Özet

Amaç: Miyofasiyal ağrı sendromunda proloterapi enjeksiyon tedavisi ile lokal anestezik (lidokain) enjeksiyon tedavisinin etkinliklerini karşılaştırılarak araştırmaktır. Gereç ve Yöntem: Prospektif randomize kontrollü olarak planlanan bu çalışma için SBÜ İstanbul Fizik Tedavi ve Rehabilitasyon SUAM, Fiziksel Tıp ve Rehabilitasyon Polikliniği'ne ayaktan başvuran ve dahil edilme kriterlerine uyan gönüllüler çalışmaya alındı. Çalışmaya gönüllü olan katılımcılar permütasyon blok randomizasyon ile çalışma grubu ve kontrol gruplarına ayrıldı. Toplam 36 hasta çalışmaya alındı. 6 hasta kendi isteği ile çalışmaya devam etmedi. Bir hasta advers olay nedeniyle çalışma dışında bırakıldı. Bu çalışma, çalışma grubu 15 hasta, kontrol grubu 14 hasta olacak şekilde tamamlandı. Üst trapez kasında tetik nokta saptanan hastalara aynı enjeksiyon yöntemi uygulandı. Çalışma grubuna %15 dekstroz enjeksiyon tedavisi uygulandı. Kontrol grubuna %1 lidokain enjeksiyon tedavisi uygulandı. Tetik noktalara toplamda 3 seans (0,2.ve 4. haftada) enjeksiyon tedavisi uygulandı. Her enjeksiyon seansı arası 2 hafta olacak şekilde planlandı. Vas, Bae ve At ölçümleri başlangıçta (0.hafta), 2-4-6-8. haftalarda toplam 5 kez yapıldı. SF-36 ölçümü ise başlangıçta ve 8. haftada olmak üzere 2 defa yapıldı. Bulgular: Her iki tedavi grubunda da AT ile değerlendirilen ağrı şiddeti anlamlı olarak azaldı Tedavi gruplarına göre AT tekrarlı ölçümlerinin karşılaştırılmasında istatistiksel olarak anlamlı fark saptanmadı (p>0,05). Grup içi tekrarlı ölçümlerin karşılaştırmalarında her iki grupta da BAE ölçümleri açısından istatistiksel olarak anlamlı fark saptandı Tedavi gruplarına göre BAE tekrarlı ölçümlerinin karşılaştırılmasında tedavi öncesi ve üçüncü ölçümlerde PRL grubu lehine tatistiksel olarak anlamlı fark saptandı (p<0,05). Tedavi gruplarına göre VAS tekrarlı ölçümlerinin karşılaştırılmasında son kontrol ölçümünde PRL grubu lehine istatistiksel olarak anlamlı fark saptandı (p<0,05). LA grubu PRL grubuna göre tedavi öncesi SF-36 formunun karşılaştırılmasında sosyal fonksiyon boyutunda istatistiksel olarak anlamlı fark saptandı(p<0,05). Tedavi gruplarına göre tedavi sonrası SF-36 formunun karşılaştırılmasında istatistiksel olarak anlamlı fark saptanmadı (p>0,05). PRL grubunun tedavi öncesi ve sonrası SF-36 formunun karşılaştırılmasında; fiziksel problemler nedeniyle olan kısıtlanma, sosyal fonksiyon ve vücut ağrısı boyutlarında istatistiksel olarak anlamlı fark saptandı (p<0,05). LA grubunun tedavi öncesi ve sonrası SF-36 formunun karşılaştırılmasında; emosyonel iyilik hali ve vücut ağrısı boyutlarında istatistiksel olarak anlamlı fark saptandı (p<0,05). Sonuç: Bizim çalışmamızda elde ettiğimiz verilere göre MAS'ta proloterapi ve lidokain enjeksiyon tedavileri etkin tedavilerdir. Proloterapi tedavisi 3. enjeksiyon kontrol ölçümünde ağrı eşiği bakımından, son kontrol ölçümünde ağrı bakımından lidokain enjeksiyon tedavisinden daha etkin bulunmuşur. Proloterapi tedavisi diğer ölçümlerde ise lidokain enjeksiyon tedavisi kadar etkili bulunmuştur. Proloterapi tedavisi; kolay uygulanabilir, ucuz ve yan etki profili açısından güvenilir olan etkin bir tedavi yöntemi olarak MAS'da kullanılabilir.

ABSTRACT COMPARISON OF THE EFFECTIVENESS OF PROLOTHERAPY INJECTION TREATMENT AND LOCAL ANESTHETIC (LIDOCAINE) INJECTION TREATMENT IN MYOFASCIAL PAIN SYNDROME: A PROSPECTIVE RANDOMIZED CONTROLLED RESEARCH Aim: The aim of this study is to Comparison of the effectiveness of prolotherapy injection treatment and local anesthetic (lidocaine) injection treatment in myofascial pain syndrome Materials and Methods: For this prospective randomized controlled study, volunteers who applied to SBU Istanbul Physical Therapy and Rehabilitation SUAM, Physical Medicine and Rehabilitation Polyclinic as outpatients and met the inclusion criteria were included in the study. Participants who volunteered for the study were divided into study group and control groups by permutation block randomization. A total of 36 patients were included in the study. 6 patients did not continue working voluntarily. One patient was excluded from the study due to an adverse event. This study was completed with 15 patients in the study group and 14 patients in the control group. The same injection method was applied to patients with trigger points detected in the upper trapezius muscle. 15% dextrose injection treatment was applied to the study group. The control group received 1% lidocaine injection treatment. A total of 3 sessions of injection treatment (at 0, 2 and 4 weeks) were applied to trigger points. Each injection session was planned to be 2 weeks apart. Vas, Bae and At measurements were taken at baseline (week 0), 2-4-6-8. It was done 5 times in total in weeks. SF-36 measurement was performed twice, at the beginning and at the 8th week. Results: Pain intensity assessed by AT decreased significantly in both treatment groups. No statistically significant difference was detected in the comparison of repeated AT measurements according to treatment groups (p>0.05). In the comparison of repeated measurements within the group, a statistically significant difference was detected in terms of UAE measurements in both groups. In the comparison of repeated measurements of UAE according to the treatment groups, a statistically significant difference was detected in favor of the PRL group in the pre-treatment and third measurements (p<0.05). In the comparison of VAS repeated measurements according to treatment groups, a statistically significant difference was detected in favor of the PRL group at the last control measurement (p<0.05). When comparing the pre-treatment SF-36 form of the LA group with the PRL group, a statistically significant difference was detected in the social function dimension (p <0.05). No statistically significant difference was detected when comparing the post-treatment SF-36 form according to treatment groups (p>0.05). In comparing the SF-36 form of the PRL group before and after treatment; A statistically significant difference was found in the dimensions of limitation due to physical problems, social function and body pain (p <0.05). Comparing the SF-36 form of the LA group before and after treatment; A statistically significant difference was found in emotional well-being and body pain dimensions (p<0.05). Conclusion: According to the data we obtained in our study, prolotherapy and lidocaine injection treatments are effective treatments for MAS. Prolotherapy treatment was found to be more effective than lidocaine injection treatment in terms of pain threshold in the third injection control measurement and in terms of pain in the last control measurement. Prolotherapy treatment was found to be as effective as lidocaine injection treatment in other measurements. Prolotherapy treatment; It can be used in MAS as an effective treatment method that is easily applicable, inexpensive and reliable in terms of its side effect profile.
