Özet

Amaç: Lokal ileri evre özofagus kanserinde neoadjuvan tedavi sonrası klinik (cCR) ve patolojik tam yanıtın (pCR) uyumunu incelemek, pCR'yi ve cCR'yi öngören faktörleri saptamak ve nüks ile genel sağkalıma etki eden bağımsız prognostik faktörleri değerlendirmektir. Gereç ve Yöntem: Temmuz 2020–Şubat 2026 arasında neoadjuvan tedavi almış 122 hasta tek merkezli ve retrospektif olarak incelendi; tanısal performans ve sağkalım analizleri özofajektomi uygulanan 109 hasta üzerinden yapıldı. pCR ve nüks için Firth penalize lojistik regresyon, prognoz için penalize Cox regresyon ve Kaplan-Meier yöntemi kullanıldı. Bulgular: Çalışma grubumuzda pCR oranı %44 bulundu. Hastaların çoğunluğunu skuamöz hücreli karsinom oluşturdu. BT/MR ve endoskopinin kombine değerlendirilmesiyle ortaya çıkan cCR, yöntemler arasında pCR ile en yüksek uyumu gösterdi (κ=0,392; duyarlılık %43,8, özgüllük %93,4, doğruluk %71,6). BT/MR (%31,2; %96,7) ve endoskopi (%37,5; %86,9) yüksek özgüllüğe karşın düşük duyarlılık sergiledi. Çok değişkenli analizlerde cCR (yaklaşık 11 kat), BT/MR yanıtı, endoskopik yanıt ve preoperatif albümin pCR'nin bağımsız belirleyicileriydi; adenokarsinom histolojisi ve FLOT rejimi düşük pCR ile ilişkiliydi. Nüks oranı %12,3 idi. Tek değişkenli analizde sigara kullanımı, cN3 evresi, pT3, pozitif cerrahi sınır ve lenfovasküler invazyon nüksle ilişkiliydi; pCR ise bağımsız koruyucu faktördü. Genel sağkalımda patolojik T ve N evresi, lenfovasküler ve perinöral invazyon, cerrahi sınır pozitifliği, downstaging, pCR ve tümör regresyon derecesi anlamlı prognostik faktörlerdi. Sonuç: BT/MR, endoskopi ve cCR'nin yüksek özgüllükleri pCR'yi doğrulamada değerli olsa da düşük duyarlılıkları aktif sürveyansı güvenle yönlendirmeye yetersizdir. EUS-FNA eklenmesi ve preSANO/SANO modeline uygun çok modaliteli yanıt değerlendirmesi gereklidir. Bu bulgular hastaya özgü tedavi kararları için referans oluşturabilir.

Objective: To evaluate the concordance between clinical complete response (cCR) and pathological complete response (pCR) after neoadjuvant therapy in locally advanced esophageal cancer, to identify the factors predicting pCR and cCR, and to determine the independent prognostic factors affecting recurrence and overall survival. Materials and Methods: A total of 122 patients who received neoadjuvant therapy between July 2020 and February 2026 were analyzed in this single-center, retrospective study; diagnostic performance and survival analyses were based on the 109 patients who underwent esophagectomy. Firth penalized logistic regression was used for pCR and recurrence, and penalized Cox regression and Kaplan-Meier analysis for prognosis. Results: The pCR rate was 44%. The majority of patients had squamous cell carcinoma. cCR, derived from the combined evaluation of CT/MRI and endoscopy, showed the highest agreement with pCR among the methods (κ=0.392; sensitivity 43.8%, specificity 93.4%, accuracy 71.6%). CT/MRI (31.2%; 96.7%) and endoscopy (37.5%; 86.9%) demonstrated high specificity but low sensitivity. In multivariate analyses, cCR (approximately 11-fold), CT/MRI response, endoscopic response and preoperative albumin were independent predictors of pCR; adenocarcinoma histology and the FLOT regimen were associated with lower pCR. The recurrence rate was 12.3%. On univariate analysis, smoking, cN3, pT3, positive surgical margin and lymphovascular invasion were associated with recurrence, whereas pCR was an independent protective factor. Pathological T and N stage, lymphovascular and perineural invasion, positive surgical margin, downstaging, pCR and tumor regression grade were significant prognostic factors for overall survival. Conclusion: Although the high specificity of CT/MRI, endoscopy and cCR is valuable for confirming pCR, their low sensitivity is insufficient to safely guide active surveillance. Incorporation of EUS-FNA and a multimodal response assessment in line with the preSANO/SANO model is warranted. These findings may serve as a reference for patient-specific treatment decisions.
