Özet

Depresyon, kanser hastalarında sık görülen ve hastalık prognozunu olumsuz etkileyebilen önemli bir komorbiditedir. Klinik uygulamada kemoterapi ile birlikte antidepresan kullanımının yaygınlaşması, bu ilaçların kanser hücre biyolojisi üzerindeki etkilerinin moleküler düzeyde araştırılmasını gerekli kılmıştır. Seçici serotonin geri alım inhibitörlerinden (SSRI) sertralinin farklı kanser türlerinde sitotoksik etkiler gösterebildiği bilinmektedir; ancak küçük hücreli olmayan akciğer kanseri (KHDAK) hücrelerinde p53-bağımlı otofaji mekanizması üzerindeki rolü tam olarak aydınlatılmamıştır. Bu çalışmada sertralin ve Etopositin A549 (p53 yabanıl tip), H1299 (p53'ten yoksun) ve H1975 (p53 mutant) hücre hatlarındaki sitotoksik etkileri değerlendirilmiştir. WST-1 ve nötral kırmızısı testleri sonucunda 24 saatlik uygulama sonrası sertralinin IC₅₀ değerleri A549 hücre hattında 2,878 μg/ml, H1299 hücre hattında 1,995 μg/ml, H1975 hücre hattında 2,183 μg/ml ve Beas-2B hücre hattında 6,582 μg/ml olarak belirlenmiştir. Etoposit IC₅₀ değerleri ise A549 hücre hattında 198,275 μg/ml, H1299 hücre hattında 133,363 μg/ml, H1975 hücre hattında 91,697 μg/ml ve Beas-2B hücre hattında 99,64 μg/ml olarak belirlenmiştir. Gerçek zamanlı PCR analizlerinde AMPK, mTOR, BECN1, BAX, BCL2, P53, SESN1, SESN2, SESN3, SIRT1 ve LKB1 genleri değerlendirilmiştir. Pearson korelasyon analizlerinde; A549 hücrelerinde SESN1 gen ifadesi ile stres yanıt genlerinin ifadesi arasında pozitif ilişki gözlenirken, H1299 hücrelerinde SESN2-LKB1 ekseninde negatif korelasyon belirlenmiştir. H1975 hücrelerinde LKB1 ve BCL2 gen ifadeleri arasında pozitif ilişki saptanmıştır. Bulgular, stres yanıtının p53 fonksiyonel durumuna bağlı olarak farklı moleküler eksenler üzerinden düzenlendiğini göstermektedir. Elde edilen deneysel bulgular; GEPIA2, UALCAN ve CCLE veri tabanları kullanılarak akciğer adenokarsinomu (LUAD) doku verileri, p53 mutasyon durumu ve hücre hattı bazlı gen ekspresyon profilleri ile entegre edilmiştir. Ayrıca iLINCS transkriptomik verileri ve protein–protein etkileşim (PPI) ağ analizleri ile ilaç yanıtına eşlik eden moleküler ağlar değerlendirilmiştir. PPI ağ analizleri sonucu sertralin uygulaması ile p53, mTOR, BECN1, TSC2, RPTOR, SESN2, SQSTM1, PRKAA2 ve LKB1 genleri merkez genler olarak belirlenmiştir. Etoposit uygulamasında ise p53, FOXO3, BECN1, MTOR, BCL2, SESN2, TSC2, CDKN1A ve SESN1 genleri merkez gen olarak belirlenmiştir. Bulgular, sertralinin hücre hatlarında doğrudan sitotoksik bir etki yerine hücresel stres dengesini yeniden düzenleyen hücre hattına özgü yanıtlar oluşturduğunu; Etopositin ise DNA hasarı temelli daha klasik apoptotik yolakları aktive ettiğini göstermiştir. Deneysel korelasyon analizlerinde gözlenen hücre hattına özgü farklılıklar, LUAD doku verileri ve ağ analizleri ile birlikte değerlendirildiğinde, stres yanıtının sabit bir yolak yerine genetik arka plana bağlı olarak farklı moleküler düğümler üzerinden organize edildiğini ortaya koymuştur. Sonuç olarak bu çalışma, sertralinin KHDAK hücrelerinde sitotoksik etki oluşturduğunu ve hücresel stres yanıtını tekil bir doğrusal yolaktan ziyade p53 durumu ve genetik bağlama bağlı olarak yeniden organize edilen bir stres ağı üzerinden düzenlediğini ortaya koymaktadır. Deneysel gen paneli verilerinin klinik ve hücre hattı temelli biyoenformatik analizlerle bütünleştirilmesi, LUAD bağlamında SESN2 merkezli bir stres düzenleme modelini desteklemektedir.

