Özet
Bu çalışmanın amacı şeffaf plaklar ve sabit ortodontik apareyler ile tedavi olan hastaların DMFT indeksi ve beyaz nokta lezyonu (BNL) değişimlerinin kontrol grubu ile karşılaştırılarak incelenmesidir. Çalışmamız şeffaf plaklar ile tedavi olan, sabit ortodontik tedavi gören ve hiçbir ortodontik tedavi görmeyen toplam 120 katılımcı ile prospektif olarak yürütülmüştür. Hastalarda başlangıç ve tedavi bitiminde (12.9 ±2.7 ay), kontrol grubunda ise başlangıçta ve 12. ayda klinik muayene yapılmış ardından rutin kayıtlar alınmıştır. Alınan kayıtlar ile muayene bulguları beraber değerlendirilerek DMFT indeksi ve beyaz nokta lezyonu teşhisleri yapılmıştır. Elde edilen verilerin tedavi gruplarına ve cinsiyete göre farklılıkları değerlendirilmiştir. Tedavi sonrası sonuçlara bakıldığında, en yüksek DMFT değeri sabit tedavi grubunda bulunmuştur. Fakat bu sonuç istatistiksel olarak anlamlı değildir. Sabit tedavi grubunda DMFT indeksinin tedavi öncesi ve sonrası değerlerinin karşılaştırılmasının istatistiksel olarak anlamlı olduğu bulunmuştur. Bu grupta tedavi sonrası ortalama DMFT değeri artışı daha yüksektir. Çalışmaya dahil edilen her üç gruptaki katılımcılar cinsiyetlerinin, DMFT indeksine etkileri anlamlı değildir. Çalışma sonrası hem şeffaf plak grubunda hem de sabit tedavi grubunda BNL görülme sıklığı artmıştır. Ancak bu artış sabit tedavi grubunda çok daha fazladır. Cinsiyet farkının BNL oluşumunda etkisi olmadığı görülmüştür. Çalışmamızın sonucu olarak beyaz nokta lezyonu oluşumu ve çürük gelişimi açısından riskli olduğu düşünülen hastaların tedavisinde şeffaf plakların kullanılması önerilebilir. Fakat maliyet ve diğer özellikler de göz önünde bulundurulmalıdır.

The aim of this study is to evaluate the changes in DMFT index and white spot lesion formation of patients undergoing treatment with clear aligners and fixed orthodontic appliances by comparing them with the control group. The study was conducted prospectively with a total of 120 participants, including those undergoing treatment with clear aligners, fixed orthodontic treatment, and those not undergoing any orthodontic treatment. The patients, clinical examinations were performed at the treatment initiation and at the end of treatment (12.9 ± 2.7 months), while in the control group, the clinical examinations were conducted at the study onset and at 12 months, followed by routine records collection. The DMFT index and white spot lesion diagnoses were made by evaluating the collected records together with the examination findings. The differences in the obtained data were evaluated based on treatment groups and gender. When analyzing the results post-treatment, the highest DMFT value was found in the fixed treatment group. However, this result is not statistically significant. It has been found that the comparison of DMFT index values before and after treatment in the fixed treatment group is statistically significant. In this group, the increase in mean DMFT value post-treatment is higher. The gender of participants in all three groups included in the study does not have a significant effect on the DMFT index. Following the study, the frequency of white spot lesion occurrence increased both in the clear aligner group and in the fixed treatment group. However, this increase is much higher in the fixed treatment group. Gender difference was observed to have no effect on WSL formation. As a result of our study, the use of clear aligners may be recommended for the treatment of patients considered at risk for white spot lesion formation and caries development. However, cost and other factors should also be taken into consideration.
