Özet
Giriş ve Amaç: Kıkırdak dokunun mevcut eğriliği, şekil hafızası ve biyomekanik özellikleri yıllardır; rinoplasti, otoplasti ve rekonstrüktif cerrahi pratiğinin bir parçası olmuştur. Geleneksel kıkırdak şekillendirme yöntemlerinden biri olan skorlama tekniği etkinliği kanıtlanmış bir yöntem olarak kullanılmakla birlikte, uygulama derinliği ve cerrah deneyiminden etkilenebilmesi nedeniyle standardizasyon açısından sınırlılıklar taşımaktadır. Bu çalışmada, kıkırdak eğriliğinin kontrolünde kullanılmak üzere geliştirilen yeni standardize perforasyon tekniğinin biyomekanik ve histopatolojik etkilerinin, geleneksel skorlama yöntemi ve kontrol grubu ile karşılaştırılması amaçlandı. Gereç ve Yöntem: Çalışmada 15 adet New Zealand cinsi dişi tavşan kullanıldı. Denekler kontrol, skorlama ve perforasyon grubu olmak üzere üç gruba ayrıldı. Her denekten elde edilen auriküler kıkırdak örnekleri standart ölçülerde hazırlanarak kalınlıklarına göre proksimal, orta ve distal olmak üzere A, B ve C isimli segmentlere ayrıldı. Kontrol grubundaki örneklere ek işlem uygulanmazken, skorlama grubunda kıkırdağın içbükey yüzeyine kısmi kat paralel insizyonlar yapıldı. Perforasyon grubunda ise çalışma kapsamında geliştirilen ve patent başvurusu yapılan, hexagonal dizilimli perforatör uçlara sahip özel cerrahi alet ile kıkırdak dokuda standartize perforasyonlar oluşturuldu. Tüm örnekler düz pozisyonda stabilize edilerek deneklerin sırt bölgesinde oluşturulan subkutan poşlara yerleştirildi. Altı haftalık takip süresi sonunda örnekler çıkarıldı. Mekanik test uygulanabilecek A ve B segmentleri üç nokta eğme testi ile değerlendirildi; C segmentleri ise doğrudan histopatolojik incelemeye ayrıldı. Biyomekanik incelemeler kontrol grubunda 6, skorlama grubunda 6 ve perforasyon grubunda 10 örnek üzerinde gerçekleştirildi. Histopatolojik değerlendirmeler ise kontrol grubunda 9, skorlama grubunda 12 ve perforasyon grubunda 15 örnek üzerinden yapıldı. Biyomekanik değerlendirmede konveksite indeksi, 4 mm ve 5 mm deplasmandaki kuvvet değerleri incelendi. Histopatolojik olarak kapsül varlığı, viable kondrosit yoğunluğu, neovaskülarizasyon, fibrozis, CD44 ile kıkırdak viabilitesi ve Col2a1 ile kondrogenezis değerlendirildi. Bulgular: Kontrol grubunda konveksite indeksi %25,17 ± 3,02, skorlama grubunda %6,69 ± 4,14 ve perforasyon grubunda %8,73 ± 1,97 olarak bulundu. Dört mm deplasmandaki kuvvet değerleri kontrol, skorlama ve perforasyon gruplarında sırasıyla 0,874 ± 0,30 N, 0,163 ± 0,16 N ve 0,308 ± 0,09 N idi. Beş mm deplasmandaki kuvvet değerleri ise sırasıyla 1,150 ± 0,43 N, 0,189 ± 0,17 N ve 0,400 ± 0,11 N olarak saptandı. Gruplar arası istatistiksel karşılaştırmada 4 mm ve 5 mm kuvvet değerleri, deplasman mesafesi ve konveksite indeksi açısından kontrol grubu ile skorlama ve perforasyon grupları arasında anlamlı farklılık izlendi (p<0.05). Buna karşın skorlama ve perforasyon grupları arasında anlamlı farklılık saptanmadı. Histopatolojik değerlendirmede kapsül oluşumu tüm gruplarda benzer özellik gösterdi. Viable kondrosit yoğunluğu ve CD44 ile değerlendirilen kıkırdak viabilitesi açısından kontrol ve perforasyon gruplarında benzer skorlar izlendi. Skorlama grubunda viable kondrosit yoğunluğu ve CD44 skorlarında daha geniş dağılım dikkati çekti. Neovaskülarizasyon skorlama grubunda daha yüksek skorlarla izlenirken, Col2a1 ile değerlendirilen kondrogenezis perforasyon ve skorlama gruplarında sınırlı düzeyde saptandı. Sonuç: Bu çalışma, geliştirilen standardize perforasyon tekniğinin kıkırdak eğriliğini azaltmada etkili bir biyomekanik değişim oluşturabildiğini göstermektedir. Perforasyon yöntemi, kontrol grubuna göre anlamlı mekanik farklılık oluşturmuş; ancak skorlama yöntemi ile karşılaştırıldığında istatistiksel olarak üstünlük göstermemiştir. Buna rağmen perforasyon grubunda konveksite indeksinin daha dar aralıkta dağılım göstermesi ve histopatolojik olarak kontrol grubuna daha yakın viable kondrosit yoğunluğu ile CD44 skorlarının izlenmesi, bu yöntemin daha kontrollü ve doku bütünlüğünü daha fazla koruyabilecek bir yaklaşım olabileceğini düşündürmektedir. Çalışma kapsamında geliştirilen perforasyon aleti, kıkırdak şekillendirmede cerraha bağlı değişkenliği azaltmayı hedefleyen, uygulanabilir ve standardize bir alternatif olarak değerlendirilebilir. Bununla birlikte, yöntemin klinik kullanıma aktarılabilmesi için daha geniş örneklemli, uzun dönem takipli ve farklı kıkırdak tipleri üzerinde yapılacak ileri çalışmalara ihtiyaç vardır.