Depression is a common and significant comorbidity in cancer patients that can negatively impact disease prognosis. The increasing use of antidepressants in combination with chemotherapy in clinical practice has necessitated investigations into the molecular-level effects of these drugs on cancer cell biology. It is known that sertraline, a selective serotonin reuptake inhibitor (SSRI), can exhibit cytotoxic effects in various cancer types; however, its role in the p53-dependent autophagy mechanism in non-small cell lung cancer (NSCLC) cells is not fully understood. In this study, the cytotoxic effects of sertraline and etoposide on A549 (p53 wt), H1299 (p53 null), and H1975 (p53 mutant) cell lines were evaluated. WST-1 and neutral red test results showed that after 24 hours of application, the IC₅₀ values of sertraline were 2878 ng/ml in the A549 cell line, 1,995 μg/ml in the H1299 cell line, 2,183 μg/ml in the H1975 cell line, and 6,582 μg/ml in the Beas-2B cell line. Etoposide IC₅₀ values were determined as 198,275 μg/ml in cell line A549, 133,363 μg/ml in cell line H1299, 91,697 μg/ml in cell line H1975, and 99.64 μg/ml in cell line Beas-2B. Real-time PCR analyses evaluated the genes AMPK, mTOR, BECN1, BAX, BCL2, P53, SESN1, SESN2, SESN3, SIRT1, and LKB1. Pearson correlation analyses showed a positive correlation between SESN1 and stress response genes in A549 cells, while a negative correlation was determined along the SESN2-LKB1 axis in H1299 cells. A positive correlation was found between LKB1 and BCL2 in H1975 cells. The findings indicate that the stress response is regulated via different molecular axes depending on the functional status of p53. The experimental findings were integrated with lung adenocarcinoma (LUAD) tissue data, P53 mutation status, and cell line-based gene expression profiles using GEPIA2, UALCAN, and CCLE databases. Furthermore, molecular networks associated with drug response were evaluated using iLINCS transcriptomic data and protein-protein interaction (PPI) network analyses. PPI network analyses identified p53, mtor, BECN1, TSC2, RPTOR, SESN2, SQSTM1, PRKAA2, and LKB1 as central genes for sertraline. For etoposide, p53, FOXO3, BECN1, MTOR, BCL2, SESN2, TSC2, CDKN1A, and SESN1 were identified as central genes. The findings demonstrated that sertraline elicits cell line-specific responses that re-regulate cellular stress balance rather than a direct cytotoxic effect, while etoposide activates more classical DNA damage-based apoptotic pathways. Cell line-specific differences observed in experimental correlation analyses, when evaluated together with LUAD tissue data and network analyses, revealed that the stress response is organized via different molecular nodes depending on the genetic background, rather than a fixed pathway. Consequently, this study demonstrates that sertraline exerts a cytotoxic effect in NSCLC cells and regulates the cellular stress response via a stress network that is reorganized depending on p53 status and genetic context, rather than a single linear pathway. The integration of experimental gene panel data with clinical and cell line-based bioinformatic analyses supports a SESN2-centric stress regulation model in the context of LUAD.
