Özet
GİRİŞ: Canlı vericiden gerçekleştirilen böbrek ve karaciğer transplantasyonlarının sağ kalım sonuçlarının daha iyi olması, kadavra sıralarındaki artmış hasta sayısı, bekleme sürelerinin uzun olması sebebi ile son yıllarda canlı donörden yapılan transplantasyon sayısı artmıştır. Türkiye'de böbrek ve karaciğer nakillerinin çoğunluğu (yaklaşık %70-80) canlı donörden yapılan nakillerdir. Bağış öncesi donör, psikososyal açıdan detaylı bir şekilde değerlendirilmelidir. Bu süreçte; donör adayının gönüllü olduğundan, baskı altında olmadığından, kendisi için karar vermekte yetkin olduğundan emin olunmalıdır. Donör adayına transplantasyon sürecinin herhangi bir zamanında donör olmaktan vazgeçebileceği ifade edilmelidir. Canlı vericiden transplantasyon, psikososyal yönden pozitif ya da negatif sonuçlara sebep olabilen tıbbi boyutunun yanı sıra, etik ve ahlaki açıdan da karmaşık bir durumdur. Bundan dolayı canlı organ vericilerinin psikososyal değerlendirme sürecinin standartlaştırılması, negatif sonuçları da öngörebilime açısından büyük önem taşımaktadır. AMAÇ: Çalışmamızdaki birincil amaç canlı organ donörlerinin psikososyal değerlendirilmesini standardize hale getirebilmek için geliştirilen Canlı Donör Değerlendirme Aracı'nın (CDDA), geçerliliğinin test edilmesidir. İkincil olarak ise böbrek ve karaciğer donörlerinin operasyon sonrası birinci, üçüncü, altıncı ve on ikinci ayda anksiyete, depresyon, yaşam kalitesi açısından incelenmesi amaçlanmaktadır. YÖNTEM: Organ Nakli Koordinatörlüğü'nden, Ege Üniversitesi Hastanesi Ruh Sağlığı Hastalıkları Anabilim Dalı Konsultasyon ve Liyezon Psikiyatrisi (KLP) Polikliniği'ne böbrek veya karaciğer donör adayı olarak yönlendirilen kişilere CDDA bir araştırmacı tarafından uygulanmıştır Katılımcılara tanısal değerlendirme için SCID-5, Hamilton Depresyon Derecelendirme Ölçeği (HDRS), Durumluluk (STAI-S) ve Süreklilik Kaygı Envanteri (STAI-T), Kısa Form (SF-36) uygulanmıştır. Ameliyat geçirmiş kişiler transplantasyon sonrası birinci, üçüncü, altıncı ve on ikinci ayda tekrar görüşmeye çağırılmış, psikososyal izlem soruları sorulmuş, Hamilton Depresyon Derecelendirme Ölçeği araştırmacı tarafından uygulanmış, Durumluluk ve Sürekli Kaygı Envanteri, SF-36 donörler tarafından doldurulmuştur. BULGULAR: Çalışmamıza toplamda 242 katılımcı dahil edilmiş, bunlardan 67'si opere olmuştur. Ölçeğin potansiyel geçerliliğinin gösterilmesi ve CDDA skorları kullanılarak klinik kararın tahmin edilmesi amacıyla lojistik regresyon analizi uygulanmıştır. Model istatistiksel olarak anlamlı bulunmuştur (p<0.001). Modelin varyansın %87,5'ini (Nagelkerke R2) açıkladığı ve vakaların %97,1'ini doğru şekilde sınıflandırdığı saptanmıştır. Donör olmasına onay verilen adayların CDDA puanlarının onay verilmeyenlere göre anlamlı olarak daha yüksek olduğu saptanmıştır (p<0,001). CDDA'nın donörlerin uygun bulunma durumuna