Özet
İnme, ileri derecede özürlülük oranına sahip olan, yüksek tekrarlama oranı ve üst seviyelerdeki ölüm oranı ile akut başlangıçlı bir hastalıktır. Bu çalışmanın amacı, inmeli bireylerde ev temelli ayna terapisi ve kognitif terapatik egzersizlerin üst ekstremite fonksiyonlarına, kongisyona, depresyona ve yaşam kalitesine etkisini incelemekti. Çalışmaya 30 inmeli birey dahil edildi ve randomize olarak Ayna Terapisi (AT) grubu (n=15) ve Ayna Terapisi (AT) + Kognitif Terapi (KT) grubu olarak iki gruba ayrıldı. Her iki gruptaki bireylere 1 saatlik seans şeklinde, haftada 3 gün, 8 hafta boyunca toplam 24 seans AT uygulandı. KT grubundaki bireylere ise, AT'ye ilaveten kognitif task verildi. Tüm katılımcıların üst ekstremite fonksiyonelliği Fugl-Meyer Üst Ektremite Motor Değerlendirmesi (FMA), kognitif açıdan Mini-Mental Durum Testi (MMDT), spastisite seviyesi Mofiye Ashworth Ölçeği (MAS), günlük yaşam aktiviteleri Barthel Günlük Yaşam Aktivite Ölçeği (BDI), yaşam kaliteleri İnmeye Özgü Yaşam Kalitesi Ölçeği (SSQOL) ve depresyon seviyeleri Beck Depresyon Ölçeği (BDÖ) ile çalışmanın başlangıcında ve bitiminde değerlendirildi. Grup içi değerlendirmelerde her iki grupta da FMA, MMDT, BDI, SSQOL, BDÖ ölçeklerinde istatiksel olarak anlamlı bir iyileşme bulundu (p<0.05). Gruplar arası değerlendirmelerde tedavi sonrasında MMDT'nde grup*zaman değerlendirmesinde BDI'da AT+KT grubu lehine istatiksel olarak anlamlı bir fark bulundu (p<0.05). Çalışmamızın sonucuna göre inmeli bireylerde ev temelli telerehabilitasyon yöntemiyle uygulanan kognitif tasklı ayna terapisi bireylerin genel kognitif durumunda iyileşmeye ve günlük yaşam aktivitelerinde bağımsızlığın artmasına neden oldu.

Stroke is an acute-onset disease with an advanced disability rate, high recurrence rate, and high mortality rate. The aim of this study was to examine the effects of home-based mirror therapy and cognitive therapeutic exercises on upper extremity functions, congestion, depression and quality of life in stroke individuals. Thirty stroke individuals were included in the study and were randomly divided into two groups as Mirror Therapy (AT) group (n=15) and Cognitive Therapy (CT) group. A total of 24 sessions of AT were applied to the individuals in both groups, in the form of a 1-hour session, 3 days a week, for 8 weeks. Individuals in the KT group were given a cognitive task in addition to AT. Upper extremity functionality of all participants Fugl-Meyer Upper Extremity Motor Assessment (FMA), cognitively Mini-Mental State Test (MMDT), spasticity level Mofiye Ashworth Scale (MAS), activities of daily living Barthel Activity of Daily Living Scale (BDI), quality of life The Stroke-Specific Quality of Life Scale (SSQOL) and depression levels were assessed with the Beck Depression Scale (BDI) at the beginning and end of the study. A statistically significant improvement was found in the FMA, MMDT, BDI, SSQOL, and BDI scales in both groups (p<0.05). In the evaluations between the groups, a statistically significant difference was found in favor of the AT+KT group in the BDI in the group*time evaluation in the MMDT after the treatment (p<0.05). According to the results of our study, cognitive task mirror therapy applied with home-based telerehabilitation method in stroke individuals led to an improvement in the general cognitive status of individuals and increase independence in daily living activities.