Introduction and Aim: The inherent curvature, shape memory, and biomechanical properties of cartilage tissue have long been an integral part of rhinoplasty, otoplasty, and reconstructive surgical practice. Although the scoring technique, one of the conventional cartilage reshaping methods, has been used as a proven effective method, it has limitations in terms of standardization due to its susceptibility to application depth and surgeon experience. In this study, it was aimed to compare the biomechanical and histopathological effects of a newly developed standardized perforation technique for the control of cartilage curvature with the conventional scoring method and the control group. Materials and Methods: Fifteen female New Zealand rabbits were used in the study. The subjects were divided into three groups as control, scoring, and perforation groups. Auricular cartilage specimens obtained from each subject were prepared in standardized dimensions and divided into proximal, middle, and distal segments designated as A, B, and C according to their thickness. No additional procedure was applied to the specimens in the control group, whereas partial-thickness parallel incisions were made on the concave surface of the cartilage in the scoring group. In the perforation group, standardized perforations were created in the cartilage tissue using a specially designed surgical instrument developed within the scope of the study and submitted for patent application, featuring hexagonally arranged perforator tips. All specimens were stabilized in a flattened position and placed into subcutaneous pockets created on the dorsal region of the subjects. The specimens were harvested after a six-week follow-up period. A and B segments, which were suitable for mechanical testing, were evaluated using a three-point bending test, whereas C segments were directly allocated for histopathological examination. Biomechanical analyses were performed on 6 specimens in the control group, 6 specimens in the scoring group, and 10 specimens in the perforation group. Histopathological evaluations were conducted on 9 specimens in the control group, 12 specimens in the scoring group, and 15 specimens in the perforation group. In the biomechanical evaluation, convexity index and force values at 4 mm and 5 mm displacement were analyzed. Histopathological assessments included capsule formation, viable chondrocyte density, neovascularization, fibrosis, cartilage viability with CD44, and chondrogenesis with Col2a1. Results: The convexity index was found to be 25.17% ± 3.02 in the control group, 6.69% ± 4.14 in the scoring group, and 8.73% ± 1.97 in the perforation group. Force values at 4 mm displacement were 0.874 ± 0.30 N, 0.163 ± 0.16 N, and 0.308 ± 0.09 N in the control, scoring, and perforation groups, respectively. Force values at 5 mm displacement were determined as 1.150 ± 0.43 N, 0.189 ± 0.17 N, and 0.400 ± 0.11 N, respectively. In the intergroup statistical comparison, significant differences were observed between the control group and the scoring and perforation groups in terms of 4 mm and 5 mm force values, displacement distance, and convexity index (p<0.05). In contrast, no significant difference was detected between the scoring and perforation groups. In the histopathological evaluation, capsule formation showed similar characteristics across all groups. Similar scores were observed in the control and perforation groups in terms of viable chondrocyte density and cartilage viability evaluated with CD44. A wider distribution in viable chondrocyte density and CD44 scores was noted in the scoring group. Neovascularization was observed with higher scores in the scoring group, whereas chondrogenesis evaluated with Col2a1 was detected at a limited level in the perforation and scoring groups. Conclusion: This study demonstrates that the developed standardized perforation technique can produce an effective biomechanical alteration in reducing cartilage curvature. The perforation method created a significant mechanical difference compared to the control group; however, it did not demonstrate statistical superiority when compared with the scoring method. Nevertheless, the narrower distribution of the convexity index in the perforation group and the histopathologically observed viable chondrocyte density and CD44 scores that were closer to those of the control group suggest that this method may represent a more controlled approach with greater preservation of tissue integrity. The perforation instrument developed within the scope of the study may be considered a feasible and standardized alternative aimed at reducing surgeon-dependent variability in cartilage reshaping. However, further studies with larger sample sizes, long-term follow-up, and different cartilage types are needed in order to translate this method into clinical practice.
