Özet

ÖZETGiriş:Nachemson ve Peterson'un breys tedavisinin, adölesan idiyopatik skolyozun doğal seyrini değiştirdiğini göstermesinden sonra bile, breys tedavisinin uzun dönemdeki etkinliği hala tartışmalı kalmıştır. Bu tartışmalara rağmen, klinik pratikte, çoğu ortopedi uzmanı idiyopatik skolyoz tedavisinde breys kullanmakta ve en azından kısa vadede breysin etkinliğine inanabilmektedir. Ancak sorun breysin etkisinin uzun süre sürüp sürmediğidir. Bu çalışmamızda adölesan idiyopatik skolyozun Boston breys ile tedavisinden 5 yıl veya daha fazla süre sonraki uzun dönem radyolojik sonuçlarını araştırmak amaçlandı.Gereç ? Yöntem:Ege Üniversitesi Ortopedi ve Travmatoloji Anabilim Dalında, Boston breys ile tedavi edilmiş ve tedavi bitiminden en az 5 yıl sonra muayenesi yapılmış adölesan idiyopatik skolyozlu hastalardan kayıtları eksiksiz olan 26 hasta çalışmaya alındı. Tedavi başlangıç, tedavi bitiş ve son kontroldeki skolyoz açıları ölçüldü. Breyssiz geçen süre hesaplandı.Bulgular:Başlangıç, tedavi sonu ve son kontroldeki skolyoz açılarının ortalaması sırasıyla 28.7º (17º-43º), 27.2º (13º- 45º), 30.8º (17º-46º ) olduğu görüldü. Tedavi sonlandıktan sonra geçen süre ortalama 138 ay (60-238) olarak hesaplandı. Tedavi sonunda skolyoz açısında ortalama 1.5 º azalma olduğu görüldü. Tedavi bittikten sonra ortalama 3.6 º (0º-10.5º) progresyon saptandı. Ortalama progresyon oranı 0.35º/ yıl (0º/yıl-1.06º/yıl) olduğu görüldü.Sonuç:Boston breys ile tedavi edilmiş adölesan idiyopatik skolyozlu hastaların çoğunda uzun dönem sonuçlar tatmin edici düzeydedir. Boston breys, adölesan idiyopatik skolyozun tedavisi için uzun dönemde bile etkindir. Tedavi sonrasında eğriliğin ilerlemesi skolyozun doğal seyrindeki ile benzerdir.

ABSTRACTBackground:Even after Nachemson and Peterson showed that the bracing alters the natural history of adolescent idiopathic scoliosis, its efficacy in the long term stil remains controversial. Despite that controversies, in the clinical situation, most of the experts are using brace for the treatment of idiopathic scoliosis and may believe its efficacy at least in a short term. However the problem is whether the effect of brace maintains for a long term. The purpose of this study is to investigate long-term radiological outcomes in adolescent idiopathic scoliosis 5 years or more after treatment with the Boston BraceMaterial-Method26 patients who have accurate records of patients with adolescent idiopathic scoliosis treated with Boston Brace and had been examined at least 5 years after weaning at the Ege University Department of Orthopaedic Surgery and Traumatology are included. Scoliosis angles at first time, weaning and the last follow-up were measured. The time without brace was calculated.ResultsThe mean scoliosis angle at first time, weaning and the last follow-up were 28.7º (17º-43º), 27.2º (13º- 45º), 30.8º (17º-46º ) respectively. The mean follow- up time after weaning was 138 month (60-238). the mean reduction of curve was 1.5 º at weaning. the mean progression after weaning was 3.6º (0º-10.5º). The mean progression rate after weaning was 0.35º/year (0º/year- 1.06º/year)Conclusion:Long-term results were satisfactory in most patients with adolescent idiopathic scoliosis treated with the Boston Brace. Treatment with the Boston Brace is effective even in the long-term for the treatment of adolescent idiopathic scoliosis. Curve progression after the treatment was similar to that in the natural history.
