Özet

Giriş ve Amaç: Minimal invaziv kardiyak cerrahi (MİKC), sternotomiye kıyasla daha az invaziv olmakla birlikte torakotomiye bağlı olarak belirgin postoperatif ağrıya neden olabilir. Bu durum solunum fonksiyonlarını ve mobilizasyonu olumsuz etkileyebilir. Serratus posterior superior interkostal plan bloğu (SPSIPB), torasik analjezi amacıyla tanımlanmış yeni bir interfasiyal bloktur. Bu çalışmada, MİKC uygulanan hastalarda SPSIPB'nin postoperatif analjezi ve derlenme kalitesi üzerindeki etkisinin değerlendirilmesi amaçlandı. Yöntem: Çalışma için yerel Klinik Araştırmalar Etik Kurulu onayı (09/07/2025, E2-25-11690) ve TİTCK onayı (11/11/2025, E-24931227-514.04.02- 5770853) alındı. Araştırma, ClinicalTrials.gov veri tabanına kaydedildi (NCT07259824). Çalışmaya dahil edilen tüm hastalardan bilgilendirilmiş gönüllü onam formu alındı. Bu tek merkezli, prospektif, randomize ve üçlü kör çalışmaya toplam 56 hasta dahil edildi. Hastalar SPSIPB (n=25) ve SHAM (n=25) gruplarına randomize edildi. SPSIPB grubuna ultrasonografi rehberliğinde 30 mL %0,25 bupivakain, SHAM grubuna ise 30 mL salin uygulandı. Primer sonlanım noktası 24 saatlik toplam opioid tüketimiydi. Sekonder sonlanımlar arasında NRS ağrı skorları, QoR-15, klinik iyileşme parametreleri ve mRAS yer aldı. Tekrarlı ölçümler karışık etkili modeller kullanılarak analiz edildi. Bulgular: SPSIPB grubunda postoperatif ilk 24 saatte toplam opioid tüketimi SHAM grubuna kıyasla anlamlı derecede daha düşük bulundu (21,26±11,21 mg vs 34,64±10,43 mg, p<0,001). Ağrı skorları tüm zaman noktalarında SPSIPB grubunda daha düşük saptandı (p<0,001). QoR-15 skorları SPSIPB grubunda 24. ve 48. saatlerde anlamlı olarak daha yüksek bulundu (p<0,001). SPSIPB grubunda oral alım daha erken (p=0,030) ve postoperatif bulantı-kusma insidansı daha düşük saptandı (p=0,003). Ekstübasyon süresi, mobilizasyon zamanı, intraoperatif opioid tüketimi ve mRAS açısından gruplar arasında anlamlı fark bulunmadı (p>0,05). Tartışma ve Sonuç: SPSIPB uygulaması, MİKC geçiren hastalarda postoperatif analjeziyi iyileştirmiş, opioid tüketimini azaltmış ve derlenme kalitesini artırmıştır. Daha düşük opioid gereksiniminin azalmış PONV ve erken oral alım ile ilişkili olması, bu blok tekniğinin iyileşme sürecine katkı sağladığını düşündürmektedir. Ancak radyolojik atelektazi skorları üzerinde anlamlı bir etkisi saptanmamıştır. Bu bulgular, SPSIPB'nin MİKC'de etkili bir multimodal analjezi bileşeni olduğunu göstermektedir.

Background and Objective: Although minimally invasive cardiac surgery (MICS) is less invasive compared to sternotomy, it may cause significant postoperative pain due to thoracotomy. This may adversely affect respiratory function and mobilization. The serratus posterior superior intercostal plane block (SPSIPB) is a newly defined interfascial block for thoracic analgesia. This study aimed to evaluate the effects of SPSIPB on postoperative analgesia and quality of recovery in patients undergoing MICS. Methods: Ethics committee approval (09/07/2025, E2-25-11690) and approval from the Turkish Medicines and Medical Devices Agency (11/11/2025, E-24931227- 514.04.02-5770853) were obtained. The study was registered at ClinicalTrials.gov (NCT07259824). Written informed consent was obtained from all patients. In this single-center, prospective, randomized, triple-blind study, a total of 56 patients were included. Patients were randomized into SPSIPB (n=25) and SHAM (n=25) groups. The SPSIPB group received 30 mL of 0.25% bupivacaine under ultrasound guidance, whereas the SHAM group received 30 mL of saline. The primary outcome was total opioid consumption within the first 24 hours. Secondary outcomes included NRS pain scores, QoR-15, clinical recovery parameters, and mRAS. Repeated measures were analyzed using mixed effects models. Results: Total opioid consumption within the first 24 hours was significantly lower in the SPSIPB group compared to the SHAM group (21.26±11.21 mg vs 34.64±10.43 mg, p<0.001). Pain scores were lower at all time points in the SPSIPB group (p<0.001). QoR-15 scores were significantly higher in the SPSIPB group at both 24 and 48 hours (p<0.001). Time to oral intake was shorter (p=0.030), and the incidence of postoperative nausea and vomiting (PONV) was lower in the SPSIPB group (p=0.003). No significant differences were found in extubation time, mobilization time, intraoperative opioid consumption, or mRAS (p>0.05). Discussion and Conclusion: SPSIPB improved postoperative analgesia, reduced opioid consumption, and enhanced quality of recovery in patients undergoing MICS. The reduction in opioid requirement, together with lower PONV incidence and earlier oral intake, suggests a positive contribution of this block to the recovery process. However, no significant effect was observed on radiological atelectasis scores. These findings indicate that SPSIPB is an effective component of multimodal analgesia in MICS.
