Özet
Human papillomavirus (HPV) dünyada serviks kanserlerinden sorumlu en yaygın viral patojendir. HPV enfeksiyonun pozitiflik oranlarının ve genotip dağılımının değerlendirilmesi, serviks kanserine yönelik önleyici stratejilerin benimsenmesinde yol gösterici olabilir. Bu çalışmada Ankara Etlik Şehir Hastanesi Kadın Hastalıkları ve Doğum Poliklinikleri'ne başvuran kadınlar arasında HPV enfeksiyonunun pozitiflik oranları ve yaşa göre dağılımının değerlendirilmesi, genotip dağılımının araştırılması, servikal smear sitolojileri ile ilişkilerinin değerlendirilmesi ve bölgesel epidemiyolojik veri elde edilmesi amaçlanmıştır. Gereç ve Yöntem Bu retrospektif çalışmaya 1 Ekim 2022-22 Mart 2023 tarihleri arasında, Ankara Etlik Şehir Hastanesi Kadın hastalıkları ve Doğum Polikliniği'ne başvuran ve HPV DNA genotipleme testi yapılan 18 yaş ve üzeri 3510 kadın dahil edildi. Servikal smear örneklerinde HPV tespiti ve genotipleme, Cobas 4800 Sistemi (Roche Molecular Diagnostics, Almanya) kullanılarak real-time PCR yöntemiyle analiz edildi. Pap smear sitolojik değerlendirmesi uzman patologlar tarafından 2014 Bethesda skorlama sistemine göre yapıldı. Bulgular Genel yüksek riskli HPV (hrHPV) enfeksiyon pozitifliği %10 (%95 GA %9,1–11,1) bulundu. En yaygın hrHPV genotipi sırasıyla diğer hrHPV (%69,7), HPV 16 (%23,2), HPV 18 (%7,1) olarak saptandı. Çoklu hrHPV pozitiflik oranı %13,6 idi. HPV 16 ve diğer hrHPV (%68,7) koenfeksiyonu, en yaygın çoklu enfeksiyondu. En yüksek hrHPV pozitifliği 18–24 yaş grubunda (%28,7) gözlendi (p <0,001). Tüm yaş gruplarında da diğer hrHPV en yaygın saptanan genotipti. HPV 16 ve HPV 18 için en yüksek pozitiflik oranı 18−24 yaş grubunda olup sırasıyla %8,9 (9/101) ve %5 (5/101) olarak bulundu (her ikisi için p<0,001). Yaş gruplarına göre tek enfeksiyon ve çoklu enfeksiyon en fazla 18–24 yaş grubunda sırasıyla %21,8 (22/101) ve %6,9 (7/101) tespit edildi (p<0,001). HPV enfeksiyonları anormal sitolojiye sahip hastalar arasında (93/221, %42,1) daha yüksekti (p<0,001). HPV pozitifliği en yüksek LSIL (24/31, %77,4) sitolojisinde tespit edildi (p<0,001). HPV 16, 18 ve diğer hrHPV pozitiflik oranları normal sitolojide sırasıyla %1,9; %0,5 ve %6,4 iken, anormal sitolojide sırasıyla %14,9; %5 ve %32,1 olarak gözlendi (her üçü için, p<0,001). Çoklu enfeksiyon, anormal sitoloji sonucu olan hastalarda (19/93, %20,4) daha yüksek bulundu (p=0,03). Tüm HPV tipleri ile enfeksiyon varlığının, HPV negatiflere göre sitoloji skoru 3 ve üzeri için artmış rölatif risk oluşturduğu belirlendi. Sonuç Bu çalışmada hrHPV pozitifliği %10 bulundu. Diğer hrHPV en sık saptanırken, bunu HPV 16 takip etti. Bu veriler dünya genelinde literatürle uyumluydu. Çalışmamızda anormal sitoloji saptanmış hastalarda diğer hrHPV pozitiflik oranlarının yüksek olması nedeniyle tüm hrHPV tiplerinin ayrı ayrı belirlendiği bölgesel çalışmaların yapılmasının, HPV 16 ve HPV 18 dışındaki tiplerle enfeksiyonun doğal seyrinin takip edilmesi ve aşı çalışmaları için yarar sağlayacağı düşüncesindeyiz.

Human papillomavirus (HPV) is the most common viral pathogen responsible for cervical cancers worldwide. Evaluating the positivity rates and genotype distribution of HPV infection can provide guidance for the adoption of preventive strategies against cervical cancer. This study aims to assess the positivity rates of HPV infection and its distribution by age among women attending the Ankara Etlik City Hospital Gynecology and Obstetrics Clinics, investigate the genotype distribution, evaluate their relationship with cervical smear cytologies, and obtain regional epidemiological data. Materials and Methods This retrospective study included a total of 3510 women aged 18 and above who presented to the Ankara Etlik City Hospital Gynecology and Obstetrics Clinics and underwent HPV DNA genotyping testing between October 1, 2022, and March 22, 2023. HPV detection and genotyping in cervical smear samples were analyzed using the Cobas 4800 System (Roche Molecular Diagnostics, Germany) with the real-time PCR method. Cytological evaluation of Pap smears was performed by expert pathologists according to the 2014 Bethesda scoring system. Results The overall positivity rate of hrHPV infection was found to be 10% (95% CI 9.1–11.1) in this study. The most common hrHPV genotypes were found to be other hrHPV (69.7%), HPV 16 (23.2%), and HPV 18 (7.1%), in that order. The rate of multiple hrHPV positivity was 13.6%. Co-infection of HPV 16 and other hrHPV (68.7%) was the most common multiple infection. The highest hrHPV positivity was observed in the 18–24 age group (28.7%) (p < 0.001). Other hrHPV was the most detected genotype in all age groups. The highest positivity rates for HPV 16 and HPV 18 were in the 18–24 age group, with 8.9% (9/101) and 5% (5/101), respectively (p < 0.001) for both. Single infection and multiple infection rates by age group were highest in the 18–24 age group, at 21.8% (22/101) and 6.9% (7/101), respectively (p < 0.001). HPV infections were higher among patients with abnormal cytology (93/221, 42.1%) (p < 0.001). The positivity rates of HPV 16, 18, and other high-risk HPV types were observed as follows in normal cytology: 1.9%, 0.5%, and 6.4%, respectively, while in abnormal cytology, they were observed as 14.9%, 5%, and 32.1%, respectively (for all three, p<0.001). Multiple infections were higher in patients with abnormal cytology (19/93, 20.4%) (p = 0.03). Presence of infection with all HPV types was found to be associated with an increased relative risk for a cytology score of 3 and above compared to HPV negative individuals. Conclusion In this study, a highrisk human papillomavirus (hrHPV) positivity rate of 10% was found. While other hrHPV types were detected most frequently, HPV 16 followed closely. These data were consistent with the literature worldwide. Given the high rates of other hrHPV positivity in patients with abnormal cytology in our study, we believe that conducting regional studies to determine each hrHPV type separately could be beneficial for monitoring the natural course of infection with types other than HPV 16 and HPV 18. It could also contribute to vaccine research efforts.
