Özet
Tiroidektomi sonrasında bulantı, kusma görülme oranı %70'lere ulaşabilmektedir. Akupresür, ameliyat sonrası bulantı, kusmanın ve öğürmenin önlenmesi ve/veya yönetiminde hemşirelerin bağımsız olarak uygulayabileceği bir hemşirelik girişimidir. Bu araştırma, tiroidektomi sonrası uygulanan Kore el akupresürünün ameliyat sonrası ağrı, bulantı, kusma ve öğürme sayısı ve şiddeti üzerindeki etkisini belirlemek amacıyla son test-kontrol gruplu deneysel tasarım olarak gerçekleştirildi. Araştırmanın verileri, Şubat 2021 - Haziran 2022 tarihleri arasında toplandı. Araştırma, İstanbul'da bulunan bir şehir hastanesinin genel cerrahi servislerinde elektif tiroidektomi uygulanan ve araştırma kriterlerine uyan 42 hasta (girişim grubu=21, kontrol grubu=21) ile gerçekleştirildi. Girişim grubundaki hastalara anestezi verilmeden 30 dakika önce maş fasulyesi tohumları kullanılarak Kore el akupresürü uygulandı. Ameliyat sonrası her iki saatte bir tohumların üzerinden üç dakikalık masaj yapılarak sekizinci saattin sonunda uygulama sonlandırıldı. Kontrol grubunda yer alan hastalara ameliyat sonrası rutin tedavi ve bakım uygulandı. Girişim ve kontrol gruplarında yer alan hastaların ameliyat sonrası 2., 6. ve 24. saatlerde ağrı şiddeti, bulantı, kusma ve öğürme sayısı ve şiddeti, antiemetik kullanımları, 24. saatin sonunda“Rhodes Bulantı, Kusma ve Öğürme İndeksi”puanı değerlendirildi. The incidence of nausea and vomiting after thyroidectomy can reach 70%. Acupressure is a nursing intervention that nurses can apply independently in the prevention and/or management of postoperative nausea, vomiting and retching. This study was carried out as an experimental design with a post-test-control group to determine the effect of Korean hand acupressure applied after thyroidectomy on postoperative pain, nausea, vomiting, and the number and severity of retching. The data of the study were collected between February 2021 and June 2022. The study was conducted with 42 patients (intervention group=21, control group=21) who underwent elective thyroidectomy in the general surgery wards of a city hospital in Istanbul and met the research criteria. Korean hand acupressure was applied to the patients in the intervention group using mung bean seeds 30 minutes before anesthesia. After the surgery, a three-minute massage was performed on the seeds every two hours, and the application was terminated at the end of the eighth hour. The patients in the control group received routine post-operative treatment and care. Pain severity, number and severity of nausea, vomiting and retching, antiemetic use, and“Rhodes Nausea, Vomiting and Regurgitation Index”score at the end of the 24th hour were evaluated at the postoperative 2nd, 6th and 24th hours of the patients in the intervention and control groups.
