A comparative evaluation of the holding strength of partially threaded tipped IM pins vs. nonthreaded tipped IM pins, Was performed in the dog femur. Measurements were made of the axial extraction force necessary to cause pin removal from the bone using the MTS servo-hydraulic test machine at 0, 4 and 8 weeks postoperatively on 30 dogs. Histologic rlsults were correlated with the biomechanical data. At all time intervals, partially threaded tipped IM pins used on fractured or intact femurs were found to have a higher holding strength than nonthreaded tipped IM pins with either fractured or intact femurs. A comparison of the holding strength of partially threaded tipped IM pins between fractured femurs and intact femurs at 0 vs 8 and 4 vs 8 weeks revealed no statistical significance. Similar remits were also obtained with nonthreaded tipped pins of fractured and intact femurs at 0 vs 8, 4 vs 8 and 0 vs 4 weeks. There was a significant difference between the holding strength of partially threaded tipped pins of the fractured femurs and that of the intact femurs at 0 as vs 4 weeks. At no time did the mean holding strength of partially threaded tipped IM pins (with either fractured or intact femurs) decline to a lev.1 below the initial (0 week) .alues. However, the mean values of nonthreaded tipped IM pins with either fractured or intact femurs were below the initial values at 4 and 8 week intervals. New bone growing into the threads of partially threaded tipped IM pins resulted in an increased holding strength at 4 weeks over 0 weeks. Bone remodeling, along with intensive bone death, fibrous tissue development, and a tremendous amount of tissue thread resorption, caused a loss of holding strength at 8 weeks. There was a continuous development of fibrous tissue in the nonthreaded tipped IM pins of intact femurs at 4 and 8 weeks. It decreased the holding strength below the initial values at 4 weeks, and below the initial and 4 week values at 8 weeks. Incomplete healing of fractured femurs at 4 weeks and progressive healing at 8 weeks were considered to be the main factors for the decrease and increase seen respectively, in the holding strength of nonthreaded tipped pins in the above groups. It was concluded that, partially threaded tipped IM pins provided a comparably higher holding strength than nonthreaded tipped IM pins.
