Özet
Giriş: 'Acil Serviste Üriner Kateter Takılıp Yataklı Servise Yatış Yapılan Hastalarda Kontaminasyon Kaynaklı Üreme' isimli tez çalışmasıdır. Acil serviste hastalara uygulan üriner kateterizasyonda asepsi kurallarına uyulmaması hastane kökenli enfeksiyona (HKE) neden olabilmektedir. Hastane kökenli enfeksiyonların önemli bir kısmını nazokomiyal üriner sistem enfeksiyonları (NÜSE) oluşturmaktadır. Amaç: Acil serviste asepsi kurallarına uygun üriner katater takılıp yataklı servislere yatış yapılan hastaların idrar kültüründe ve mikroskopilerinde işlem kaynaklı bir kontaminasyon veya bir patojen bakteri/mantar üremesinin olup olmadığının değerlendirilmesidir. Bu değerlendirmeden elde edilen sonuçlara göre acil serviste takılan üriner kataterler sonrası olası bir enfeksiyon durumu gözlendiğinde bunun önlenmesi için uygulayıcının eğitimi dahil çözümler üretmek ve literatüre katkı sağlamak amaçlanmaktadır. Gereç ve Yöntem: Çalışmamız 2024 yılında Sivas Cumhuriyet Üniversitesi Acil Servise başvurup üriner kateterizasyon sonrası yatış yapılan hastaları kapsamaktadır ve sisoft sistemi üzerinden retrospektif olarak yapılmıştır. Bulgular: 2024 yılında Cumhuriyet Üniversitesi Acil Servisi'nde üriner kateter (ÜK) takılarak yataklı servislere yatırılan toplam 72 hasta çalışmaya dahil edilmiştir. Hastaların yaş ortalaması 65,72 olup yaş aralığı 20-95'tir. %54,2'si erkek, %45,8'i kadındır. İlk idrar kültürlerinde %11,1 oranında üreme saptanmış, en sık izole edilen mikroorganizmalar Coryneform türleri olmuştur. Son kültürlerde ise üreme oranı %19,4'e yükselmiş, Staphylococcus spp., E. coli ve Enterococcus spp. gibi patojenler saptanmıştır. Kateter takılma saati 18:00-24:00 arasında yoğunlaşmış ve bu zaman aralığında üreme oranı yüksek olmasına rağmen istatistiksel olarak anlamlı bulunmamıştır. 18F sonda kullanılan hastalarda üreme oranı yüksek çıkmasına rağmen fark anlamlı değildir. Cinsiyete göre üreme farkı da istatistiksel olarak anlamlı değildir ancak kadınlarda daha yüksek üreme görülmüştür. Tüm kateterizasyon işlemleri intern doktorlar tarafından yapılmıştır. Sonuç: Acil serviste takılan üriner kateterler sonrası gelişen enfeksiyon oranları literatürle benzerlik göstermektedir. Enfeksiyon riskinin artmasında kateter süresi, hastanın durumu ve enfeksiyon kontrol önlemlerine uyum gibi faktörler rol oynamaktadır. Kateter çapı, biyokimyasal belirteçler ve yatılan servis gibi değişkenler tek başına anlamlı etkiler göstermemiştir. Enfeksiyon gelişimini önlemede: asepsi kurallarına uyum, uygun kateter çapı seçimi, gereksiz kateterizasyondan kaçınma ve işlem süresinin kısaltılması büyük önem taşımaktadır. Sağlık personelinin düzenli enfeksiyon kontrol eğitimi alması gerektiği vurgulanmaktadır.

Introduction: Urinary catheterization performed in the emergency department can lead to hospital-acquired infections (HAIs) if aseptic techniques are not followed. Among these infections, nosocomial urinary system infections (NUSI) are one of the most common and significant health problems in hospital settings. Urinary catheter procedures are frequently performed in patient care, and when aseptic principles are not adhered to, they can provide a suitable environment for the growth of pathogens such as bacteria and fungi. Therefore, evaluating the infection risk in patients following urinary catheterization in the emergency department and taking the necessary precautions is of vital importance. Aim: The aim of this study is to evaluate the urine cultures and microscopic examination results of patients who were admitted to the emergency department of Sivas Cumhuriyet University and had urinary catheterization followed by hospitalization in inpatient services, in order to determine whether there is any contamination or pathogen growth resulting from the procedure. Based on the findings of this study, we aim to increase compliance with aseptic techniques during urinary catheterization in the emergency department and propose solutions, including training programs for healthcare providers, to prevent potential infections. This research aims to contribute to the literature and provide practical solutions for preventing hospital-acquired infections. Materials and Methods: This retrospective study includes patients who were admitted to the emergency department of Sivas Cumhuriyet University Hospital in 2024 and underwent urinary catheterization followed by admission to inpatient services. The study was conducted retrospectively using the Sisoft system of Sivas Cumhuriyet University Hospital. The results of urine cultures and microscopy tests were evaluated to assess whether any infection developed following urinary catheterization. The study aims to examine the impact of properly performed urinary catheterization on the risk of infection. Findings: A total of 72 patients who had urinary catheters (UC) inserted in the emergency department and were subsequently admitted to inpatient wards at Cumhuriyet University in 2024 were included in the study. The mean age of patients was 65.72 years (range: 20–95). Of these, 54.2% were male and 45.8% were female. In the initial urine cultures, bacterial growth was observed in 11.1% of the cases, with Coryneform species being the most frequently isolated microorganisms. In the final cultures, the growth rate increased to 19.4%, with pathogens such as Staphylococcus spp., E. coli, and Enterococcus spp. identified. Most catheterizations occurred between 18:00–24:00, and although a higher growth rate was observed in this time frame, it was not statistically significant. Growth rates were higher in patients using 18F catheters, but the difference was not statistically significant. No statistically significant difference in culture growth was found between sexes, although females had slightly higher rates. All catheterization procedures were performed by intern doctors. Conclusion: The infection rates following urinary catheterization in the emergency department were consistent with the existing literature. Factors such as catheter duration, patient condition, and adherence to infection control measures contributed to infection risk. Variables such as catheter size, biochemical markers, and hospital ward were not independently significant. Preventing infections requires strict adherence to aseptic techniques, selecting appropriate catheter sizes, avoiding unnecessary catheterization, and minimizing the duration of catheter use. Regular infection control training for healthcare personnel is strongly recommended.
