Özet
Amaç: Bu çalışmada meme kanseri cerrahisi geçiren olgularda uygulanan yüzeyel ve derin serratus anterior plan bloklarının post-operatif analjezik tüketimi, analjezik talepleri, ağrı skorları, hasta memnuniyeti ve bulantı kusma insidansı üzerine olan etkisini araştırmaktır. Materyal ve metod: Bu çalıma kontrollü bir olgu-kontrol çalınmasıdır. Araştırma Kayseri'de Erciyes Üniversitesi Tıp Fakültesi Hastanesi Merkezi Ameliyathanelerinde meme cerrahisi geçiren 18 yaş üstü, 60 hastada yapılmıştır. Hastalar preoperatif görüşmede değerlendirilmiş, araştırmanın amacı hakkında hasta yakınlarına ve hastalara bilgi verilerek izinleri alınmış ve bütün hastalara SAPB'nın nasıl yapıldığı anlatılmıştır. İşlemi kabul eden hastalar çalışmaya dahil edilmiştir. Her iki gruptaki hastalara EKG, kan basıncı, SpO2 monitörizasyonu tamamlandıktan sonra rutindeki gibi genel anestezi uygulanmıştır. Hastalar iki gruba ayırılarak genel anestezi sonrası cerrahiden önce yüzeyel ve derin SAPB yapılmıştır. Postoperatif ağrı kontrolü her iki gruptaki hastalarda aynı şekilde sağlanmıştır. Hastaların postoperatif 0. saatteki ağrıları ayılma ünitesinde, postoperatif 2., 6., 12., ve 24. saatlerdeki ağrıları genel cerrahi servisinde değerlendirilmiştir. Aynı zamanlarda hastaların kullandıkları analjezik miktarları, bulantı kusma insidansları ve memnuniyet skorları da takip edilmiştir. Bulgular: Gruplar arasında yaş ortalaması, cerrahi türü ve süresi açısından istatistiksel fark yoktu (p>0.05). Derin SAPB grubunda morfin tüketimi daha az olsa da bu fark istatistiksel olarak anlamlı değildi (p>0.05). Derin SAPB grubunda VAS skorları 0., 6., 12. ve 24. saatlerde istatistiksel olarak anlamlı daha düşük bulundu (p<0.05). Derin SAPB grubunda 12. ve 24. saatlerde HKA talep sayıları ve 6. saatteki ek analjezik ihtiyacı istatistiksel olarak daha düşüktü (p<0.05). Yine Derin SAPB grubunda hastanede kalış süresi ve antiemetik ihtiyacı istatistiksel olarak daha düşük bulundu (p<0.05). Sonuç: Meme cerrahisi geçiren hastalarda Derin SAPB uygulaması daha düşük VAS skorlarını sağlamakta, antiemetik ihtiyacını azaltmakta ve hastanede kalış süresini kısaltmaktadır. Bu özellikleri ile Derin SAPB'un multimodal analjezinin bir parçası olarak meme cerrahisi geçiren hastalarda etkin bir şekilde kullanılabileceğini göstermektedir.

Objective: This study aims to investigate the effects of superficial and deep serratus anterior plane blocks on postoperative analgesic consumption, analgesic demands, pain scores, patient satisfaction and the incidence of nausea and vomiting in patients undergoing breast cancer surgery. Material and method: This study is a controlled case-control study. The study was conducted in 60 patients over the age of 18 who underwent breast surgery in the Central Operating Rooms of Erciyes University Medical Faculty Hospital in Kayseri. The patients were evaluated in the preoperative interview, the patients' relatives and the patients were informed about the purpose of the study and their permissions were obtained, and all patients were told how SAPB was performed. Patients who accepted the procedure were included in the study. General anesthesia was applied to the patients in both groups as routine after ECG, blood pressure, and SpO2 monitoring were completed. The patients were divided into two groups and superficial and deep SAPB was applied before surgery after general anesthesia. Postoperative pain control was provided in the same way in both groups. The patients' pain at the 0th postoperative hour was evaluated in the recovery unit, and the pain at the 2nd, 6th, 12th, and 24th postoperative hours was evaluated in the general surgery ward. The amount of analgesics used by the patients, the incidence of nausea and vomiting, and satisfaction scores were also monitored at the same time. Results: There was no statistical difference between the groups in terms of mean age, type of surgery and duration (p>0.05). Although morphine consumption was lower in the Deep SAPB group, this difference was not statistically significant (p>0.05). VAS scores were statistically significantly lower in the Deep SAPB group at 0, 6, 12 and 24 hours (p<0.05). The number of PCA requests at 12 and 24 hours and the need for additional analgesics at 6 hours were statistically lower in the Deep SAPB group (p<0.05). Again, the length of hospital stay and the need for antiemetics were statistically lower in the Deep SAPB group (p<0.05). Conclusion: Deep SAPB application in patients undergoing breast surgery provides lower VAS scores, reduces the need for antiemetics and shortens the duration of hospital stay. These features show that Deep SAPB can be used effectively as a part of multimodal analgesia in patients undergoing breast surgery.
