Özet
Amaç: Timpanik membran perforasyonlarının cerrahi onarımında amaç, yalnızca perforasyonun kapatılması değil, aynı zamanda uzun dönem anatomik stabilitenin ve işitme fonksiyonunun korunmasıdır. Bu nedenle greft materyalinin biyolojik uyumu kadar biyomekanik özellikleri de cerrahi başarıda belirleyicidir. Kartilaj greftler, dayanıklılıkları nedeniyle özellikle riskli olgularda sık tercih edilmekle birlikte, eğilme, bükülme ve yer değiştirme gibi mekanik sorunlar gösterebilmektedir. Bu çalışmada, her iki yüzü perikondriumdan soyulmuş kartilaj ile perikondriyal greftlerin kombine kullanımına dayanan yeni bir timpanoplasti tekniğinin anatomik ve fonksiyonel sonuçları değerlendirildi ve bu yöntem iki farklı kartilaj timpanoplasti grubu ile karşılaştırıldı. Genel Bilgiler: Orta kulak ve timpanik membran, işitme fizyolojisinin temel bileşenleri olup ses iletiminin sağlanması ve orta kulak bütünlüğünün korunmasında kritik rol oynar. Timpanik membranın anatomik yapısı, ossiküler zincirle ilişkisi ve orta kulağın havalanma özellikleri, hem işitme fonksiyonunu hem de cerrahi onarımın başarısını doğrudan etkilemektedir. Timpanik membran perforasyonları ise bu bütünlüğü bozarak enfeksiyona yatkınlık, orta kulak mekanik iletiminde bozulma ve iletim tipi işitme kaybına yol açmaktadır; perforasyonun boyutu, yerleşimi ve eşlik eden yapısal özellikler işitme kaybının derecesini belirleyebilmektedir. Bu nedenle timpanoplasti cerrahisinde amaç yalnızca perforasyonun kapatılması değil, aynı zamanda orta kulak mekanik dengesinin yeniden kurulması ve tatmin edici işitsel sonuçların elde edilmesidir. Timpanoplasti teknikleri zaman içinde anatomik kapanmadan çok, uzun dönem stabilite ve fonksiyonel başarıyı önceleyen yaklaşımlara evrilmiştir. Bu süreçte temporal fasya, perikondriyum ve kartilaj en sık kullanılan greft materyalleri olarak öne çıkmış; özellikle kartilaj ve perikondriyum temelli yaklaşımlar, negatif orta kulak basıncına karşı dayanıklılıkları ve yüksek riskli olgulardaki stabilite avantajları nedeniyle önem kazanmıştır. Bu genel bilgiler, çalışmada değerlendirilen yeni tekniğin biyomekanik ve klinik temelini oluşturmaktadır. Gereç ve Yöntem: Bu tez, etik kurul onayı alınmış retrospektif karşılaştırmalı bir klinik çalışma olarak planlandı. Şubat 2019–Şubat 2025 tarihleri arasında kronik otitis media/perforasyon ve/veya kolesteatom nedeniyle opere edilen 391 hastanın klinik ve odyolojik kayıtları geriye dönük olarak incelendi; dahil edilme ve dışlanma kriterlerini karşılayan olgular çalışmaya alındı. Ana analizde, kolesteatom içermeyen ve timpanik membran rekonstrüksiyonu amacıyla timpanoplasti uygulanan hastalar kullanılan greft tekniğine göre üç gruba ayrıldı: iki tarafı soyulmuş kartilaj ve ayrı perikondriyal katman kullanılan grup, kartilaj ada greft grubu ve akordiyon kartilaj greft grubu. Ayrıca