Özet
ÖZET Sağlık hizmeti sunumunda sağlık ekibinin üyesi olan hekimler ve hemşireler arasındaki işbirlikçi tutum, olumlu hasta sonuçlarının temel belirleyicisidir. Her bir takım üyesi, işbirlikçi bilgi alışverişi gerçekleştirdiğinde hasta bakımı ve tedavi planı uygulaması en uygun şekilde yapılmaktadır. Bu çalışmanın amacı, hekimlerin ve hemşirelerin işbirliğine ilişkin tutumlarını ölçmektir. Çalışma Okmeydanı Eğitim ve Araştırma Hastanesi' nde çalışan hekimler ve hemşireler arasında yapılmıştır. Toplam hekim (hekim n: 649) ve hemşire sayısı (n: 643) 1292 olup, örnekleme alınan hekim sayısı 149, hemşire sayısı ise 148' dir. Çalışmanın verilerini toplamak için Jefferson Hekim Hekimliği İşbirliği anketine yönelik tutum ölçeği kullanılmıştır. Jefferson Hekim ve Hemşire İşbirliği Tutum Ölçeği (JHHIS) dört alt ölçeğe sahipti: (a) ortak eğitim ve takım çalışması (7'den 4'e, 7'den 28'e kadar olan bir alt ölçeğe kadar 7 madde); (b) hemşirenin tedavi ve bakımdaki rolü (3 puan 3-12 alt ölçek puanları ile); (c) hemşirenin özerkliği (3-12 alt ölçeği olan 3 madde) ve doktor hakimiyeti (2 madde, alt ölçek puan aralığı 2 ile 8 arasında). Verilerin istatistiksel analizi için ortalama, standart sapma, yüzde, Oneway Anova testi, post hoc scheffe testi ve Student t testi kullanıldı. HHİ puanları hemşirelerde 52.135 ± 4.717, uzman hekimler 48,217 ± 5,012 ve asistan hekimlerde 46,803 ± 5,424 bulunmuştur. Hemşirelerin her bir alt boyutta hekimlere göre daha olumlu bir tutum sergiledikleri ve işbirliğine daha yatkın oldukları görülmüştür. Cinsiyet, yaş, eğitim düzeyi ve hemşirelik çalışma alanı değişkenleri gibi demografik özellikler arasında istatistiksel olarak anlamlı bir ilişki bulunmuştur (p <0,05). Hekim çalışma alanı, tecrübe, vizit durumu gibi demografik özellikleri arasında istatistiksel olarak anlamlı bir ilişki bulunmamıştır (p> 0,05). İşbirliği tutumunu geliştirecek güçlendirecek ortak mesleki eğitim programlarının uygulanması ve mesleki saygı ortamının oluşturulması ile sağlık hizmet sunumunda olumlu sonuçların elde edileceği sonucuna varılmıştır.

COLLABORATION OF PHYSICIANS AND NURSES: THE EXAMPLE OF EDUCATION AND RESEARCH HOSPITAL Collaborative attitudes between physicians and nurses, who are members of the healthcare team in the provision of health services, are the main determinant of positive patient outcomes. Each team member makes the most appropriate patient care and treatment plan application when collaborative information exchange is carried out. The aim of this study is to measure attitudes of physicians and nurses to collaboration. The study was conducted among physicians and nurses working at Okmeydanı Education and Research Hospital. The total number of doctors (physician n: 649) and the number of nurses (n: 643) is 1292 and the number of physicians sampled is 149 and the number of nurses is 148. The Jefferson Scale of Attitudes toward Physician-Nurse Collaboration questionnaire was used to collect data in the study. The Jefferson Physician and Nurse Collaboration Attitude Scale (JHHIS) had four subscales: (a) shared education and teamwork (7 items ranging from 1-4 to a subscale score of 7 to 28); (b) the role of the nurse in treatment and care (3 items 3-12 with subscale scores); (c) autonomy of the nurse (3 items with a subscale score range of 3-12) and physician dominance (2 items, subscale score range between 2 and 8). Mean, standard deviation, percent, Oneway Anova test, Post hoc Scheffe test and Student t test were used for statistical analysis of the data. HHI scores were found to be 52.135 ± 4.717 for nurses, 48,217 ± 5,012 for consultants physicians and 46,803 ± 5,424 for assistant physicians. It was observed that nurses had a more positive attitude and more tendency to collaboration in each sub-dimension physicians. There was a statistically significant relationship between demographic characteristics such as gender, age, educational level and nursing study field variables (p <0.05). There was no statistically significant relationship between demographic characteristics such as physician work area, experience, and studying together status (p> 0,05). The implementation of joint vocational training programs that will strengthen the the collaboration stance and the establishment of an environment of professional respect have resulted in positive results in health service delivery.