göre duyarlılığı ve özgüllüğü, ROC eğrisi ile değerlendirilmiştir. Eğri altında kalan alan 0,992 olarak saptanmıştır. Youden indeksi ile kesme puanı 61,5 olarak belirlendiğinde ise en yüksek duyarlılık (%95,6) ve özgüllük (%92,3) sonuçlarına ulaşılmıştır. Donör olmasına onay verilen adayların tüm SF-36 alt ölçek puanlarının onay verilmeyen adaylara göre anlamlı olarak daha yüksek olduğu (p<0,001); HDRS, STAI-T ve STAI-S puanlarının ise anlamlı olarak daha düşük olduğu saptanmıştır (p<0,001). Donörlerin CDDA puanları ile donörlere transplantasyon sonrası uygulanan ölçeklerin puanları arasındaki korelasyon değerlendirildiğinde; CDDA puanları ile SF-36'nın ruhsal ve sosyal değerlendirme ile ilgili alt ölçekleri olan“sosyal işlevsellik”,“duygusal problemlere bağlı rol kısıtlılıkları”,“ruhsal sağlık”,“sağlığın genel algılanması”puanlarının pozitif korelasyon gösterdiği saptanmıştır. CDDA puanları ile HDRS, STAI-T, STAI-S puanları arasında ise negatif korelasyon saptanmıştır. SF-36 alt ölçek puanları incelendiğinde operasyon sonrası birinci ayda bütün ölçek puanlarında anlamlı azalma görülmüştür. Fiziksel sağlık ile ilgili alt ölçek puanlarında ameliyat öncesi döneme göre ameliyat sonrası 6. aydan itibaren anlamlı fark görülmediği saptanmıştır (p>0,05). Ruhsal sağlık ile ilgili alt ölçek puanlarında ameliyat öncesi döneme göre ameliyat sonrası üçüncü aydan itibaren anlamlı farkın olmadığı bulunmuştur (p>0,05). Çalışmamızda STAI-S puanlarında operasyon öncesine göre operasyon sonrası altıncı ayda anlamlı düşüş saptanmıştır (p=0,01). STAI-T puanlarında preop döneme göre postop birinci ayda anlamlı artış saptanmış (p<0,001), postop üçüncü aydan itibaren ise anlamlı fark saptanmamıştır (p>0,05). HDRS puanlarında ise operasyon öncesi dönemle operasyon sonrasındaki dönemler arasında anlamlı fark bulunmamıştır (p>0,05). Psikososyal izlem sorularına verilen cevapların CDDA puanları ile ilişkisi incelenmiştir. Olumlu ve nötr cevap verenlerin CDDA puanları karşılaştırıldığında anlamlı fark saptanmamıştır (p>0,05). Alıcının sağlık durumunu kötü bulan ve pişmanlık yaşayan birer donörün CDDA puanları 60 ve 65 olarak saptanmıştır. Bedenini aynı veya kötü bulanların CDDA puanları arasında anlamlı fark saptanmamıştır (p>0,05). Psikolojik yardım alanların CDDA puanları anlamlı derecede düşük bulunmuştur (p<0,01). Alıcı ve eş ile ilişkideki değişim ile CDDA puanları arasında anlamlı fark saptanmamıştır (p>0,05). SONUÇ: Çalışmamızda CDDA'nın donör olmaya uygun adayların belirlenmesi ve donörlerin transplantasyon sonrasındaki psikososyal durumunun öngördürücüsü olması açısından geçerli bir test olduğu gösterilmiştir. CDDA puanı yüksek olan donörlerin, düşük olanlara göre postop bir yıllık izlemde depresyon, anksiyete puanlarının daha düşük olduğu, işlevselliklerinin daha iyi olduğu ve psikososyal sonuçlarının daha iyi olduğu saptanmıştır.

