Özet

Sleeve Gastrektominin Yüksek Yağlı Diyetle İndüklenen Rat Obezite Modelinde Hipokampüsteki CB1R Ve CB2R İfadesi Üzerine Etkilerinin İncelenmesi Amaç: Obezite, periferik metabolizmanın yanı sıra merkezi sinir sistemini, özellikle hipokampus bağımlı bilişsel süreçleri etkileyen kronik bir patolojidir. Enerji homeostazı ve sinaptik plastisitede rol oynayan endokannabinoid sistemin (EKS) bu süreçteki rolü dikkat çekicidir. Bu çalışmada, yüksek yağlı diyetle (YYD) indüklenen obezite modelinde sleeve gastrektomi (SG) ve standart diyete geçiş müdahalelerinin fizyolojik, davranışsal ve hipokampal CB1R/CB2R ekspresyonları üzerindeki etkilerinin karşılaştırmalı olarak incelenmesi amaçlanmıştır. Gereç ve Yöntem: Wistar albino erkek sıçanlar (n=72), YYD veya standart diyet ile beslenerek obezite modeli oluşturulmuştur. Hayvanlar daha sonra Kontrol, Kontrol-Sham, Obez, Obez-Sham, Obez-SG ve Obez-Diyet gruplarına ayrılmıştır. Operasyon sonrası 7 hafta boyunca izlenen hayvanlarda visseral yağlanma, Lee indeksi, OGTT ve HOMA-IR değerlendirilmiştir. Bilişsel performans açık alan testi, modifiye yeni obje tanıma testi ve Morris su labirenti (MSL) ile incelenmiştir. Hipokampal CB1R/CB2R mRNA ve protein düzeyleri sırasıyla RT-qPCR ve Western blot ile analiz edilmiştir. Bulgular: YYD, vücut ağırlığını ve visseral adipoziteyi artırarak ve dinamik glukoz toleransını bozarak başarılı bir obezite fenotipi oluşturmuştur. Standart diyete geçiş, morfolojik ve metabolik parametrelerde SG'ye kıyasla daha istikrarlı bir düzelme sağlamıştır. SG erken dönemde belirgin ağırlık kaybı sağlasa da izlem sürecinde kısmi kilo geri alımı gözlenmiştir. Davranışsal testlerde obezite, lokomotor aktiviteyi veya klasik tanıma belleği performansını belirgin biçimde bozmamış; ancak MSL'de uzamsal navigasyon ve rota verimliliği düzeyinde seçici bir stratejik etkilenme oluşturmuştur. Moleküler düzeyde, obezite hipokampal CB2R'de belirgin bir değişiklik yaratmazken, CB1R protein düzeyini anlamlı biçimde azaltmıştır. Planlı karşılaştırmalar, SG sonrasında azalan hipokampal CB1R protein düzeyinde kısmi bir toparlanma eğilimi olduğunu göstermiştir. Sonuç: YYD'ye bağlı obezite, periferik metabolik bozulmanın yanı sıra hipokampal CB1R proteininde azalma ve uzamsal öğrenme stratejisinde seçici bir bozulma ile ilişkilidir. Standart diyet periferik metabolik iyileşmede daha tutarlı bir profil sunarken, SG hipokampal CB1R protein düzeyinde kısmi toparlanmaya işaret eden bir merkezi yanıt paterni göstermiştir. Bu bulgular, obezite tedavisinde periferik metabolik düzelme ile merkezi nöroadaptif onarım süreçlerinin eş zamanlı ilerlemeyebileceğini göstermektedir.

Investigation of the Effects of Sleeve Gastrectomy on Hippocampal CB1R and CB2R Expression in a High-Fat Diet-Induced Rat Model of Obesity Objective: Obesity is a chronic pathology affecting not only peripheral metabolism but also the central nervous system, particularly hippocampus-dependent cognitive processes. The endocannabinoid system (ECS), which is involved in energy homeostasis and synaptic plasticity, may play an important role in this process. This study aimed to comparatively investigate the effects of sleeve gastrectomy (SG) and switching to a standard diet on physiological, behavioral, and hippocampal CB1R/CB2R expression in a high-fat diet (HFD)-induced rat obesity model. Materials and Methods: Male Wistar albino rats (n=72) were fed either an HFD or a standard diet to establish the obesity model. The animals were then assigned to Control, Control-Sham, Obese, Obese-Sham, Obese-SG, and Obese-Diet groups. During a 7-week postoperative follow-up period, visceral adiposity, Lee index, OGTT, and HOMA-IR were evaluated. Cognitive performance was assessed using the open field test, modified novel object recognition test, and Morris water maze (MWM). Hippocampal CB1R/CB2R mRNA and protein levels were analyzed using RT-qPCR and Western blotting, respectively. Results: HFD successfully induced an obesity phenotype, characterized by increased body weight and visceral adiposity, as well as impaired dynamic glucose tolerance. Switching to a standard diet provided a more consistent recovery in morphological and metabolic parameters compared with SG. Although SG induced significant early weight loss, partial weight regain was observed during follow-up. Behaviorally, obesity did not markedly impair locomotor activity or classical recognition memory; however, it caused a selective strategic impairment in spatial navigation and route efficiency in the MWM. At the molecular level, obesity significantly reduced hippocampal CB1R protein levels without causing a marked change in CB2R expression. Planned comparisons suggested a trend toward partial recovery in reduced hippocampal CB1R protein levels following SG. Conclusion: HFD-induced obesity is associated with peripheral metabolic impairment, reduced hippocampal CB1R protein levels, and selective deficits in spatial learning strategies. While switching to a standard diet offers a more consistent profile for peripheral metabolic recovery, SG appears to induce a central response pattern suggesting partial recovery in hippocampal CB1R protein expression. These findings suggest that peripheral metabolic recovery and central neuroadaptive repair processes may not progress simultaneously in obesity treatment.
