Özet

Amaç: Bu tez çalışmasındaki amacımız, Acil Servis'e başvurusunda COVID- 19 hastalığı tanısı alan hastalara artan VTE riskinden dolayı non-invaziv bir yöntem olan ve yatak başında hızlıca uygulanabilen alt ekstremiteye yönelik iki nokta kompresyon ultrasonografisi ile DVT insidansını saptamak ve Modified Erly Warning Score (MEWS) Kullanarak hastaların progrnozunu öngörmektir. Gereç-Yöntem: Çalışmamız prospektif olarak Sağlık Bilimleri Üniversitesi Antalya Eğitim ve Araştırma Hastanesi Klinik Araştırmalar Etik Kurulu'ndan onayı alındıktan sonra Ocak 2022-Haziran 2022 tarihleri arasında Acil Servis'e başvuran COVID-19 tanısı alan hastalar üzerinde gerçekleştirildi. Hastaların demografik bilgileri, komorbid hastalıkları, vital bulguları, hesaplanan MEWS puanları, Acil Servis'te aldıkları solunum destek tedavisi, hastaların Acil Servis'ten sonlanımları ve hastane içi mortalitesi Çalışma Formu'na kaydedildi. Hastalara en az 3 yıllık USG deneyimine sahip Acil Servis Hekimi tarafından DVT insidansını saptamak amacıyla her iki alt ekstremiteye 2 nokta kompresyon USG uygulandı ve çalışma formuna not edildi. Bulgular: Çalışmaya 312 hasta dahil edildi. Hastaların hastaların %59,6 (n=186)'sı erkek, %40,4 (n=126)'ü kadındı. Hastaların yaş ortalaması 56,617,6 olarak hesaplandı. Çalışma hastalarının MEWS triyaj puanları, 0 puan %16,3 (n=51), 1 puan %26,6 (n=83), 2 puan %31,1 (n=97), 3 puan %17 (n=53), 4 puan % 6,4 (n=20), ve 5 puan %2,6 (n=8) olarak tespit edildi. Çalışmaya alınan hastaların alt ekstremite DVT sıklığı %10,9 (n= 34) olarak bulundu. Çalışma hastalarının DVT pozitifliği ve MEWS arasındaki ilişki incelendi ve MEWS puanı arttıkça hastaların DVT saptanma oranının artması istatiksel olarak anlamlı bulunmuştur (p<0,01). Çalışmaya alınan 312 hastanın hastane içi mortalitesi %21,5 (n=67) olarak hesaplandı. Çalışma hastalarının MEWS puanları ile hastane içi mortalitesi arasındaki ilişki incelendiğinde hastane içi mortalitenin MEWS arttıkça yükselmesi istatiksel olarak anlamlı bulunmuştur (p<0,01). Hastaların hastane içi mortalitesi ve DVT arasındaki ilişki incelendiğinde DVT tanısı alan 34 hastanın 22'si hastane içi takiplerinde exitus oldu. Derin ven trombozu pozitif olarak saptanan hastaların hastane içindeki mortalitesinin artması istatiksel olarak anlamlı bulunmuştur (p<0,01). Sonuç: Bu çalışmada, Acil Servis'e başvuran COVID-19 hastalığına sahip kişilerde iki nokta kompresyon USG ile tespit edilen DVT insidansı %10,9 (n=34) olarak hesaplandı. Hastaların MEWS puanı arttıkça DVT insidansındaki artış ve hastane içi mortalitedeki artış istatiksel olarak anlamlıydı. Derin ven trombozu saptanan hastaların mortalitesinde ve MEWS puanında, DVT saptanmayan hastalara göre anlamlı fark vardı.

Objective: Our aim in this thesis study is to determine the incidence of DVT with two-point compression ultrasonography for the lower extremity, which is a non- invasive method and can be applied quickly at the bedside, in patients diagnosed with COVID-19 disease at the time of admission to the Emergency Department, and to determine the incidence of DVT and the Modified Erly Warning Score (MEWS) to predict the prognosis of patients. Materials and Methods: Our study was conducted prospectively on patients diagnosed with COVID-19 who applied to the Emergency Service between January 2022 and June 2022, after obtaining the approval of the Health Sciences University Antalya Training and Research Hospital Clinical Research Ethics Committee. Demographic information of the patients, comorbid diseases, vital signs, calculated MEWS scores, respiratory support treatment received in the Emergency Service, outcomes of the patients in the Emergency Service and in-hospital mortality were recorded in the Study Form. In order to determine the incidence of DVT, 2-point compression USG was applied to both lower extremities by an Emergency Department Physician with at least 3 years of USG experience and noted in the study form. Results: 312 patients were included in the study. Of the patients, %59,6 (n=186) were male and %40.4 (n=126) were female. The mean age of the patients was calculated as 56.617.6. MEWS triage scores of study patients, 0 points %16,3 (n=51), 1 point %26,6 (n=83), 2 points %31,1 (n=97), 3 points %17 (n=53), 4 points %6,4 (n=20), and 5 points %2,6 (n=8). The frequency of lower extremity DVT in the patients included in the study was found to be %10,9 (n= 34). The relationship between DVT positivity and MEWS in study patients was examined, and it was found that as the MEWS score increased, the rate of DVT detection increased statistically significant (p<0.01). In-hospital mortality of 312 patients included in the study was calculated as %21,5 (n=67). When the relationship between MEWS scores of study patients and in- hospital mortality was examined, it was found that the increase in in-hospital mortality as MEWS increased was statistically significant (p<0.01). When the relationship between the in-hospital mortality of the patients and DVT was examined, 22 of 34 patients diagnosed with DVT were considered dead during their in-hospital follow-up. The increase in in-hospital mortality of patients with positive deep vein thrombosis was found to be statistically significant (p<0.01). Conclusion: In this study, the incidence of DVT detected by two-point compression USG in people with COVID-19 who applied to the Emergency Service was calculated as %10,9 (n=34). As the MEWS score of the patients increased, the increase in the incidence of DVT and the increase in in-hospital mortality were statistically significant. There was a significant difference in mortality and MEWS scores of patients with deep vein thrombosis compared to patients without DVT.
