Özet
Radius distal uç kırıkları toplumda sık karşılaşılan ve tedavisi kişinin günlük aktivitesini yakından ilgilendirmesi nedeniyle önemli bir kırık tipidir. İnsanların daha fazla ince el bilek hareketleri gerektiren işlerde çalışmaları ve yaşam beklentilerinin artması nedeniyle çok çeşitli tedavi yöntemleri geliştirilmiştir. Tedavide kapalı redüksiyon ve alçılamanın yanı sıra cerrahi tedavi yöntemi olarak kapalı redüksiyon ve perkütan telleme, açık redüksiyon ve internal tespit, eksternal fiksatör uygulama ve artroskopi destekli tespit sayılabilir. Bu çalışmada, radius distal uç kırığı bulunan hastalarda başsız konik kompresif vida ile tespit yönteminin sonuçları değerlendirildi. 18 ile 75 yaşları arasında değişen 40 olgu (41 kırık) çalışmaya dahil edildi. Olguların 23'ü kadın ve 17'si erkek olup ortalama yaşları 45.9 yıl (18-75) idi. Olguların kırıkları; ameliyathanede anestezi altında kapalı olarak redükte edildikten sonra perkütan 2 veya 3 adet başsız konik vida ile tespit edildi. Operasyondan sonra 1-3 günde taburcu edildiler. Olgulara operasyon sonrası dönemde atel veya alçı ile tespit uygulanmadı. Operasyondan 1 gün sonra pasif ve kontrollü aktif el bilek hareketleri başlandı. Olgular ortalama 24 ay takip edildi. Olguların fonksiyonel sonuçları DASH ve Gartland-Werley Skoru kullanarak, radyolojik sonuçları ise Steward ve arkadaşlarının modifiye ettiği radyolojik kriterlere göre değerlendirildi. Olguların klinik sonuçları DASH skorlama sistemine göre 1. ayda %74, 3. ayda %32,6, 12. ayda %14.2 olarak bulundu. Gartland Werley klinik değerlendirme kriterlerine göre 41 radius distal uç kırığının 32`sinde (%78.0) mükemmel sonuç, 7'sinde (%17,1) iyi sonuç, 2'sinde (%4.9) orta sonuç elde edildi. Olguların radyolojik sonuçları Stewart radyolojik skorlama sisteminde kırıkların 30'unda (%73.1) mükemmel, 8'inde (%19.6) iyi, 3'ünde (%7.3) orta olarak bulundu. Kırıkların %90'nından daha fazlasında klinik ve radyolojik olarak mükemmel ve iyi sonuç alındı. Kullandığımız cerrahi ve tespit tekniği ile olgular minimal komplikasyon ile tedavi edildiler ve en kısa sürede günlük yaşamlarına geri döndüler.

Distal radius fractures are important fracture types because of the high encounter frequency in the society and the close association of the treatment with personel daily activities. A wide variety of treatment methods have been developed because of people working mostly in jobs needing subtle wrist movements and the increased expectancy in life. Besides closed reduction and casting in the treatment, as a surgical treatment method closed reduction and percutaneous pinning, open reduction and internal fixation, external fixation techniques and arthroscopy can be aided fixation. In this study patients with distal radius fractures, treated by the fixation method with headless conic compressive screws were evaluated. 40 patients (41 fractures) ages between 18-75 were included in the study. There were 23 females and 17 males and the average age was 45.9 (range 18-75 years) years. Patients? fractures after being reduced in a closed way, was fixed with 2 or 3 percutaneous headless conic compressive screws in the operating room under anesthesia.They were discharged after about 1-3 days after the operation. In the period after the operation fixation with cast and splint were not done to the cases. Passive and controlled active wrist movements were begun 1 day after the operation. Patients were followed up for about 24 months. Patients? functional results were evaluated by using DASH and Gartland Werley score and the radiological results were evaluated by the modified radiological criteria by Stewart and his friends. Patients? clinical results according to the DASH scoring system was found %74 in the 1st month, 3rd month %32.6, 12th month %14.2. According to the Gartland Werley clinical evaluation criteria in 32 of the 41 distal radius fractures (%78.0) excellent results, in 7 (%17.1) good results and in 2 (%4.9) fair results were found. Patients? radiological results according to Stewart radiological scoring system 30 of the fractures (%73.1) excellent, in 8 (%19.6) good, in 3 (%7.3) fair results were found. More than %90 of the fractures both clinical and radiological excellent and good results were found. By the surgical and fixation technics used the patients were treated with minimal complications and returned to their daily activities soon