Transplantations from living donors has increased in recent years due to the better survival rates of kidney and liver transplantations from living donors, the increased number of patients in cadaver lists, and elongated waiting times. The majority of kidney and liver transplants in Turkey (approximately 70-80%) are transplants from living donors.Before donation, the donor should be evaluated in detail from a psychosocial perspective.In this process, it should be ensured that the donor candidate is voluntary, not under pressure, and is competent to make a decision for herself/himself.It should be stated to the donor candidate that he/she can give up being a donor at any time during the transplantation process. Transplantation from a living donor is a complex situation in terms of ethics and morals, as well as the medical aspect that can cause positive or negative psychosocial outcomes. Therefore, standardizing the psychosocial evaluation process of living organ donors is of great importance in terms of predicting negative outcomes. AIM: The primary aim of our study is to test the validity of the Live Donor Assesment Tool (LDAT) which was developed to standardize the psychosocial evaluation of living organ donors. Secondly, it is aimed to examine kidney and liver donors in terms of anxiety, depression and quality of life in the first, third, sixth and twelfth months after the operation.LDAT was administered by a researcher to parcipitants who were referred to Ege University Hospital Consultation and Liaison Psychiatry (CLP) outpatient clinic from the Organ Transplantation Coordinator as a kidney or liver donor candidate.For diagnostic evaluation, SCID-5, Hamilton Depression Rating Scale, State and Trait Anxiety Inventory, SF-36 were administered to the participants by the researcher. Donors who had undergone surgery were called for re-interview at the first, third, sixth and twelfth months after transplantation, psychosocial follow-up questions were asked, the Hamilton Depression Rating Scale was administered by the researcher, State and Trait Anxiety Inventory and SF-36 were filled by donorS. RESULTS: 242 paticipants were included in our study and 67 of them were operated. Logistic regression analysis was performed in order to demonstrate the potential validity of the scale and to predict clinical decision using CDDA scores. The model was found to be statistically significant (p<0.001). It was found that the model explained 87.5% of the variance (Nagelkerke R2) and correctly classified 97.1% of the cases. The CDDA scores of the candidates who were approved to be donors were found significantly higher than those who were not approved (p<0.001). The sensitivity and specificity of CDDA according to the approval status of the donors were evaluated with the ROC curve. The area under the curve was determined as 0.992. When the cut-off point was determined as 61.5 with Youden index, the highest sensitivity (95.6%) and specificity (92.3%) results were obtained. All SF-36 subscale scores of the candidates who were approved to be donors were found to be significantly higher, HDRS, STAI-T and STAI-S scores were found to be significantly lower than the candidates who were not approved (p<0.001). The correlation between the CDDA scores of the donors and the scores of the scales applied to the donors after transplantation was evaluated. It was determined that there was a positive correlation between CDDA scores and SF-36's subscales related to mental and social assessment (“social functionality”,“role limitations due to emotional problems”,“mental health”,“general perception of health”). A negative correlation was found between LDAT scores and HDRS, STAI-T, and STAI-S scores.When the SF-36 subscale scores were examined, a significant decrease was observed in all scale scores in the first month after the operation. It was determined that there was no significant difference in physical health-related subscale scores from the sixth month after the surgery compared to the preoperative period (p>0.05). It was found that there was no significant difference in the subscale scores related to mental health from the third month after the surgery compared to the preoperative period (p>0.05).When the SF-36 subscale scores were examined, a significant decrease was found in all subscale scores in the first month after the operation.It was determined that there was no significant difference in physical health-related subscale scores from the sixth month after the surgery compared to the preoperative period (p>0.05).It was found that there was no significant difference in the subscale scores related to mental health from the third month after the surgery compared to the preoperative period (p>0.05). In our study, a significant decrease was found in the STAI-S scores in the sixth month after the operation compared to the preoperative period (p=0.01). There was a significant increase in STAI-T scores in the postoperative 1st month compared to the preoperative period (p<0.001), but there was no significant difference from the postoperative third month (p>0.05). On the other hand, there was no significant difference between the pre-operative and post-operative periods in HDRS scores (p>0.05). The relationship between the answers given to the psychosocial follow-up questions and the LDATscores of the donors was examined. When LDATscores of positive and neutral responders were compared, no significant difference was found (p>0.05). The LDATscores of two donors who found the recipient's health status to be bad and regretted were determined as 60 and 65.No significant difference was found between LDATscores of those who found their body the same or bad (p>0.05). LDATscores of those who received psychological help were found to be significantly lower (p<0.01). There was no significant difference between the LDAT scores and thechange in the relationship with the recipient and spouse (p>0.05). RESULTS: In our study, it was shown that CDDA is a valid test in terms of identifying suitable donors and predicting the psychosocial outcomes of donors after transplantation. It was determined that donors with high CDDA scores had lower depression and anxiety scores, better functionality and better psychosocial outcomes in the postoperative one-year follow-up compared to those with low CDDA scores.