olgular, kartilajın hazırlanma şekline göre iki tarafı soyulmuş kartilaj kullanılanlar ile tek taraflı perikondriyum korunmuş kartilaj kullanılanlar şeklinde ikincil olarak da karşılaştırıldı. Kolesteatom nedeniyle mastoidektomi uygulanan hastalar ise ayrı bir kohort olarak değerlendirildi. Tüm olgularda demografik, klinik, cerrahi ve odyolojik veriler kaydedildi. Anatomik başarı; greft bütünlüğünün korunması ve perforasyon, belirgin retraksiyon, lateralizasyon ya da persistan otorenin olmaması şeklinde tanımlandı. Fonksiyonel başarı ise postoperatif ABG 20 dB ve altında olması olarak kabul edildi. Sonuçlar ayrıca MERI ve CES skorları ile desteklenerek değerlendirildi; istatistiksel analizlerde uygun parametrik ve non-parametrik testler ile lojistik regresyon analizi kullanıldı. Bulgular: Çalışma döneminde taranan 391 hastanın 281'i çalışmaya alındı ve bunların 228'i ana karşılaştırmalı analize dahil edildi. Gruplar yaş ve cinsiyet açısından benzerdi. Postoperatif değerlendirmede, iki tarafı soyulmuş kartilaj ve ayrı perikondriyal katman kullanılan grupta anatomik başarı en yüksek bulundu (%98,4); bu oran kartilaj ada greft ve akordiyon kartilaj greft gruplarında sırasıyla %87,0 ve %86,3 idi. Buna karşılık fonksiyonel başarı ve postoperatif odyolojik sonuçlar özellikle akordiyon kartilaj greft grubunda daha iyi izlendi. Bununla birlikte, her üç grupta da ameliyat sonrası işitsel parametrelerde ve hasta bildirimi sonuçlarında anlamlı iyileşme saptandı. Anatomik başarısızlık; geçirilmiş operasyon öyküsü, postoperatif enfeksiyon, yüksek MERI skoru ve kemikçik zinciri onarımı gereksinimi ile ilişkili bulunurken, fonksiyonel başarısızlık; preoperatif ve postoperatif daha yüksek ABG ve AC PTA değerleri, yüksek MERI skoru ve ossiküler rekonstrüksiyon gereksinimi ile ilişkiliydi. Alt grup ve ek analizler de, iki tarafı soyulmuş kartilaj tekniğinin anatomik stabilite açısından daha güçlü; buna karşılık tek taraflı perikondriyum korunmuş tekniklerin fonksiyonel sonuçlar açısından daha avantajlı olabileceğini düşündürdü. Sonuç: Bu çalışmada, iki tarafı soyulmuş kartilaj ve perikondriyum tekniğinin özellikle anatomik başarı açısından üstün sonuçlar verdiği gösterildi. Total ve subtotal perforasyonun bu grupta yüksek bulunması ve preoperatif daha yüksek ABG ve AC PTA değerleri, yüksek MERI skoru bulgularına rağmen bu grupta anatomik başarının daha yüksek bulunması, tekniğin özellikle riskli kulaklarda etkili bir seçenek olabileceğini düşündürdü. Fonksiyonel sonuçların anatomik başarı ile tamamen paralel olmaması ise işitsel sonucun yalnızca greft tekniğine değil, orta kulak patolojisinin derecesi ve kemikçik zincirin durumuna da bağlı olduğunu gösterdi. Sonuç olarak bu teknik, özellikle anatomik başarının ön planda olduğu olgularda güvenilir bir rekonstrüksiyon yöntemi olarak değerlendirilebilir.

