Özet

ÖZET Giriş ve Amaç: Rinoplasti, fonksiyonel ve estetik amaçlarla sık uygulanan bir cerrahi olup, postoperatif dönemde uyanma ajitasyonu (UA) gelişme riski yüksektir. Nazal tamponlara bağlı tıkanıklık hissi, ödem ve sekresyonlar ajitasyonu tetikleyerek hem hasta konforunu hem de cerrahi sonuçları olumsuz etkileyebilir. UA'nın önlenmesi bu nedenle klinik açıdan önemlidir. Farmakolojik yönetimde kortikosteroidler, antiinflamatuvar ilaçlar ve sedatif özellikleriyle propofol ile deksmedetomidin öne çıkmaktadır. Pediatrik hastalarda propofolün UA azaltıcı etkisine dair güçlü kanıtlar bulunmakla birlikte, erişkinlerde veriler sınırlıdır. Deksmedetomidin ise yetişkin ve çocuklarda ajitasyonu azaltmada etkinliği gösterilmiş bir ajandır. Literatürde pediatrik karşılaştırma çalışmaları bulunmasına rağmen yetişkin popülasyonda karşılaştırmalı veri eksikliği mevcuttur. Bu çalışma, rinoplasti geçiren yetişkinlerde ekstübasyon öncesi uygulanan tek doz deksmedetomidin ile subanestezik doz propofolün uyanma ajitasyonu, hasta memnuniyeti, uyanma kalitesi, derlenme süresi, hemodinamik yanıtlar ve yan etkiler üzerine etkilerini karşılaştırmayı amaçlamaktadır. Gereç ve Yöntemler: ASA I–II sınıfı, 18–65 yaş arası 80 hasta rastgele iki gruba ayrıldı. Propofol grubuna cerrahi bitimine 10 dakika kala 0,5 mg/kg propofol, deksmedetomidin grubuna ise 0,5 µg/kg deksmedetomidin 10 dakikalık İV infüzyonla uygulandı. Standart anestezi protokolü uygulandı. Hemodinamik parametreler intraoperatif ve postoperatif belirli zamanlarda kaydedildi. UA, ekstübasyon sonrası 1. dakikada RASS ile değerlendirildi; hasta memnuniyeti taburculuk öncesi 4'lü Likert ölçeği ile ölçüldü. Bulgular: Deksmedetomidin grubunda derlenme süresi anlamlı olarak daha uzun bulundu (p<0.05). RASS skorları ve ajitasyon bulguları propofol grubuna göre daha düşük seyretti (p<0.05). İntraoperatif ve postoperatif ilk 15 dakikada hemodinamik değerler deksmedetomidin grubunda daha stabil ve daha düşük izlendi (p<0.05). Yan etki profilleri arasında anlamlı fark yoktu (p>0.05). Hasta memnuniyeti deksmedetomidin lehine belirgin daha yüksekti (p<0.05). Sonuç: Deksmedetomidin, propofole göre UA'yı daha etkin azaltmış, hemodinamik stabiliteyi artırmış ve hasta memnuniyetini yükseltmiştir; ancak derlenme süresini uzatmıştır. Hızlı derlenmenin önemli olduğu olgularda propofol uygun bir seçenek olabilir. Rinoplasti gibi UA riskinin yüksek olduğu cerrahilerde deksmedetomidin daha avantajlı görünmektedir.

ABSTRACT Introduction and Aim: Rhinoplasty is a commonly performed surgical procedure for both functional and aesthetic purposes; however, it carries a high risk of postoperative emergence agitation (EA). The sensation of nasal obstruction caused by tampons, along with edema and secretions, can trigger agitation, thereby negatively affecting patient comfort and surgical outcomes. Preventing EA is therefore of significant clinical importance. Among pharmacological strategies, corticosteroids, anti-inflammatory agents, and sedative medications such as propofol and dexmedetomidine play a prominent role. Although strong evidence supports the EA-reducing effect of propofol in pediatric patients, data in adults remain limited. Dexmedetomidine, on the other hand, has demonstrated efficacy in reducing agitation in both pediatric and adult populations. While comparative studies exist in children, evidence in adult patients is insufficient. This study aimed to compare the effects of a single pre-extubation dose of dexmedetomidine with a subanesthetic dose of propofol on emergence agitation, patient satisfaction, quality of emergence, recovery time, hemodynamic responses, and adverse events in adult patients undergoing rhinoplasty. Materials and Methods: Eighty ASA I–II patients aged 18–65 years were randomly assigned to two groups. The propofol group received 0.5 mg/kg propofol 10 minutes before the end of surgery, whereas the dexmedetomidine group received 0.5 µg/kg dexmedetomidine via a 10-minute intravenous infusion. A standardized anesthesia protocol was applied in all patients. Hemodynamic parameters were recorded intraoperatively and postoperatively at predetermined intervals. EA was assessed at the 1st minute after extubation using the RASS scale, and patient satisfaction was evaluated before discharge from the PACU using a 4-point Likert scale. Results: Recovery time was significantly longer in the dexmedetomidine group (p<0.05). RASS scores and agitation indicators were significantly lower compared to the propofol group (p<0.05). Hemodynamic parameters during intraoperative and early postoperative periods (first 15 minutes) were more stable and lower in the dexmedetomidine group (p<0.05). No significant differences were observed between the groups regarding adverse events (p>0.05). Patient satisfaction was markedly higher in the dexmedetomidine group (p<0.05). Conclusion: Dexmedetomidine more effectively reduced emergence agitation, enhanced hemodynamic stability, and improved patient satisfaction compared with propofol, albeit at the cost of prolonged recovery time. Propofol may be preferable when rapid recovery is prioritized, whereas dexmedetomidine appears more advantageous in procedures such as rhinoplasty where agitation risk is high.
