Özet

Tinnitus, harici bir ses kaynağı olmaksızın hasta tarafından algılanan işitsel bir duyum hissini ifade eder. Tinnitus nedeniyle işitsel yolun birçok seviyesinde işitsel işlemleme süreçleri etkilenebilmektedir. Tinnituslu bireylerde, gürültüde konuşmayı anlamada zorluk, işitsel spektral, zamansal (temporal) ve eşiküstü işitsel işlemlemenin etkileri literatürde belirtilmektedir. Subjektif tinnituslu bireylerde zamansal ve eşiküstü işitsel işlemleme becerilerini bütüncül olarak birlikte değerlendiren bir çalışmaya literatürde rastlanılmamıştır. Bu nedenle çalışmamızın birinci amacı; subjektif tinnituslu bireylerin zamansal ve eşiküstü işitsel işlemleme becerilerinin değerlendirmesidir. Transkutanöz Elektriksel Sinir Stimülasyonu (TENS), uzun yıllardır tinnitus tedavisi için alternatif bir yöntem olarak kullanılmaktadır. Çalışmamızın ikinci amacı; TENS uygulama öncesi-sonrası zamansal ve eşiküstü işitsel işlemleme becerileri arasındaki değişimin karşılaştırılmasıdır. Bu amaçlar doğrultusunda subjektif tinnitusu olan 34 birey (çalışma grubu), tinnituslu katılımcılar ile aynı yaş ve cinsiyette tinnitusu olmayan 34 birey (kontrol grubu) olacak şekilde 18-55 yaş aralığında toplam 68 katılımcı dâhil edilmiştir. Subjektif tinnituslu bireylerin zamansal ve eşiküstü işitsel işlemleme becerilerini değerlendirmek üzere eşiküstü psikoakustik deneyler uygulanmıştır. Zamansal işitsel işlemleme becerileri için, Intensity Discrimination Test (IDT), Frequency Discrimination Test (FDT), Gap Detection Test (GDT), Modulation Detection Test (MDT), Backward Binaural Unmasking (BBU), Frequency Pattern Test (FPT) ve Duration Pattern Test (DPT); eşiküstü işitsel işlemleme becerileri için, Temporal Fine Structure-Adaptive Frequency (TFS-AF) ve Temporal Envelope (TE) testleri uygulanmıştır. Ayrıca çalışma grubunda bulunan ve aralarında rastgele seçim yapılan 11 bireye (TENS grubu), TENS tedavisi uygulaması sonrasında zamansal ve eşiküstü işitsel işlemleme becerileri tekrardan değerlendirilmiş ve tedavi sonrasında psikoakustik deney sonuçlarının değişiminin istatiksel olarak karşılaştırılması yapılmıştır. Zamansal işitsel işlemleme becerileri incelendiğinde; gruplar arasında zamansal ayırt etme (FDT), zamansal çözünürlük (MDT ve GDT) ve zamansal maskeleme (BBU) testleri skorlarında anlamlı fark elde edilmiş (p<0,05), zamansal ayırt etme becerileri dışında subjektif tinnituslu bireylerin skorları kötü bulunmuştur. Eşiküstü işitsel işlemleme becerileri incelendiğinde; TFS-AF ve TE testleri skorlarında anlamlı fark elde edilmiş (p<0,05) ve subjektif tinnituslu bireylerin skorları kötü bulunmuştur. TENS uygulaması sonrasında zamansal ayırt etme becerilerinde (FDT) uygulama öncesi ve sonrası arasında test skorlarında anlamlı fark elde edilmiştir (p<0,05). Uygulama öncesi ve sonrası arasında diğer test skorlarındaki farkların anlamlı olmadığı bulunmuştur (p>0,05). Sonuç olarak zamansal ve eşiküstü işitsel işlemleme becerilerinin değerlendirilmesine yönelik yapılan psikoakustik testlerde, subjektif tinnitusu olan bireyler genel anlamda düşük performans göstermiş ve tinnitusu olmayan bireyler ile aralarında anlamlı farklılıklar elde edilmiştir. Subjektif tinnituslu bireyler TENS uygulaması kapsamında, uygulama sonrasında zamansal ve eşiküstü işitsel işlemleme becerilerinde yüksek performans göstermiş fakat uygulama öncesine göre anlamlı farklılıklar görülmemiştir. Bu çalışma subjektif tinnitusu olan bireylerde zamansal ve eşiküstü işitsel işlemleme performanslarının değerlendirilmesinin gerekliliğini bütüncül yaklaşım ile vurgulayan bir ön çalışma niteliğindedir. Çalışmamızdan elde edilen bulguların, uygun zamansal ve eşiküstü işitsel işlemleme becerilerine yönelik değerlendirmelerin/tedavilerin planlanması ve uygulanmasında yol gösterici olabileceği düşünülmektedir.

