Özet

Bu araştırma, bebeği yenidoğan yoğun bakım ünitesinde (YYBÜ) yatan annelere Dennis'in emzirme öz-yeterlilik kuramına göre verilen hemşirelik bakımının emzirme öz-yeterlilik ve anksiyeteye etkisinin değerlendirilmesi amacıyla yapılmıştır. Araştırma tek merkezli, ön test–son test kontrol gruplu yarı deneysel desende yapılmıştır. Araştırma için gerekli kurum izni ve etik kurul onayı alınmıştır. Örneklemi 30 deney, 30 kontrol grubu olmak üzere toplam 60 anne-bebek çifti oluşturmuştur. Deney grubundaki annelere yoğun bakıma yatışın ilk günü ön testler (Durumluk Kaygı Envanteri [DKE] ve Postpartum Emzirme Öz-Yeterlik Ölçeği [EÖYÖ]) uygulanmıştır. Daha sonra deney grubundaki annelere ve destekçilerine Dennis'in emzirme öz-yeterlilik kuramına göre ortalama 60 dakika (dk) süren tek oturum eğitim verilmiştir. Deney grubundaki annelere 15 gün boyunca her gün, meme uyarısını hatırlatmak amacıyla telefon ile mesaj gönderilmiştir. Destekçilerden ise annelere, süt salınımını arttırmak için destek olması istenmiştir. Araştırmanın son gününde annelere son testler (DKE ve EÖYÖ) uygulanmıştır. Kontrol grubundaki annelere ise araştırma öncesi ön testler ve araştırma sonrası son testler yapılmıştır. Kontrol grubundaki annelere araştırma süresi boyunca herhangi bir girişimde bulunulmamıştır. Bu araştırmada, deney grubundaki annelerin bebeklerine anne sütü dışında verdikleri öğün sayısının kontrol grubundaki annelere göre anlamlı ölçüde düşük olduğu saptanmıştır (p<0,001). Deney grubundaki annelerin son değerlendirme emzirme öz-yeterlilik puanlarının, kontrol grubuna göre anlamlı ölçüde yüksek olduğu belirlenmiştir (p<0,001). Deney grubundaki annelerin son değerlendirme kaygı düzeylerinin, kontrol grubundaki annelere göre daha düşük olduğu bulunmuştur (p<0,001). Araştırma sonuçları bebeği YYBÜ'nde yatan annelere Dennis'in emzirme öz-yeterlilik kuramına göre yapılan hemşirelik bakımının annelerin emzirme öz-yeterlilik düzeyini arttırdığı ve kaygı düzeyini azalttığını göstermektedir.

This study was carried out to evaluate the effect of nursing care given to mothers whose babies are in neonatal intensive care units (NICU) according to Dennis' breastfeeding self-efficacy theory, on breastfeeding self-efficacy and anxiety. The research was carried out in a single-center, semi-experimental pattern with pretest-posttest control group. The necessary institutional permission and ethics committee approval were obtained for the research. The sample consisted of a total of 60 mother-infant couples, 30 of which were in the experimental group and 30 in the control group. Pre-tests (State Anxiety Inventory [SAI] and Postpartum Breastfeeding Self-Efficacy Scale [BSES]) were applied to the mothers in the experimental group on the first day of admission to the intensive care unit. Then, a single-session training was given to the mothers and their supporters in the experimental group, lasting 60 minutes on average, according to Dennis' breastfeeding self-efficacy theory. A phone message was sent to the mothers in the experimental group every day for 15 days to remind them of the breast warning. The supporters were asked to support the mothers in order to increase milk secretion. On the last day of the study, post-tests (SAI and BSES) were applied to the mothers. The mothers in the control group were applied pre-tests before the research and post-tests after the research. No similar intervention was provided for the mothers in the control group during the study period. It was determined that the number of meals given to their babies other than breast milk by the mothers in the experimental group was significantly lower than the mothers in the control group (p<0,001). It was found out that the post-evaluation breastfeeding self-efficacy scores of the mothers in the experimental group were significantly higher than the control group (p<0,001). It was determined that the post-evaluation anxiety scores of the mothers in the experimental group were lower than the mothers in the control group (p<0,001). The results of the study show that nursing care given to mothers whose babies are in NICU according to Dennis' breastfeeding self-efficacy theory, increases the breastfeeding self-efficacy level of the mothers and reduces the level of anxiety.