Objective: In the surgical repair of tympanic membrane perforations, the aim is not only closure of the perforation, but also preservation of long-term anatomical stability and hearing function. For this reason, not only the biological compatibility of the graft material but also its biomechanical properties are determinant in surgical success. Although cartilage grafts are frequently preferred especially in risky cases because of their durability, they may show mechanical problems such as bending, curling, and displacement. In this study, the anatomical and functional results of a new tympanoplasty technique based on the combined use of cartilage stripped of perichondrium on both sides and perichondrial grafts were evaluated, and this method was compared with two different cartilage tympanoplasty groups. General Information: The middle ear and tympanic membrane are the main components of hearing physiology and play a critical role in the transmission of sound and the preservation of middle ear integrity. The anatomical structure of the tympanic membrane, its relationship with the ossicular chain, and the aeration characteristics of the middle ear directly affect both hearing function and the success of surgical repair. Tympanic membrane perforations disrupt this integrity, causing susceptibility to infection, deterioration in the mechanical transmission of the middle ear, and conductive hearing loss; the size, location, and accompanying structural features of the perforation may determine the degree of hearing loss. Therefore, in tympanoplasty surgery, the aim is not only closure of the perforation, but also re-establishment of middle ear mechanical balance and achievement of satisfactory auditory results. Tympanoplasty techniques have evolved over time from approaches focused on anatomical closure to approaches prioritizing long-term stability and functional success. In this process, temporalis fascia, perichondrium, and cartilage have come to the forefront as the most commonly used graft materials; cartilage- and perichondrium-based approaches in particular have gained importance because of their resistance to negative middle ear pressure and their stability advantages in high-risk cases. This general information constitutes the biomechanical and clinical basis of the new technique evaluated in the study. Materials and Methods: This thesis was planned as a retrospective comparative clinical study with ethics committee approval. The clinical and audiological records of 391 patients who were operated on for chronic otitis media/perforation and/or cholesteatoma between February 2019 and February 2025 were retrospectively reviewed; cases meeting the inclusion and exclusion criteria were included in the study. In the main analysis, patients without cholesteatoma who underwent tympanoplasty for tympanic membrane reconstruction were divided into three groups according to the graft technique used: the group in which bilaterally stripped cartilage and a separate perichondrial layer were used, the cartilage island graft group, and the accordion cartilage graft group. In addition, the cases were also secondarily compared according to the method of cartilage preparation as those in whom bilaterally stripped cartilage was used and those in whom cartilage with unilateral preserved perichondrium was used. Patients who underwent mastoidectomy because of cholesteatoma were evaluated as a separate cohort. Demographic, clinical, surgical, and audiological data were recorded in all cases. Anatomical success was defined as preservation of graft integrity and the absence of perforation, significant retraction, lateralization, or persistent otorrhea. Functional success was accepted as a postoperative ABG of 20 dB or less. The results were also evaluated supported by MERI and CES scores; appropriate parametric and non-parametric tests and logistic regression analysis were used in the statistical analyses. Results: Of the 391 patients screened during the study period, 281 were included in the study and 228 of these were included in the main comparative analysis. The groups were similar in terms of age and sex. In the postoperative evaluation, anatomical success was found to be highest in the group in which bilaterally stripped cartilage and a separate perichondrial layer were used (98.4%); this rate was 87.0% and 86.3% in the cartilage island graft and accordion cartilage graft groups, respectively. In contrast, functional success and postoperative audiological results were observed to be better especially in the accordion cartilage graft group. However, significant improvement was detected in postoperative auditory parameters and patient-reported outcomes in all three groups. Anatomical failure was found to be associated with a history of previous surgery, postoperative infection, high MERI score, and the need for ossicular chain repair, whereas functional failure was associated with worse preoperative and postoperative audiological values, high MERI score, and the need for ossicular reconstruction. Subgroup and additional analyses also suggested that the bilaterally stripped cartilage technique is stronger in terms of anatomical stability; in contrast, techniques with unilateral preserved perichondrium may be more advantageous in terms of functional results. Conclusion: In this study, it was shown that the bilaterally stripped cartilage and perichondrium technique gave superior results especially in terms of anatomical success. Despite the higher rates of total and subtotal perforations, poorer preoperative hearing parameters as reflected by higher ABG and AC PTA values, and higher MERI scores, anatomical success was greater in this group. This finding suggests that the technique may represent an effective surgical option, particularly for high risk ears.The fact that the functional results were not completely parallel with anatomical success showed that the auditory result depends not only on the graft technique but also on the degree of middle ear pathology and the status of the ossicular chain. As a result, this technique can be considered a reliable reconstruction method, especially in cases in which anatomical stability is at the forefront.