Tinnitus refers to the perception of an auditory sensation without an external sound source. Auditory processing processes can be affected at many levels of the auditory pathway due to tinnitus. In individuals with tinnitus, difficulties speech recognition in noise, and the effects on auditory spectral, temporal, and suprathreshold auditory processing are reported in the literature. No study has been found in the literature that holistically evaluates temporal and suprathreshold auditory processing abilities in individuals with subjective tinnitus. Therefore, the first aim of our study is to evaluation of temporal and suprathreshold auditory processing abilities of individuals with subjective tinnitus. Transcutaneous Electrical Nerve Stimulation (TENS) has been used as an alternative method for tinnitus treatment for many years. The second aim of our study is to compare the changes between temporal and suprathreshold auditory processing abilities before and after TENS application. For these purposes, a total of 68 participants between the ages of 18-55 were included, including 34 individuals with subjective tinnitus (study group) and 34 individuals without tinnitus of the same age and gender as the participants with tinnitus (control group). To evaluate the temporal and suprathreshold auditory processing abilities of individuals with subjective tinnitus, suprathreshold psychoacoustic experiments were applied. For temporal auditory processing abilities, Intensity Discrimination Test (IDT), Frequency Discrimination Test (FDT), Gap Detection Test (GDT), Modulation Detection Test (MDT), Backward Binaural Unmasking (BBU), Frequency Pattern Test (FPT), and Duration Pattern Test (DPT) were applied. For suprathreshold auditory processing abilities, Temporal Fine Structure-Adaptive Frequency (TFS-AF) and Temporal Envelope (TE) tests were applied. In addition, the temporal and suprathreshold auditory processing abilities of 11 randomly selected individuals in the study group (TENS group) were re-evaluated after TENS treatment and the changes in the psychoacoustic test results after the treatment were compared statistically. When temporal auditory processing abilities are examined; significant differences were found between the groups in the scores of temporal discrimination (FDT), temporal resolution (MDT and GDT) and temporal masking (BBU) tests (p<0,05). Except for temporal discrimination abilities, the scores of individuals with subjective tinnitus were found to be poor. When suprathreshold auditory processing abilities are examined; significant differences were found in TFS-AF and TE test scores (p<0,05) and the scores of individuals with subjective tinnitus were found to be poor. Post-TENS application, a significant difference was found in temporal discrimination abilities (PDT) test scores between before and after application (p<0,05). It was found that the differences in other test scores between before and after the application were not significant (p>0,05). As a result, in psychoacoustic tests performed to evaluate temporal and suprathreshold auditory processing abilities, individuals with subjective tinnitus showed generally poor performance and significant differences were obtained between them and individuals without tinnitus. Within the scope of TENS application, individuals with subjective tinnitus showed high performance in temporal and suprathreshold auditory processing abilities after the application, but no significant differences were observed compared to before the application. This study is a preliminary study that emphasizes the necessity of evaluating temporal and suprathreshold auditory processing performances in individuals with subjective tinnitus with a holistic approach. The findings from our study are thought to guide the planning and implementation of appropriate assessments/treatments for temporal and suprathreshold auditory processing abilities.
