Özet
Araştırma, artroskopik diz ameliyatı sonrası kullanılan üç farklı soğuk uygulama yönteminin etkisini belirlemek amacıyla klinik çalışma olarak yapılmıştır. Araştırmanın evrenini Hacettepe Üniversitesi Erişkin Hastanesi Bölüm 52 Ortopedi Kliniği'nde dizinden artroskopi ameliyatı uygulanan 107 hasta, örneklemini ise 60 hasta oluşturmuştur. Araştırmanın gerçekleştirilebilmesi için Hacettepe Üniversitesi Tıp Fakültesi Tıbbi, Cerrahi ve İlaç Araştırmaları Etik Kurulu'ndan yazılı izin alınmıştır. Araştırmada önce soğuk uygulama yöntemine ilişkin standart uygulama rehberi geliştirme çalışması yapılmıştır. Bunu takiben artroskopi ameliyatı olan hastalarda üç farklı soğuk uygulama yönteminin deri sıcaklığına, diz çevresine ve eklem hareket açıklığına olan etkisi incelenmiş ve hastaların yöntemlere ilişkin memnuniyet durumları ile hemşirelerin görüşleri belirlenmiştir. Araştırmada veriler, hemşirelere ve hastalara ait tanıtıcı özellikler formları, gözlem ve değerlendirmelere ilişkin formlar ile hasta memnuniyeti anket formu kullanılarak toplanmıştır. Hastalara kombine soğuk kompresyon sistemi, buz paketi ve soğuk paket yöntemleri ile soğuk uygulama yapılmış, değerlendirmelerde Data Ölçüm ve Kaydedici cihaz, diz üstü bilgisayar, gonyometre ve mezura kullanılmıştır. Verilerin değerlendirilmesinde Tek Yönlü Varyans Analizi, Tukey HSD, Mann-Whitney U ve Bağımsız Gruplarda t testi kullanılmıştır. Araştırmada, hemşirelerin standart uygulama rehberinde yer alan işlem basamaklarını doğru biçimde gerçekleştirdikleri, buz paketinin 15. dakikadan sonra deri sıcaklığını daha fazla düşürdüğü, soğuk paketin 10. dakikadan sonra deri sıcaklığını düşürmede etkili olmadığı, diz çevresi ve eklem hareket açıklığı değerleri açısından yöntemler arasında fark olmadığı, hemşirelerin en fazla buz paketi uygulamasından, hastaların ise kombine soğuk kompresyon sistemi uygulamasından memnun oldukları belirlenmiştir. Araştırmadan elde edilen bulgular doğrultusunda artroskopi ameliyatlarından sonra deri sıcaklığını azaltmada buz paketinin etkili bir yöntem olarak kullanılabileceği ancak diğer uygulama yöntemlerinin de avantajlarının göz önüne alınması gerektiği önerilmiştir.

This study was carried out as clinical trial to determine the effects of three different cold application methods which used after arthroscopic knee surgery. The population of the study consisted of 107 patients who underwent arthroscopic knee surgery in the Department of Orthopedics, at Hacettepe University Hospital. The sample of the study consisted of 60 patients. The written permission was obtained Hacettepe University Medical, Surgical and Pharmaceutical Research Ethics Board. At the beginning of the study, the standard practice guide for cold application method was prepared. Then the effect of three different cold-application methods on the skin temperature, joint range of motion around the knee, patients satisfaction about the effect of conditions and nurses' opinions regarding the methods were determined. Data were collected by Demographic Data Sheets of patients and nurses, observations and evaluations forms for application and patient satisfaction form. Patients had Combined Cold Compression System, Ice Pack and Cold Pack application. Data recorder device, laptop computer, goniometer and tape measure were used for the measurement and evaluation of the methods. Data were analyzed by one way Anova, Tukey HSD, and independent t test, Mann-Whitney U test. According to the findings, nurses perform standard practice guide steps correctly, Ice Pack method had started to reduce the temperature of the skin after 15 minutes application, Cold Pack method was not effective in reducing the skin temperature after 10 minutes application and there was no difference between the methods regarding the knee circumference and range of motion of the knees. Nurses were satisfied with the application of Ice Pack application whereas their patients were satisfied with the application of Combined Cold Compression System. Base on the result of the research, it is suggested that the Ice Pack can be used as an effective method for reducing skin temperature after arthroscopic surgery; however, the advantages of other application methods should also be taken into account in the benefits.
