Özet
Giriş: Postoperatif batın içi yapışıklıklar (adezyonlar), abdominal cerrahi sonrası mortalite ve morbiditesi yüksek durumlar olan bağırsak tıkanıklıkları gibi ciddi sekellere yol açan yaygın bir komplikasyondur. Yapışıklık oluşumu, cerrahi travma sonrası koagülasyon ve fibrinoliz arasındaki dengenin bozulması, inflamasyonun aktivasyonu ve fibrozis ile karakterizedir. Bu çalışmanın temel amacı, antioksidan, anti-inflamatuvar ve yara iyileştirici özellikleri geleneksel tıpta bilinen Anchusa strigosa (Sığırdili) bitkisinin etanol ekstraktının, postoperatif yapışıklık oluşumu üzerindeki olası düzenleyici ve önleyici etkilerini araştırmaktır. Materyal ve Metot: Çalışmada, rastgele ve eşit sayıda 3 gruba ayrılan toplam 30 adet Wistar albino cinsi erkek rat kullanılmıştır. Tüm ratlar cerrahi işlem öncesinde 6 saat aç bırakılmış ve anestezi altına alınmıştır. Cerrahi prosedür ardından çekumun serozal yüzeyinin gazlı bezle ovuşturularak postoperatif yapışıklık modelinin oluşturulmasını içermiştir. 3 grup şeklinde ayrılan ratların birinci grubuna müdehale edilmedi, ikinci gruba serum fizyolojik ve üçüncü gruba ekstrakt uygulandı. Denekler 21 gün sonra yüksek doz anestezi ile sakrifiye edilmiş ve yapışıklık kopma direnci, yaygınlık ve görünümlerine göre Modifiye Diamond Skalası ile adezyonlar kantitatif olarak değerlendirilmiştir. Adezyon gelişen/gelişmeyen bölgelerden alınan örnekler, histopatolojik inceleme ve grupların istatistiksel karşılaştırılması için kullanılmıştır. Bulgular: Çalışma, Kontrol Grubu'ndan (Grup 1) ölüm gözlenmezken, Serum Fizyolojik Grubu (Grup 2) ve Anchusa Strigosa Grubu'ndan (Grup 3) ikişer ratın çalışma dışı bırakılmasıyla toplam 26 rat üzerinden tamamlanmıştır. Ratlarda saptanan ortalama inflamasyon, vaskülarite ve fibrozis skorları sırasıyla 1,692±0,8376, 1,88±0,65 ve 1,65±0,89 olarak belirlenmiştir. Gruplar arasında yapılan histopatolojik karşılaştırmalarda anlamlı farklılıklar gözlenmiştir (Sırasıyla p=0,002, 0,003, <0,001). Sonuç: Anchusa strigosa'yı postoperatif adezyonların önlenmesi için biyolojik temelli potansiyel bir alternatif ajan olarak işaret etmektedir.

Introduction: Postoperative intra-abdominal adhesions are a prevalent complication that can result in significant sequelae, including intestinal obstruction. This condition is associated with a high mortality and morbidity rate following abdominal surgery. The process of adhesion formation is characterized by an imbalance between coagulation and fibrinolysis following surgical trauma, activation of inflammation, and fibrosis. The primary objective of this study is to examine the prospective regulatory and preventive impacts of the ethanol extract of Anchusa strigosa (Sığırdili), a plant recognized in traditional medicine for its antioxidant, anti-inflammatory, and wound-healing properties, on the development of postoperative adhesions. Methods: In the study, a total of 30 male Wistar albino rats were randomly divided into three equal groups. Prior to undergoing surgery, all rats were subjected to a 6-hour fast and were anesthetized. The surgical procedure entailed the creation of a postoperative adhesion model, which was achieved by applying pressure to the serosal surface of the caecum using a gauze pad. The rats were divided into three groups: the first group received no intervention, the second group received physiological saline, and the third group received the extract. The subjects were sacrificed 21 days later under high-dose anesthesia, and the adhesions were quantitatively evaluated using the Modified Diamond Scale according to their adhesion rupture resistance, extent, and appearance. Samples were obtained from regions exhibiting and non-exhibiting adhesion development for the purpose of histopathological examination and statistical comparison of the groups. Results: The study was conducted on a total of 26 rats, with no mortalities observed in the Control Group (Group 1). However, two rats each from the Physiological Serum Group (Group 2) and the Anchusa Strigosa Group (Group 3) were excluded from the study. The mean inflammation, vascularity, and fibrosis scores detected in the rats were determined to be 1.692 ± 0.8376, 1.885 ± 0.6528, and 1.654 ± 0.8918, respectively. A substantial discrepancy was identified in the histopathological evaluations conducted on the subjects within each group (Respectively p=0,002, 0,003, <0,001). Conclusion: Anchusa strigosa has been identified as a promising biological alternative agent for the prevention of postoperative adhesions.
