Özet
Sistemik İzotretinoin Tedavisi Alan Akne Vulgarisli Hastalarda Serum B12 ve Folik Asit Düzeyleri Amaç: Bu çalışmanın amacı izotretinoin tedavisi alan nodülokistik akne vulgarisli hastalarda izotretinoinin B12 vitamini ve folik asit düzeylerine etkisini araştırmaktır. Gereç ve Yöntem: Çalışmaya Kasım 2014 - Şubat 2014 tarihleri arasında İzmir Katip Çelebi Üniversitesi Atatürk Eğitim ve Araştırma Hastanesi Dermatoloji Polikliniği'ne nodülokistik akne vulgaris nedenli başvuran ve 1 mg/kg/gün dozunda izotretinoin tedavisi başlanan hastalar alındı. 40 akne vulgarisli hastanın izotretinoin tedavisi öncesi, tedavinin üçüncü ve altıncı ayları hemogram, karaciğer fonksiyon testleri, lipidler, B12 vitamini ve folik asit düzeyleri ölçüldü. Bulgular: Çalışmamıza aldığımız akne vulgarisli hastaların karaciğer fonksiyon testleri ve HDL hariç lipidleri izotretinoin tedavisinin üçüncü ve altıncı ayında tedavi öncesine göre artmış olarak bulundu. HDL düzeyindeki düşüş istatistiksel olarak anlamlıydı.Hemoglobin değerlerinde tedavi öncesi ve sonrasında anlamlı bir değişiklik saptanmadı.B12 vitamini ve folik asit düzeylerinde tedavi öncesi, tedavinin üçüncü ve altıncı ayı ölçülen değerler arasında istatistiksel olarak anlamlı fark saptanmadı. Sonuç:İzotretinoin tedavisi B12 vitamini ve folik asit seviyeleri üzerinde anlamlı bir düşüşe sebep olmaz. Homosistein metabolizmasında B12 vitamini ve folik asit rol oynadığı için, izotretinoin tedavisine başlamadan önce B12 vitamini ve folik asit düzeylerine bakılarak, eksiklik saptanan hastalarda B12 vitamini ve folik asit replasmanı yapılmalıdır.

Serum Vitamin B12 and Folic Acid Levels In Patients with Acne Vulgaris Who Are Using Systemic Isotretinoin Objective:The objective of the study is to search the effect of systemic isotretionin treatment on vitamin B12 and folic acid levels in patients with nodulocystic acne vulgaris. Methods:We included the patients with nodulocystic acne who had applied the İzmir Katip Celebi Univercity Faculty of Medicine Ataturk Training and Research Hospital Outpatients Dermatology Clinic from november 2014 until February 2015 , and who had been using 1mg/kg/d isotretionin. Liver function tests, complete blood count, ivitamin B12, folic acid and blood lipid profiles were measured three times for each 40 patients: just before the treatment, in third month and the sixth month of the treatment. Results:Liver function tests and blood lipid profile except HDL levels were significantly higher in the third and the sixth months of the treatment in comparison with the beginning. We didn't find a significant relation between the CBC and isotretionin treatment. And there is no significant changes for vitamin B12 and folic acid levels before,during and after the treatment. Conclusion:Isotretionin treatment does not cause a decrease in vitamin B12 and folic acid levels. Because of the important role of vitamin B12 and folic acid, in homocystein metabolism, we need to determine the levels of these vitamins before the isotretionin treatment. And supplementation of these vitamins should be recommended in cases of their deficiency, so as to decrease the risks of neuropsychiatric and occlusive vascular diseases releated to hyperhomocysteinemia.
