Özet
36 ÖZET Kliniğimizde 1986-1991 yıllan arasında yapılan 198 transvezikal prostatektomi (TVP) ve 261 transüretral prostatektomi (TURP) olgusundan, arşivden dosyalan çıkanlabilen ve takipleri düzenli olan 347 olgu (163 TVP, 184 TURP) retrospektif olarak değerlendirildi. Ameliyat esnasında ve sonrasında kan transfüzyonu gereksinimi, ameliyat sonrası kateterli kalış süresi ve hastanede kalış süresinin TURP'da TVP'ye göre anlamlı derecede düşük olduğu bulundu. Her iki ameliyat yönteminde üretral darlık gelişimi açısından anlamlı fark bulunamadı. Dosyalarında adresleri bulunan 196 kişiye mektupla çağrı yapıldı. Bunlardan çağrıya cevap veren 65 (21 TVP, 44 TURP) kişi değerlendirildiğinde ameliyat sonrasında irritatif ve obstrüktif semptomların anlamlı ölçüde düzelmiş olduğu görüldü. Bu grupta da her iki ameliyat yöntemi arasında üretral darlık gelişimi açısından anlamlı fark bulunamadı. Ameliyat sonrası impotans ve retrograd ejekülasyonun TURP'da TVP'ye göre anlamlı derecede düşük olduğu bulundu. \ Uygun olgularda TURP'un TVP'ye tercih edilebileceği sonucuna varıldı.

37 SUMMARY There has been a total of 198 transvesical prostatectomy (TVP) cases and 261 transurethral prostatectomy (TURP) cases in our clinic between the years 1986 and 1991. In this study, 347 of the cases (163 TVPs and 184 TURPs) whose records were available and who were followed-up regularly were evulated retrospectively. It has been found that the need for peroperative and postoperative blood transfusion, duration of postoperative catheterization, and duration of hospital stay were significantly lower in the TURP group than the TVP group. There was no difference between the two groups in terms of urethral stricture development. Of the patients whose addresses were found in the records, 196 were called by letter fora check-up. 65 of them responded quickly (21 TVP and 44 TURP cases) and it was observed that irritative and obstructive symptoms had recovered significantly in postoperative period. In this check-up group, there was no difference between the two approaches in terms of urethral stricture, either. Development rates of retrograde ejaculation and impotence in TURP cases were less than that öf TVP cases with a statistical significance. As a result, TURP should be preferred to TVP in appropriate cases.
