Objective: The aim of this study was to compare the analgesic efficacy of intravenous ibuprofen and dexketoprofen in the management of acute pain due to vertebral process fractures and to evaluate their early analgesic effects. Materials and Methods: This prospective, randomized, controlled, single-blind clinical study included 100 patients who presented to the emergency department with vertebral process fractures and met the inclusion criteria. Patients were randomly allocated into two equal groups (n=50 each) to receive either intravenous ibuprofen (800 mg) or dexketoprofen (50 mg). Pain levels were assessed using the Visual Analog Scale (VAS) at baseline, 30, 60, and 120 minutes. The primary outcome measures were changes in VAS scores (ΔVAS and %ΔVAS). Secondary outcomes included the need for rescue analgesia and observed side effects. Statistical analyses were performed using appropriate parametric and non-parametric tests. Results: Both treatment groups showed statistically significant reductions in VAS scores after administration (p<0.001). However, the ibuprofen group demonstrated significantly lower VAS scores at 30, 60, and 120 minutes compared to the dexketoprofen group (p<0.05). The ibuprofen group also had greater ΔVAS and %ΔVAS values. Rescue analgesia was required in only 5 patients in the ibuprofen group, compared to 19 patients in the dexketoprofen group (p<0.01). No serious adverse events were reported in either group. Conclusion: Both ibuprofen and dexketoprofen were found to be safe and effective analgesics for acute pain associated with vertebral process fractures. However, ibuprofen demonstrated superior analgesic efficacy and reduced need for rescue analgesia in the early period. These results suggest that ibuprofen may be considered as a first-line option in the management of traumatic musculoskeletal pain.
