmanın evrenini Kandıra Merkez Aile Sağlığı Merkezi'nde aile hekimliği birimlerine kayıtlı sezaryen doğum gerçekleştiren, sırt ağrısı yaşayan, primipar ve multipar, geç postpartum birinci hafta ve dördüncü haftada olan katılımcılar oluşturdu. Araştırmanın örnekleminin belirlenmesinde G-Power 3.1.9.2 programı kullanılmış olup olası veri kayıpları öngörülerek toplamda 70 katılımcı dahil edildi. (Akupresur grubu 35 katılımcı, kontrol grubu 35 katılımcı). Çalışmanın verileri araştırmacı tarafından konu ile ilgili literatür çerçevesinde hazırlanan“Katılımcı Bilgi Formu, Sırt Ağrısının Değerlendirildiği VAS (Visual Analog Scale), McGill Ağrı Anketi (Kısa Form), Bilgilendirilmiş Gönüllü Olur Formu”kullanılarak toplandı. Verilerin analizinde, IBM SPSS Statistic 29.0 (IBM Corp., Armonk, NY, USA) programı kullanıldı. Bulgular: Deney ve kontrol grubu sosyo-demografik ve obstetrik özellikler bakımından birbirine benzerdi (p>0,05). VAS puanı ile deney ve kontrol grupları incelendiğinde; birinci hafta uygulama öncesi gruplar arası anlamlılık yok iken (u=-0,165; p=0,869), uygulama sonrasında anlamlı fark bulundu (u=-6,059; p<0,001). Dördüncü hafta uygulama öncesi gruplar arasında anlamlılık yok iken (u=-0,495; p>0,05), dördüncü hafta uygulama sonrası anlamlılık saptandı (u=-6,963; p<0,001). McGill puanı ile deney ve kontrol grupları incelendiğinde; birinci hafta uygulama öncesi deney grubu ve kontrol grubunun karşılaştırılmasında istatistiksel olarak anlamlı fark bulunmazken (u=-0,330; p=0,741), birinci hafta uygulama sonrası gruplar arasında anlamlılık bulundu (u=-6,656; p<0,001). Dördüncü hafta uygulama öncesi anlamlılık bulunmazken (t=0,088; p=0,930), dördüncü hafta uygulama sonrası anlamlılık bulundu (u=-6,952; p<0,001). Kontrol grubunda zamana göre VAS ve McGill ağrı puan ortalamasında artma gözlenirken, deney grubunda ağrı puan ortalamasının birinci hafta uygulama sonrası azaldığı fakat dördüncü hafta uygulama öncesi tekrar artarak en yüksek seviyeye ulaştığı, dördüncü hafta uygulama sonrası azaldığı gözlenmiştir. dördüncü hafta uygulama öncesi hem deney hem kontrol grubunda VAS ve McGill sırt ağrısı puanının en yüksek seviyeye ulaştığı gözlendi. Sonuç: Geç postpartum dönemde uygulanan akupresurun sezaryen sonrası sırt ağrısını azaltmada etkili olduğu belirlendi.

Özet (Çeviri)

Purpose: This study was conducted to examine the effect of acupressure applied in the late postpartum period on back pain after cesarean section. Method: The experimental type randomized controlled study was conducted between June 2023 and November 2023 in family medicine units at Kandıra Central Family Medicine Center affiliated with Kocaeli Kandıra District Health Directorate. The population of the study consisted of participants registered in the family medicine units at Kandıra Central Family Medicine Center, who had a cesarean section, had back pain, were primiparous and multiparous, and were in the first and fourth weeks of postpartum. G-Power 3.1.9.2 program was used to determine the sample of the study, and a total of 70 participants were included, anticipating possible data loss. (Acupressure group 35 participants, control group 35 participants). The data of the study were collected using the“Participant Information Form, VAS (Visual Analog Scale for Evaluation of Back Pain), McGill Pain Questionnaire (Short Form), Informed Volunteer Consent Form”prepared by the researcher within the framework of the literature on the subject. IBM SPSS Statistic 29.0 (IBM Corp., Armonk, NY, USA) program was used to analyze the data. Findings: The experimental and control groups were similar to each other in terms of socio-demographic and obstetric characteristics (p>0.05). When the experimental and control groups are examined by VAS score; While there was no significance between the groups before the first week application (u=-0.165; p=0.869), a significant difference was found after the application (u=-6.059; p<0.001). While there was no significance between the groups before the fourth week application (u=-0.495; p>0.05), significance was detected after the fourth week application (u=-6.963; p<0.001). When the experimental and control groups are examined with McGill score; While there was no statistically significant difference comparing the experimental group and control group before the first week application (u=-0.330; p=0.741), a significance was found between the groups after the first week application (u=-6.656; p<0.001). While there was no significance before the application in the fourth week (t=0.088; p=0.930), significance was found after the application in the fourth week (u=-6.952; p<0.001). While an increase was observed in the VAS and McGill pain score average over time in the control group, it was observed that in the experimental group, the pain score average decreased after the first week of application, but increased again before the application in the fourth week and reached the highest level, and decreased after the fourth week of application. It was observed that VAS and McGill back pain scores reached the highest level in both the experimental and control groups before the fourth week of application. Conclusion: It was determined that acupressure applied in the late postpartum period was effective in reducing back pain after cesarean section.
