Özet
ÖZET Bu çalışma, GATA Fiziksel Tıp ve Rehabilitasyon AD. ve TSK Rehabilitasyon ve Bakım merkezine diz ağrısı şikayetiyle başvurmuş ve ACR kriterlerine göre primer gonartroz tanısı almış, toplam 54 hasta üzerinde yapıldı. Tüm hastaların başlangıç proprioseptif algı değerlendirmeleri, stabilometrik ölçümleri ve WOMAC skorları kaydedildi ve tedavi grubundaki hastalar fizik tedavi ve proprioseptif rehabilitasyon programına alındı. Bu çalışmada, geç dönem diz osteoartrozu olan hastalarda, hazırladığımız fizik tedavi ve proprioseptif rehabilitasyon programının, proprioseptif algı, denge fonksiyonu ve klinik bulgular üzerine etkileri araştırıldı. Proprioseptif algı, dize pasif olarak verilen açıyı yeniden oluşturma (reprodüksiyon) yöntemiyle, denge fonksiyon değerlendirmesi stabilometrik (KAT denge sistemi) ölçümle ve klinik değerlendirme ise WOMAC indeksi ile gerçekleştirildi. Uyguladığımız 3 haftalık fizik tedavi ve proprioseptif rehabilitasyon programı sonunda, hastaların propioseptif algılarında belli oranda düzelme olmasına rağmen bu istatistiksel olarak anlamlı değildi. Statik stabilometre skorlarında istatistiksel olarak anlamlı düzelme tesbit edildi. Ancak dinamik stabilometre skorlarında istatistiksel anlam taşıyan bir farklılık gözlenmedi. WOMAC skorlarında, kontrol grubunda sadece ağrı skorunda düzelme gözlenmişken; tedavi grubunda tüm skorlarda istatistiksel olarak anlamlı düzelmeler tesbit edildi. Sonuç olarak, diz osteoartritinin tedavisinde genellikle gözden kaçan proprioseptif algı ve denge fonksiyon bozuklukları da dikkate alınmalı ve tedavi programlarına hastalığın bu komplikasyonlarmm da giderilmesine yönelik çeşitli tedavi yaklaşımları eklenmelidir. 59

SUMMARY This study was carried out in 54 patients who referred to GMMA Department of Physical Medicine and Rehabilitation and TAF Rehabilitation and Care Center with knee pain and were diagnosed as primary gonarthrosis according to the ACR criteria. For all of the patients, the initial results of evaluation of proprioceptive perception, scores of stabilometric measurements, and the scores of WO MAC index were registered and the patients who were in the study group were taken into the physical therapy and rehabilitation program. In this study, the effects of the physical therapy and the proprioceptive rehabilitation program we made on the proprioceptive perception, balance function and the clinical findings were investigated. The proprioceptive perception was assesed by the ability to reproduce passive positioning of the leg ( reproduction method ), the balance function was assesed by stabilometric evaluation ( KAT system ) and the clinical evaluation was made by using the WOMAC index. By the end of the three weeks proprioceptive rehabilitation program we applied, although some improvements were detected in the proprioceptive perceptions of the patients, these were not statistically significant. The improvements in the scores of the static stabilometry were statistically significant. But there was no significant changes in dynamic stabilometry scores. With regard to WOMAC scores, in the control group pain scores improved only; but in the study group all of the WOMAC scores improved significantly. In conclusion, in the management of gonarthrosis, the generally overlooked proprioceptive perceptive problems and the disorders of balance must also be considered and in order to correct these complications of the disease some therapeutic approaches must be added to the treatment programs. 60
