Özet

Amaç: Lomber radikülopati (LR) tanılı olgularda piriformis kasına ultrasonografi (USG) rehberliğinde yapılan tanısal enjeksiyon testiyle eşlik eden piriformis sendromu (PS) varlığını araştırmak ve bu enjeksiyonların tedavideki etkinliğini değerlendirmek. Gereç ve Yöntemler: Çalışmaya 18 yaş ve üzerinde, tek taraflı bel, kalça, bacak ağrısıyla başvuran, klinik olarak L4/L5/S1 radikülopati bulgularına sahip, manyetik rezonans görüntüleme ve/veya elektrodiagnostik testlerle klinik tablosu desteklenen, aynı taraf piriformis lojunda hassasiyeti olan 39 olgu dahil edildi. Olgular enjeksiyon öncesinde detaylıca değerlendirildi. Tüm olguların piriformis kasına USG rehberliğinde 5 ml %2'lik lidokain enjeksiyonu yapıldı. Enjeksiyon sonrası 30. dakika, 1. hafta ve 1. ayda semptomlardaki azalma yüzdeleri sorgulandı. İstirahat, hareket, ayakta durma, oturma, yatma, gece ağrıları sayısal eşleştirme ölçeği (SEÖ) kullanılarak enjeksiyon öncesi, enjeksiyon sonrası 1. hafta ve 1. ayda sorgulandı. Bulgular: Enjeksiyon sonrası 30. dakikada şikayetlerinde en az %50 azalma bildiren 33 olguya (%84,6) PS tanısı konuldu. Bu olgular LR'ye eşlik eden PS olarak değerlendirildi. Bu olguların enjeksiyon sonrası semptomlarda azalma yüzdesi ortalamalarının 30. dakikada 65,75±15,36, 1. haftada 56,81±31,27, 1. ayda 71,06±31,44 olduğu görüldü. İstirahat ederken, hareket ederken, ayakta dururken, yatarken ve gece SEÖ skorlarında ise enjeksiyon öncesine göre; enjeksiyon sonrası 1. hafta ve 1. ayda, otururken de enjeksiyon sonrası 1. ayda istatistiksel olarak anlamlı düşüş gözlendi (p<0,05). Sonuç: PS, LR'ye eşlik edebilen bir patolojidir. PS tanısı için LR başta olmak üzere lomber patolojilerin dışlanmasına gerek yoktur. Ayrıca LR'ye eşlik eden PS olgularında piriformis kasına USG eşliğinde yapılan lokal anestezik enjeksiyonları, 1 aylık süreçte tedaviye %70'ten fazla katkı sağlamaktadır.

Aim: To investigate the presence of accompanying piriformis syndrome (PS) in patients with lumbar radiculopathy (LR) with diagnostic piriformis muscle injection test performed under the guidance of ultrasonography (USG) and to evaluate the efficacy of these injections in the treatment. Material and Methods: 39 patients aged 18 years or older, having low back and unilateral hip and leg pain, tenderness in the ipsilateral piriformis area and clinical signs of ipsilateral L4/L5/S1 radiculopathy which were supported by magnetic resonance imaging and/or electrodiagnostic tests were included. All the patients received 5 ml of lidocaine 2% into the piriformis muscle under the guidance of USG. The patients whose pain resolved at least 50% from the baseline after the injection were diagnosed as having PS. The percentage of patients' symptom reduction at 30 minutes, 1 week and 1 month after the injection was questioned. Pain at resting, in motion, and at night; and with long duration of standing, sitting and lying was also questioned before and 1 week and 1 month after the injection using the numerical rating scale (NRS). Results: PS was diagnosed in 33 cases with LR (84,6%). The mean percentage of reduction in symptoms after injection was 65,75±15,36 at 30 minutes, 56,81±31,27 at the 1st week and 71,06±31,44 at the 1st month. Compared to the pre-injection scores at resting, in motion, at night, and with long duration of standing and lying; a statistically significant decrease was observed at the 1st week and 1st month after the injection and a statistically significant decrease was observed with sitting 1 month after the injection. (p<0,05). Conclusion: PS can accompany LR. Exclusion of lumbar pathologies, especially LR, is not necessary for the diagnosis of PS. In addition, in patients with PS accompanying LR local anesthetic injections into the piriformis muscle under the guidance of USG can contribute more than 70% improvement in patients' symptoms.
