Özet

Amaç: Kutanöz skuamöz hücreli karsinomların (KSHK) çoğunun prognozu mükemmel olsa da bazıları kötü seyretmektedir. Bu hastalığın prognozunu öngörmede bazı yüksek risk faktörleri tespit edilmiştir ve farklı evreleme sistemleri, bu yüksek risk faktörlerine dayalı olarak geliştirilmiştir. KSHK'de güvenilir ek risk faktörlerinin tanımlanması, prognozun daha iyi öngörülebilmesi için gereklidir. Çalışmamızda, pek çok kanser türünde prognostik ve prediktif değeri olan tümör tomurcuklanmasının KSHK'lerde prognostik parametreler ile ilişkisini araştırmayı amaçladık. Gereç ve Yöntem: Çalışmamız Sağlık Bilimleri Üniversitesi Antalya Eğitim ve Araştırma Hastanesi Patoloji Kliniği'nde 2022-2025 yılları arasında KSHK tanısı almış 73 materyalin dahil edildiği retrospektif bir çalışmadır. Hematoksilen ve Eozin (H&E) boyalı biyopsi preparatları mikroskopta (Olympus CX23, Japonya) tümör çapı, tümör derinliği, subkutan yağ doku invazyonu, Clark evresi, tümör farklılaşma derecesi, lenfovasküler invazyon (LVİ) varlığı, perinöral invazyon (PNİ) varlığı, tümörü infiltre eden lenfosit yoğunluğunu belirlemek amacı ile orijinal raporlardan bağımsız olarak yeniden değerlendirildi. Ayrıca yaş, cinsiyet ve tümör yerleşim yeri bilgilerine Sarus Bilgi Sistemi'nden ulaşılarak kaydedildi. Tümör tomurcuklanması (TT) için, 20x'lik büyütme objektifinde, tümörün en derin izlendiği alanın ötesinde, en fazla TT izlenen odak değerlendirilmiştir. TT, tümör tomurcuklarının sayısına göre 1'den 3'e kadar skorlanmış olup; skor 1: 0-4 tomurcuk, skor 2: 5-9 tomurcuk ve skor 3: 10 ve üzeri tomurcuk olarak belirlendi. Bulgular: Çalışmamızdaki olguların 41'inde (%56,2) 0-4 tomurcuk (skor I), 25'inde (%34,2) 5-9 tomurcuk (skor II), 7'sinde (%9,6) ise 10 ve üzeri tomurcuk (skor III) olduğu belirlendi. Tümör çapı ve tümör derinliği ile tümör tomurcuk sayısı arasında anlamlı bir ilişki saptandı (p<0.001). Subkutan yağ dokusu invazyonu olanlarda, olmayanlara göre tümör tomurcuk sayısı değerleri istatistiksel olarak anlamlı derecede yüksek saptandı (p<0.001). Clark evresi ile tümör tomurcuk sayısı arasında anlamlı bir ilişki tespit edildi (p=0.014). Perinöral invazyon olanlarda, olmayanlara göre tümör tomurcuk sayısı değerleri istatistiksel olarak anlamlı derecede yüksek saptandı ancak tümör tomurcuk skoru grupları dağılımı arasında anlamlı bir fark bulunmadı. Yaş, cinsiyet, tümör yerleşim yeri, tümörün farklılaşma derecesi ve tümörü infiltre eden lenfosit şiddeti ile tümör tomurcuklanma skoru grupları arasında anlamlı bir fark yoktu. Sonuç: Çalışmamızda KSHK'larda yüksek risk faktörü kabul edilen ve prognostik belirteç olarak kullanılan; tümör çapı, derinliği, subkutan yağ doku invazyonu, Clark evresi ile TT ilişkili olarak saptandı. Tüm prognostik faktörlerle tümör tomurcuklanması arasındaki ilişki hakkında net bir sonuç elde edilebilmek ve TT'yi KSHK raporlarına prognostik bir faktör olarak dahil edilebilirliğini belirlemek için daha geniş örneklemlerde bu konuda yapılacak ek çalışmalar gerekmektedir.

Aim: Although most cutaneous squamous cell carcinomas (CSCC) have an excellent prognosis, some have a poor prognosis. Some high-risk factors have been identified to predict the prognosis of this disease and different staging systems have been developed based on these high-risk factors. Identification of reliable additional risk factors in CSCC is necessary to better predict the prognosis. In our study, we aimed to investigate the relationship between tumor budding, which has prognostic and predictive value in many types of cancer, and prognostic parameters in CSCC. Materials and Methods: Our study is a retrospective study that included 73 materials diagnosed with KSCC between 2022-2025 at the Pathology Clinic of the Health Sciences University Antalya Education and Research Hospital. Hematoxylin and Eosin (H&E) stained biopsy preparations were re-evaluated under a microscope (Olympus CX23, Japan) independently of the original reports to determine tumor diameter, tumor depth, subcutaneous fat tissue invasion, Clark stage, tumor differentiation degree, presence of lymphovascular invasion (LVI), presence of perineural invasion (PVI), and tumor infiltrating lymphocyte density. In addition, age, gender, and tumor location information were recorded by accessing the Sarus Information System. For tumor budding (TT), the focus with the most TT beyond the area where the tumor was most deeply observed was evaluated under the 20x magnification objective. TT was scored from 1 to 3 according to the number of tumor buds; score 1: 0-4 buds, score 2: 5-9 buds and score 3: 10 and above buds. Results: In our study, 41 (56.2%) of the cases had 0-4 buds (score I), 25 (34.2%) had 5-9 buds (score II), and 7 (9.6%) had 10 or more buds (score III). A significant relationship was found between tumor diameter and tumor depth and tumor bud count (p<0.001). Tumor bud count values were found to be statistically significantly higher in patients with subcutaneous fat tissue invasion than in those without (p<0.001). A significant relationship was found between Clark stage and tumor bud count (p=0.014). Tumor bud count values were found to be statistically significantly higher in patients with perineural invasion than in those without, but no significant difference was found between the distribution of tumor bud score groups. There was no significant difference between the tumor bud score groups in terms of age, gender, tumor location, degree of differentiation of the tumor, and tumor infiltrating lymphocyte intensity. Conclusion: In our study, tumor diameter, depth, subcutaneous fat tissue invasion, and Clark stage, which are considered high-risk factors and used as prognostic markers in KSCC, were found to be associated with TT. Additional studies on this subject with larger samples are required to obtain a clear result about the relationship between all prognostic factors and tumor budding and to determine the inclusion of TT as a prognostic factor in KSCC reports.
