Malignant glioma is one of the most aggressive and progressive brain tumors, characterized with considerable therapy resistance and poor prognosis. Despite TMZ's effectiveness in glioma, the recurrence prevention and prognosis improvement are limited, so far is still the most widely used alkylating agent for the last three decades in the treatment of both primary and secondary glioma. The key factor barrier to glioma therapy is chemoresistance development. Recently, combined treatments have become a target to leverage drugs sensitivity and promote the antitumor effect of chemotherapeutic agents. Therefore, in this study two combined treatment strategies have been investigated to improve the therapeutic response of TMZ in glioma tumors. Including Low Level Laser Therapy (LLLT) combined with TMZ, and Electrochemotherapy (ECT). By using in vitro model to analyze the effect of LLLT and ECT induced cell death, LLLT irradiation was applied for 5 days once a day with 785nm wavelength for 7.24min (fluence 200J/cm2) alone and with TMZ. In ECT experiments, variable treatment groups used: control (untreated group), chemotherapy (100µM TMZ), electroporation (cells exposed to 500 Voltage for 1ms, 1 pulse), and electrochemotherapy (cells were exposed to EP and TMZ), ECT performed once followed with 3 days of analysis. While the efficiency of LLLT and ECT were evaluated by cell viability analysis, nucleus and cytoskeleton fluorescence staining, gene and protein expressions tested after 3-5 days treatment. Experiment findings showed that LLLT+TMZ has a time dependent effect on cells viability, in which the cells viability inhibition effect of the 200J/cm2 laser was statically significant, gene and protein expression also confirmed that LLLT and ECT can enhance TMZ therapeutic outcome of glioma by downregulating AKT/MTOR and MDM2 expressions in glioma.
