Özet

Amaç: Endobronşiyal ultrasonografi (EBUS) kılavuzluğunda transbronşiyal iğne aspirasyonu (TBİA) yapılan olgularda anestezi yönetiminde kullanılan supraglottik havayolu araçlarının (SHA) intraoperatif etkinlik ve postoperatif komplikasyonlar açısından karşılaştırmaktır. Gereç ve Yöntem: Etik kurul onayı ve çalışmaya katılmak için yazılı onamı alınan, EBUS-TBİA yapılan 18-80 yaş arası Amerikan Anestezistler Derneği (ASA) fizyolojik sınıfı I-III olan hastalar Klasik laringeal maske (LMA), Proseal LMA ve I-gel gruplarına randomize edildi. Yaş, cinsiyet, boy, ağırlık, ek hastalıklar, kullanılan ilaçlar ve anatomik havayolu ölçümleri kaydedildi. Preoperatif standart monitörizasyon ve bispektral indeks (BIS) monitörizasyonu yapıldı, hastalar uygun boyutta maske ile ekspiryum oksijen düzeyi ≥%90 hedeflenerek preoksijenize edildi. 1 mg/kg lidokain, 1mcg/kg fentanil, 2-3 mg/kg propofol ve 0,6 mg/kg rokuronyum ile anestezi indüksiyonu yapıldı. İki dakika maske ventilasyon sonrasında SHA yerleştirildi. SHA boyutu, anestezistin SHA'yı yerleştirme süresi, yerleştirme kolaylığı; pulmonoloğun EBUS probunu vokal kordlardan kaçıncı denemede geçirdiği, probun geçişi için ek havayolu açma manevrası gerekliliği, Brimacombe Skoru ile vokal kordların ilk görüntüsü kaydedildi. İntraoperatif hemodinamik veriler ve ventilasyon parametreleri 5 dk aralıklarla kaydedildi. İntraoperatif ve postoperatif komplikasyonlar kaydedildi. Bulgular: Her grupta 61 hasta olmak üzere toplam 183 hasta verisi analiz edildi. Gruplar arasında demografik özellikler benzer bulundu (p>0,05). Vokal kord görüntülenme zorluğu Klasik LMA grubunda Proseal LMA ve I-gel gruplarından yüksekti; sırasıyla %45,9, %13,1, %14,8 (p<0,01). Klasik LMA grubunda Proseal LMA ve I-gel grubuna göre EBUS probunun vokal kordlardan geçişi için ek manevra uygulanma oranı daha yüksekti; sırasıyla 2,46±1,88, 1,34±0,75, 1,38±0,9 (p<0,01). SHA'nın ilk denemede dirençsiz yerleştirilme oranı Klasik LMA grubunda Proseal LMA ve I-gel grubuna göre daha yüksektir; sırasıyla %73,8, %31,1, %41 (p<0,01). Orofarengeal kaçak basınçları Proseal LMA (27,25±2,95 cmH2O) ve I-gel (27,05±3,41cmH2O) gruplarında Klasik LMA (24,48±3,3 cmH2O) grubuna göre daha yüksek ölçülmüştür (p<0,01). Gruplar işlem süreleri ve perioperatif komplikasyonlar bakımından benzerdi (p=0,565; p>0,05). Sonuç: EBUS-TBİA yapılan hastalarda Klasik LMA daha kolay yerleştirilebilmesine rağmen, Proseal LMA ve I-gel'e kıyasla EBUS probunun vokal kordlardan geçişi için daha fazla girişim ve daha sık ek havayolu açma manevrası gerektirir ve vokal kordların görüntülenmesi daha zordur. EBUS-TBİA işlemlerinde Proseal LMA ve I-gel kullanımı ile bu girişim daha kolay yapılabilmekte, Klasik LMA işlemi güçleştirmektedir.

Objective: To compare the efficacy and complications of three different supraglottic airway devices (SAD), in patients undergoing endobronchial ultrasonography (EBUS)-guided transbronchial needle aspiration (TBNA). Materials and Methods: After obtaining ethics committee and written patient consent, American Society of Anesthesiologists (ASA) physiologic class I-III patients, aged 18-80 years who were to undergo EBUS-TBNA were randomized into Classical laryngeal mask (LMA), Proseal LMA and I-gel groups. Patient's age, gender, height, weight, comorbidities, medications and anatomical airway measurements were recorded. Bispectral index (BIS) and standard monitoring was used. Patients were preoxygenated until expired oxygen ≥90% was achieved, with face mask ventilation. Lidocaine 1mg/kg, fentanyl 1mcg/kg, propofol 2-3 mg/kg, rocuronium 0,6 mg/kg was used for induction, after 2 minutes of mask ventilation, SAD was placed. Mask ventilation difficulty, size and time required to place SAD, the ease of placement were recorded. The number of attempts required by the pulmonologist to pass the EBUS probe through the vocal cords, the need for additional airway maneuvers to facilitate probe passage, and the initial view of the vocal cords assessed using the Brimacombe Score were recorded. Intraoperative hemodynamic data and ventilation parameters were recorded at 5-minute intervals. Intraoperative and postoperative complications were recorded. Results: Data of 183 patients, 61 in each group, were analysed. Demographic characteristics were similar between groups (p>0.05). Visualization of the vocal cords was more difficult in Group Classic LMA (45.9%) compared to Proseal LMA (13.1%) and I-gel (14.8%) (p<0.01). The use of Classic LMA (2.46±1.88) required more advancement attempts to pass the EBUS probe through the trachea than the use of Proseal LMA (1.34±0.75) and I-gel (1.38±0.9) (p<0.01). The rate of easy (resistance-free) SAD placement was higher in Group Classic LMA (73.8%) compared to Group Proseal LMA (31.1%) and Group I-gel (41%), (p<0.01). Oropharyngeal leak pressures were higher in the Proseal LMA (27.25±2.95cmH2O) and I-gel (27.05±3.41 cmH2O) groups than in the Classic LMA (24.48±3.3 cmH2O) group (p<0.01). Procedure times were similar between the 3 groups (p:0.565). There was no significant difference between the groups in terms of perioperative complications (p>0.05). Conclusion: During EBUS-TBNA procedures although the use of Classic LMA provides easy insertion it requires more attempts to pass the EBUS probe through the vocal cords, additional airway opening maneuvers are performed more frequently, and visualization of the vocal cords is more difficult compared to the use of Proseal LMA and I-gel.
