Özet
ÖZET Ocak 1997 ve Kasım 1997 tarihleri arasında, kliniğimize PE şikayetiyle başvuran toplam 48 hastadan, çalışma kriterlerine uyan, yazılı/sözlü onay alınan 40 hasta çalışmaya alındı. Çalışmaya alınan hastalar, randomize ve çift-kör sistemle, yirmişer kişiden oluşan çalışma ve kontrol grupları olmak Üzere iki gruba ayrıldı. Çalışma grubuna bir ay süreyle fluoksetin, 20mg/gün, ve kontrol grubuna bir ay süreyle plasebo verildi. Tedavi öncesi ve tedavi sonu dönemde hastalar vajen içi kalış süresi ve yan etkiler yönünden sorgulanıp, penil duyu eşiği tespiti sonrası, SUC ve KSUP değişkenleri yönünden incelendi. Bir aylık tedavi sonunda, fluoksetin verilen grupta vajen içi kalış süresi ve duyu eşiği değerinin tedavi öncesi duruma ve kontrol grubuna göre anlamlı şekilde artmış olduğu görüldü. Her iki grupta da SUC ve KSUP latans ve genliklerinde bir aylık tedavi sonrasında anlamlı bir değişiklik saptanmadı. Bu bulgularla, fluoksetinin PE tedavisindeki etkinliğinin, SUC ve KSUP latans ve genliklerini değiştirmeden, penis derisinde duyu eşiğini yükseltmesine bağlı olabileceği sonucuna varıldı. 37

SUMMARY Between January 1997 and November 1997, 40 out of 48 patients who applied our clinic with the complaint of PE, adjusting to the study criteria and accepting the written/oral consent were randomly, and in a double-blinded fashion, divided into two groups of twenty patients. The study group received fluoxetine 20mg/day and the control group received placebo for one month. The patients were evaluated, in the pre-treatment and post-treatment visits, for intravaginal latency, penile sensory threshold, sacral evoked response (SER) and cortical somatosensorial evoked potential (CSEP) variables. The adverse drug reactions were recorded in the post-treatment visit, as well. At the end of the treatment, intravaginal latency and penile sensory threshold were seen to be increased in the study group when compared to the pre-treatment period and to the control group. No change was observed in both groups when the amplitudes and latencies of SER and CSEP were concerned. These findings suggest that fluoxetine is effective in the treatment PE probably by its effect on increasing the penile sensory threshold, without changing the amplitudes and latencies of SER and CSEP. 38
