Özet

İÜ Cerrahpaşa TF Yenidoğan Yoğun Bakım Bebeklerinde işitme kaybı insidansı(UZMANLIK TEZİ)Dr. Rassul RashıdovDanışman Öğretim Üyesi: Prof. Dr. İrfan PapilaİSTANBUL-2012Amaç:Yenidoğan Yoğun Bakımda takip edilen hastalarda işitme kaybı insidansını etkileyen risk faktörlerini araştırmak. Uygulanan programın sonuçlarının ve kullanılan tarama protokolunun etkinliğinin değerlendirmek.Gereç ve yöntemler:Kasım 2011 ? Haziran 2012 tarihleri arasında İstanbul Üniversitesi Cerrahpaşa Tıp Fakültesi Yenidoğan Yoğun Bakım ünitesinde takip edilen hastalara uyarılmış otoakustik emisyon ölçümü (transiet evoked otoacoustic emissions,TEOAE) ile protokolün ilk basamağını oluşturacak şekilde işitme taraması yapıldı. Taburculuk sonrası hastalar İkinci basamağa yönlendirildi. İkinci basamakta TEOAE, Timp (Timpanometry ) testi ile ABR ( Auditory Brainstem Response, İşitsel Beyinsapı Yanıtı ) ve KBB ( Kulak Burun Boğaz) muayenesi yapıldı. İkinci basamaktan geçmeyen hastalar 3 ay içinde uzman odyolog eşliğinde klinik ABR testine yönlendirildi.Bulgular :a)Birinci basamakta 7 hastada (%14) unilateral işitme kaybı saptandı ve 6 hastada (%12) bilateral işitme kaybı saptandı.b) İkinci basamakta toplam 3 hasta (%6) kontrole gelmedi, birinci basamakta işitme testinde unilateral işitme kaybı saptanan 7 (%14) hastadan 5'i (%10) testlerden geçti 2 hasta (%4) ise ikinci basamağa kontrole gelmedi . Birinci basamakta bilateral işitme kaybı saptanan 6 (%12) hastadan 5'i (%10) hasta TEOAE ve ABR'den geçti 1 hasta (%2) ise ikinci basamakta testlerden geçmedi.c) Üçüncü basamakta, ikinci basamakta işitme testinden geçemeyen hasta kontrole gelip TEOAE ve ABR'den geçti. Birinci basamakta unilateral işitme kaybı saptanıp ikinci basamağa gelmeyen 2 hastaya yine ulaşılamadı.Sonuçlar :Sağlıklı yenidoğanlarda işitme kaybı 1:1000 ile 3:1000 oranında olduğundan, ayrıca yoğun bakım ünitesinde tedavi gören bebeklerde bu oran %2 ile %4'te (3,5,17) olmasına rağmen, İstanbul Üniversitesi Cerrahpaşa Tıp Fakültesi Yenidoğan Yoğun Bakım ünitesinde takip edilen 50 hasta üzerinde yapılan yaklaşık sekiz ay süren araştırmada işitme kaybına rastlanmadı .

The incidence of hearing loss in the Neonatal Intensive Care Babies in Medical Faculty of Istanbul University (Expertise Thesis)Dr.RassulRashidovFaculty Advisor: Prof. Dr.IrfanPapilaISTANBUL-2012Objective:To investigate the risk factor saffecting the incidence of hearing lossin Neonatal Intensive Care Unit patients. To evaluate the results of the applied program and effective ness of screening protocol.Materials and methods:Transiet evoked otoacoustic emissions (TEOAE) measurement of hearing has been performed to patients at the Neonatal Intensive Care Unit of Istanbul University Cerrahpasa Faculty of Medicine between November 2011and June 2012,so as to form the first step ofthe protocol.After discharge they have proceeded to the second step. In the second step TEOAE, Tympanometrytest, ABR (Auditory Brainstem Response) and ENT (Ear, Nose and Throat) examination have been performed.Patients that have not passed the second step have been forwarded to ABR test under supervision of expert audiologist Second ordernot exceeding 3 monthsin the period of 3 months.Results:Unilateral hearing loss has been detected in 7 patients(14%)andbilateral hearing loss was found in 6 patients(12%) during thefirststep.In total 3 patients (6%) did not rtiküla control. Among 7 patients (14%) patients that showed up unilateral hearing loss during the first stage hearing test ,the primary level,unilateral hearing loss,5 patients(10%)passed tests and 2 patients (4%) did not rtiküla the second step control.Among 6 patients(12%) for which bilateral hearing loss has been detected during the first step 5 (10%) patients passed the TEOAE and ABR tests, but 1 patient(2%) did not passed.The patient that did not passed the second step hearing test, rtiküla the third step and passed TEOAE and ABR hearing tests. We couldn?t reach two patients for which unilateral hearing loss has been detected during first step and which didn?t rtiküla the second step.Results:Despite that theratio of hearing loss in newborns is between 1:1000 to 3:1000 and that this ratio for infants who were treated in the intensive care unit is between 2% to 4% (3,5,19) there was no hearing loss observed in any of 50 patients at Neonatal Intensive Care Unit of Istanbul University Cerrahpasa Faculty of Medicine, during the examination carried out for about eight month-long study.
